Diatom-Positive Cadaver: Drowning or Homicide?

Medico-legal investigations should be performed on all unnatural (homicide, suicide, or accident), unexpected, and suspicious deaths to evaluate the possibility of homicide and ascertain the exact cause of death. However, in some scenarios, burial takes place before an autopsy can be conducted. In such cases, exhumation is performed, which involves excavating the remains of previously buried or cremated individuals for medico-legal investigations. Although the diatom test is a very useful microbiological approach in concluding death by drowning, its reliability remains controversial. Our study…

Prevention of needless deaths from drowning.

OBJECTIVE: To determine whether faulty pool maintenance and substandard lifeguard performance critically delayed retrieval and resuscitation of a significant number of pool drowning victims. METHODS: One hundred and eighty drowning incidents that resulted in litigation from 1998 to 2008 were studied to determine whether faulty pool maintenance and/or substandard lifeguard performance delayed retrieval and thereby contributed to the death of these persons. A total of 180 swimming pools-commercial and private-were included. Ninety-seven of these pools were manned by lifeguards. Subjects who und…

Fatal drowning accident and undiagnosed hydrocephalus.

Bodies found in water present several diagnostic challenges for the forensic examiner, such as the identification of the time and mechanism of death, the postmortem submersion time, and above all the circumstances of drowning. We report the case of a 33-year-old security guard found dead in Venice, in the water of the lagoon, who at autopsy presented a previously undiagnosed hydrocephalus with macrocephaly. The victim remained asymptomatic until 2 months before death and had never undergone a neurological or radiological examination. In the article, we emphasize the need to perform a thorough …

Scedosporium apiospermum infection after near-drowning.

Scedosporium apiospermum and its teleomorph (sexual form) Pseudallescheria boydii are ubiquitous saprophytic fungi, which under specific conditions, such as near-drowning, may cause therapy-refractory and life-threatening infections. We reviewed 22 cases (eight children and 14 adults) of S. apiospermum infection after near-drowning reported in the literature including an additional paediatric case from our institution. Scedosporiosis after near-drowning was associated with high mortality (16/23, 70%) even in immunocompetent hosts. It affected mainly young (mean age 24 years) and immunocompeten…

Hypercalcemia complicating an industrial near-drowning.

A 28-year-old man presented with lethargy, solmulence, and polyuria following near-drowning in a vessel of an offshore oil rig. Laboratory evaluation demonstrated severe hypercalcemia that responded to saline diuresis and nasogastric suctioning. Calcium salts are used frequently in the drilling and completion of oil wells, and it is presumed that this patient's hypercalcemia represented acute intoxication from swallowed and aspirated fluid. This case highlights the need to consider the potential constituents of the drowning fluid in victims of near-drowning, particularly if unexplained clinica…

Neisseria mucosus septicemia after near-drowning.

Neisseria mucosus is a normal inhabitant of the upper respiratory tract which may rarely cause serious infection. A 21-year-old woman had N mucosus bacteremia and the adult respiratory distress syndrome (ARDS) after a near-drowning episode. Despite appropriate antibiotic therapy and intensive respiratory support, the patient died. Bacteremia with this organism is rare, and we believe it has not previously been reported in association with near-drowning.

Invasive pulmonary aspergillosis after near-drowning.

After near-drowning following a car accident, a 27-year-old man developed severe ARDS. Six days later Aspergillus fumigatus was isolated in his sputum, and invasive pulmonary aspergillosis developed thereafter. Aspergillus titre increased, and chest tomograms revealed cavities in both lungs. The treatment consisted essentially of Amphotericin B and 5 fluorocytosine, intravenously and by inhalation, intensive postural drainage and mechanical ventilation with PEEP. After 41 days he was cured and discharged from the ICU. Six months later he returned to his job and clinical examination was normal.

Fifteen years of child drowning--a 1967-1981 analysis of all fatal cases from the Brisbane Drowning Study and an 11 year study of consecutive near-drowning cases.

A total population study of childhood fresh water drowning accidents (fatalities) for the 15 year period, 1967-1981, is reported. These data are from the ongoing Brisbane Drowning Study which has now also analysed 255 fresh water child immersions (both fatalities and near-fatalities) over the eleven year period, 1978-1981, and as such forms a consecutive unselected series for over one decade. The annual fatality (drowning) rate is 3.53 per 100,000. Details of immersion accidents by site, sex and by outcome (survivors versus fatalities) are presented. An analysis of secular trends revealed that…

Impact of lifeguard oxygen therapy on the resuscitation of drowning victims: Results from an Utstein Style for Drowning Study.

INTRODUCTION: No published evidence was identified regarding the use of oxygen in the treatment of drowning in two recent systematic reviews. The aim of our study was to investigate the impact of on scene, pre-Emergency Medical Services (EMS) oxygen therapy by lifeguards in the resuscitation of drowning victims. METHOD: We conducted a retrospective case match analysis of drowning patients presenting to the EDs of Sunshine Coast Hospital and Health Service. Patients were matched for age, sex and severity of drowning injury. The primary outcome was in-hospital mortality. Secondary outcomes incl…

Drowning and near-drowning in the Australian Capital Territory: a five-year total population study of immersion accidents.

A total population study of all serious immersion accidents is reported from the Australian Capital Territory. The annual immersion accident rate of 4-69 per 100,000 of the population at risk is low, and the fresh water fatality rate of 2-58 per 100,000 is very low. The annual fatality rate for childhood swimming pool accidents is 0-34 per 100,000 children (that is, under 16 years of age) at risk. Serious immersion accidents are not increasing in frequency, in striking contrast to the epidemic trends seen from other centres. Factors which might explain this are discussed.

Ten-year study of pediatric drownings and near-drownings in King County, Washington: lessons in injury prevention.

The factors associated with submersion events among less than 20-year-old persons that occurred in King County from 1974 to 1983 were studied to focus prevention efforts. Near-drowning (n = 103) and drowning (n = 96) victims were identified from medical examiners' reports, paramedics' reports, and hospital discharge registers. Annual incidence was 5.5; the mortality rate was 2.6 per 100,000 children. Although preschool-aged children had the largest incidence (12.8), followed by older adolescents (4.9), adolescents had the largest case fatality rate, 77%. Lake and river victims had the largest …

Drowning of a patient with epilepsy while showering.

In Japan, because the most common site of drowning among patients with epilepsy is the bathtub, showering is generally recommended as an alternative to bathing. We herein report a case involving a female patient with epilepsy who drowned while showering. She had been diagnosed with epilepsy approximately 25 years previously, and her condition had progressed to refractory epilepsy. Carbamazepine, levetiracetam, lamotrigine, clobazam, and perampanel were prescribed daily. One day while showering, the patient was found lying with her face immersed in water that had accumulated on the floor of the…

Systematic review of definitions for drowning incidents.

OBJECTIVES: In preparation for the World Congress on Drowning uniform reporting consensus document of drowning incidents we reviewed systematically the medical literature for the terms and definitions used to describe drowning incidents to assess the uniformity of these terms in the medical literature. METHODS: The search strategy included a literature search of PubMed, MEDLINE and the Cochrane Database from 1966 to April 2002, as well as a review of reference lists of identified studies and a hand search of relevant textbooks and reference works. Search terms used included drowning, near-dro…

[Place of pathology in the forensic diagnosis of drowning].

The diagnosis of drowning is one of the most difficult in forensic pathology. Drowning is death through the aspiration of fluid into the air passages. Signs of immersion only demonstrate submersion of the body for a period of time but are not signs of drowning. The best signs of drowning are froth around the mouth and nostrils and lung distension. Lung histology in drowning victims shows non-specific lesions such as "emphysema aquosum" and alveolar edema. The diatom test for drowning requires a careful protocol to avoid possible contamination. Sample collection (cardiac blood, lung, liver, kid…

Musculoskeletal consequences of near-drowning in children.

Nearly drowned children sustaining cardiac or respiratory arrest or altered neurologic status developed anoxic encephalopathy (30%), died in the ensuing days (36%), or were discharged neurologically normal (34%). In encephalopathic children, spasticity was more malignant than in children with other causes of spasticity. The most common musculoskeletal problem was contracture, especially equinus, hip adductor, hamstring, and quadriceps. Hip subluxation or dislocation occurred in 34% as early as 1 month after injury. Scoliosis developed in 18%, with some requiring bracing and surgery. Thirty-one

The epidemiology of drowning in adulthood: implications for prevention.

Previous epidemiological studies of drowning in the United States have dealt primarily with children. The epidemiology and prevention of drowning in adulthood may be very different. To test this general hypothesis, we analyzed the 293 drownings occurring among Sacramento County residents 20 years of age and above during 1974 to 1985. Drowning rates were highest for men 20-29 years of age (11.5 per 100,000 population) and blacks (7.5 per 100,000 population). Swimmers, boaters, and motor vehicle occupants were most frequently represented. Alcohol use was involved in 48% of cases overall and 77% …
