[OFF-LABEL DRUG USAGE IN THE TREATMENT OF URINARY TRACT INFECTION OF NEWBORNS].

Infectious pathology in the neonatal period is of the leading causes of neonatal morbidity and mortality. Urinary tract infection occurs in 23% of all infectious diseases. The treatment of infectious diseases mainly employs eradication therapy, mostly with anti-bacterial drugs. In practice, the therapy of infants frequently employs prescribing off-label drugs (cephalosporins 1 and 4 generation, macrolides, carbapenem, sulfonamides), which extends the duration of therapy on the average by 37.1% and increases the incidence of adverse reactions on the average by 11.4%. We believe that neonatologi

Awareness about and views of parents on the off-label drug use in children.

BACKGROUND: Off-label use of drugs is widely prevalent in children mainly due to a limited data generated in children during drug development process. Parents play a critical role in giving consent for their child to participate in clinical trials. Very few studies have assessed the opinion of the parents regarding such use and permitting their child to participate in clinical trials. OBJECTIVE: In view of lack of information about the awareness among parents regarding both off-label drug use in children as well as about allowing their child to participate in clinical research especially from

Association between the commercial characteristics of psychotropic drugs and their off-label use.

BACKGROUND: Off-label prescribing, or the use of a medicine for non-Food and Drug Administration (FDA)-approved indications, is especially common for psychotropic therapies and often lacks scientific support. We quantified the association between 4 commercial characteristics of prescription medicines-product age, therapeutic class age, drug volume, and promotional expenditures-and off-label use of antidepressants, antipsychotics, and mood stabilizers from 1998 through 2009. METHODS: We linked data from the IMS Health National Disease and Therapeutic Index, a nationally representative audit of

'He was like a zombie': off-label prescription of antipsychotic drugs in dementia.

This paper explores the legal position of the off-label prescription of antipsychotic medications to people with dementia who experience behavioural and psychological symptoms of dementia (BPSD). Dementia is a challenging illness, and BPSD can be very difficult for carers to manage, with evidence that this contributes to carer strain and can result in the early institutionalisation of people with dementia. As a result, the prescription of antipsychotic and other neuroleptic medications to treat BPSD has become commonplace, in spite of these drugs being untested and unlicensed for use to treat

A comparison of bare-metal and drug-eluting stents for off-label indications.

BACKGROUND: Recent reports suggest that off-label use of drug-eluting stents is associated with an increased incidence of adverse events. Whether the use of bare-metal stents would yield different results is unknown. METHODS: We analyzed data from 6551 patients in the National Heart, Lung, and Blood Institute Dynamic Registry according to whether they were treated with drug-eluting stents or bare-metal stents and whether use was standard or off-label. Patients were followed for 1 year for the occurrence of cardiovascular events and death. Off-label use was defined as use in restenotic lesions

Sustained response to open-label venlafaxine in drug-resistant major depression.

The aim of this study was to evaluate the response to venlafaxine in patients with treatment-resistant depression during an extension phase of an open-label study of venlafaxine. After completing the initial 8 weeks of the study, patients could continue venlafaxine treatment for an additional period of up to 10 months. Efficacy results are given for 149 patients with treatment-resistant depression. Response was defined as a 50% reduction in scores on the Montgomery-Asberg Depression Rating Scale (MADRS); 69% were responders after 8 weeks of treatment in the initial study phase, and 73% were re

The pipeline for drugs for control and elimination of neglected tropical diseases: 2. Oral anti-infective drugs and drug combinations for off-label use.

In its 'Road map for neglected tropical diseases 2021-2030', the World Health Organization outlined its targets for control and elimination of neglected tropical diseases (NTDs) and research needed to achieve them. For many NTDs, this includes research for new treatment options for case management and/or preventive chemotherapy. Our review of small-molecule anti-infective drugs recently approved by a stringent regulatory authority (SRA) or in at least Phase 2 clinical development for regulatory approval showed that this pipeline cannot deliver all new treatments needed. WHO guidelines and coun

Leveraging FDA Labeling Documents and Large Language Model to Enhance Annotation, Profiling, and Classification of Drug Adverse Events with AskFDALabel.

BACKGROUND: Drug adverse events (AEs) represent a significant public health concern. US Food and Drug Administration (FDA) drug labeling documents are an essential resource for studying drug safety such as assessing a drug's likelihood to cause certain organ toxicities; however, the manual extraction of AEs is labor-intensive, requires specialized expertise, and is challenging to maintain, due to frequent updates of the labeling documents. OBJECTIVE: To automate the extraction of AE data from FDA drug labeling documents, we developed a workflow based on AskFDALabel, a large language model (LL…

OnSIDES database: Extracting adverse drug events from drug labels using natural language processing models.

BACKGROUND: Adverse drug events (ADEs) are the fourth leading cause of death in the US and cost billions of dollars annually in increased healthcare costs. However, few machine-readable databases of ADEs exist, limiting our capacity to study drug safety on a broader, systematic scale. Recent advances in natural language processing methods, such as BERT models, present an opportunity to accurately extract relevant information from unstructured biomedical text. METHODS: We fine-tune a PubMedBERT model to extract ADE terms from text in FDA Structured Product Labels for prescription drugs. Here,

Attitudes of caregivers of children labeled with drug allergy after delabeling by provocation tests.

BACKGROUND: Mislabeling children as drug-allergic leads to suboptimal, costly, and potentially harmful treatments. Although drug provocation tests (DPTs) are the gold standard for excluding drug hypersensitivity, the impact of these interventions on caregivers' willingness to reuse delabeled drugs remains elusive. We evaluated DPT outcomes and post-DPT drug use and caregiver attitudes in pediatric patients. METHODS: We analyzed children who underwent DPTs for suspected drug allergy at a tertiary pediatric allergy center between January 2019 and June 2023. Demographic and clinical data were re

Machine learning models automate classification of penicillin adverse drug reaction labels.

There is a growing interest in the appropriate evaluation of penicillin adverse drug reaction (ADR) labels. We have developed machine learning models for classifying penicillin ADR labels using free-text reaction descriptions, and here report external and practical validation. The models performed comparably with expert criteria for the categorisation of allergy or intolerance and identification of high-risk allergies. These models have practical applications in detecting individuals suitable for penicillin ADR evaluation. Implementation studies are required.
