Keratosis obturans (KO) and external auditory canal cholesteatoma (EACC) are two distinct keratinaceous lesions of the external ear. This article reviews the signs, symptoms, pathophysiology, workup, and treatment of each. Patients with either pathology can often be managed in the clinic with debridement; however, EACC is more likely to involve osteonecrosis and require more extensive operative management if disease is not confined to the canal on account of the bony erosion characteristic of cholesteatoma. If required for extensive disease, surgical approaches to both pathologies are similar.

BACKGROUND: Stereotactic radiotherapy (SRT) may represent a new treatment option for individuals with auditory canal or middle ear cancer.
METHODS: Study participants with pathologically proven ear cancer were treated with SRT (35 Gy for 3 fractions or 40 Gy for 5 fractions) as first-line therapy. When local tumor recurrence developed following SRT, subtotal temporal bone resection and postoperative chemoradiotherapy were performed as salvage treatment. Boluses were used for the initial 14 study patients.
RESULTS: Twenty-nine study participants were enrolled and staged with T1 (n = 3), T2 (n

Wideband immittance measures can be useful in analyzing acoustic sound flow through the ear and also have diagnostic potential for the ident … the foam ear tip itself. It is suggested that acoustic leaks in the coupling of the ear tip to the ear canal lead to substantial variations and that this issue needs further work in terms of potential criteria to identify an acoustic leak. In addition, test–retest data from the literature are reviewed.

INTRODUCTION: Etiopathogenetically, there are two types of chollesteatomas: congenital, and acquired. Numerous theories in the literature try to explain the nature of the disease, however, the question about cholesteatomas remain still unanswered. The aim of the study was to present a case of external ear canal cholesteatoma (EEC) developed following microsurgery (ventilation tube insertion and mastoidectomy), as well as to point ant possible mechanisms if its development.
CASE REPORT: A 16-year-old boy presented a 4-month sense of fullness in the ear and otalgia on the left side. A year befo…

OBJECTIVE: We discuss the first reported case of a chronic leukemia involving the external auditory canal.
STUDY DESIGN: Case report and review of the literature (Medline, 1962-2003).
METHODS AND RESULTS: An adult man in good health came to receive medical attention because of a sensation of aural fullness. Radiologic and physical examination demonstrated an ovoid soft tissue mass of the lateral external auditory canal that, after biopsy and flow cytometry studies, was found to be T-cell prolymphocytic leukemia. The patient presented with minimal disease in a highly symptomatic location, all

The effect of ear-canal air pressure on click-evoked otoacoustic emissions was measured for pressures ranging from 200 to -200 daPa and stimulus levels ranging from 60-90 dB PeSPL. Positive and negative ear-canal pressures (relative to ambient pressure) reduced the emission amplitude by 3-6 dB. A spectral analysis of the emissions revealed that the effect of ear-canal air pressure is that of a high-pass filter with a cutoff frequency of 2600 Hz and a slope of 4 dB/oct. The spectral changes are the expected effect of an increase in stiffness of the middle ear and were independent of pressure po

A simple method of reconstructing a previously removed posterior ear canal with an autogenous, bilaminar membrane is described. The resulting air-filled mastoid cavity is an anatomic extension of the middle ear cleft and is separated from the ear canal by a functional barrier that is continuous with the tympanic membrane. The acoustic characteristics of an associated tympanoplasty are not significantly altered, and many of the problems that are associated with an exteriorized cavity are avoided. In contradistinction to other methods of mastoid obliteration or reconstruction, the semitransparen…

OBJECTIVES: The first objective of the study was to compare approaches to eardrum electrode insertion as they relate to the likelihood of introducing an acoustic leak between the ear canal and eartip. A common method for placing a tympanic membrane electrode involves securing the electrode in the canal by routing it underneath a foam eartip. This method is hypothesized to result in a slit leak between the canal and foam tip due to the added bulk of the electrode wire. An alternative approach involves creating a bore in the wall of the foam tip that the electrode can be threaded through. This m…

Description of the ear canal's geometry is essential for describing peripheral sound flow, yet physical measurements of the canal's geometry are lacking and recent measurements suggest that older-adult-canal areas are systematically larger than previously assumed. Methods to measure ear-canal geometry from multi-planar reconstructions of high-resolution CT images were developed and applied to 66 ears from 47 subjects, ages 18-90 years. The canal's termination, central axis, entrance, and first bend were identified based on objective definitions, and the canal's cross-sectional area was measure…

OBJECTIVES: Providing hearing compensation to patients with aural atresia is considerably challenging. Hearing aid transducers vibrating the aural cartilage (cartilage conduction; CC) have been devised, and hearing aids utilizing them (CC hearing aids) have quickly become a beneficial option for aural atresia in clinical applications. However, it remains unclear which placement (on the aural cartilage or mastoid) is beneficial to signal transmission.
METHODS: This study included 35 patients (53 ears with an abnormal ear canal and severe conductive hearing loss) who were using CC hearing aids.…

Verruca vulgaris is commonly seen on the hands, feet, and face, but rarely in the external auditory canal. We report the case of a 57-year-old woman with a huge papillary mass filling the entire external auditory canal, with destruction of the posterior canal wall and excavation into the mastoid bone. We performed en bloc resection of the whole region affected by verruca vulgaris, including the external auditory canal, mastoid skin, and tympanic membrane using canal wall-down mastoidectomy plus meatoplasty. There was no evidence of recurrence 12 months postoperatively. Complete surgical remova

OBJECTIVE: To establish a high-fidelity finite element method (FEM) model of the human inner ear and explore the biomechanical effects of superior semicircular canal dehiscence (SCD) on both cochlear and vestibular function.
METHODS: A detailed FEM model of the entire human ear was reconstructed from high-resolution computed tomography (CT) data. The model was validated through comparison with established experimental data, including basilar membrane (BM) displacement patterns, cochlear tonotopy, inner ear impedance, and middle-ear transfer function. After validation, the model was adapted to

BACKGROUND AND OBJECTIVE: The literature gives hardly any information about the benefits for children of using a bilateral bone-anchored hearing aid (BAHA) as opposed to a bilateral conventional bone conducting hearing aid.
PATIENTS AND METHODS: Within a period of 12 years, three children were selected for treatment with bilateral BAHA. At the time of surgery, the children were aged between 8 and 10 years. Two of the children suffered from Franceschetti syndrome and one child from Goldenhar's syndrome. The clinical course of these three patients is presented.
RESULTS: In all cases, treatment

Objective:To study the influence of the ear canal and middle ear cavity on air conduction and bone conduction. Method:A finite element model of the human middle ear was established. By establishing the external ear canal and the middle ear cavity, we evaluated the effects of the external canal and the middle ear cavity on air conduction and bone conduction. Result:In air conduction, the external canal improved the stapes response at the frequency range of 0.5 kHz to 6 kHz, and the maximum increase was 11 dB at 3 kHz. The middle ear cavity mainly reduced the response of stapes at mid-low freque…

Wideband acoustic immittance (WAI), in particular, ear-canal absorbance, is a useful clinical tool for assessing the middle-ear status and diagnosing conductive hearing disorders. However, little evidence documents the measurement accuracy of WAI in human ears, and, because its clinical adoption is still in its infancy, no international standards exist to define appropriate requirements for commercial instrumentation. A challenge from a standardization point of view is the lack of an absorbance reference, i.e., an acoustic load similar to the adult ear canal with a known absorbance. This paper…

OBJECTIVE: To examine the effects of Castellani's paint on symptomatic relief and skin flora in patients with an itchy external ear canal.
METHODS: Subjective pruritus scores, and erythema and desquamation scores, were noted in 61 patients with an itchy external ear canal. External ear canal skin swabs were taken for bacterial and fungal cultures. Patients were then randomly divided into three groups: either Castellani's paint (group one) or steroid ear drops (group two) were instilled, or non-impacted cerumen was removed (group three). Patients were re-assessed at one month after the initial…

A 3 yr old male castrated Labrador retriever presented for evaluation and treatment of bilateral atresia of the external ear canals. The owners reported that the dog could hear only loud and high-pitched noises. Computed tomography of the head revealed intact vertical and horizontal ear canals filled with debris and a debris-filled right tympanic bulla. Air- and bone-conducted brainstem auditory evoked response (BAER) testing revealed an elevated response threshold to air-conducted stimuli and greater amplitude waveforms evoked by bone-conducted stimuli. The ear canals were surgically correcte…
