Comparison of eight prehospital early warning scores in life-threatening acute respiratory distress: a prospective, observational, multicentre, ambulance-based, external validation study.

BACKGROUND: A myriad of early warning scores (EWSs) exist, yet there is a need to identify the most clinically valid score to be used in prehospital respiratory assessments to estimate short-term and midterm mortality, intensive-care unit admission, and airway management in life-threatening acute respiratory distress. METHODS: This is a prospective, observational, multicentre, ambulance-based, external validation study performed in 44 ambulance services and four hospitals across three Spanish provinces (ie, Salamanca, Segovia, and Valladolid). We identified adults (ie, those aged 18 years and

The Barthel Index and the Cumulated Ambulation Score are superior to the de Morton Mobility Index for the early assessment of outcome in patients with a hip fracture admitted to an acute geriatric ward.

PURPOSE: To examine clinimetric properties of the de Morton Mobility Index (DEMMI) in patients with hip fracture in comparison with the modified Barthel Index (BI), Cumulated Ambulation Score (CAS), and 30-s Chair Stand Test (30-s CST). MATERIALS AND METHODS: Two hundred and twenty two patients with a hip fracture admitted to a geriatric ward following surgery were assessed on day 1 and at discharge (mean of 9 [SD 5.1] post-surgery days). RESULTS: Ninety eight percent and 89% of patients were not able to perform the 30-s CST at baseline and at discharge (large floor effect), respectively. Co

Thoracolumbar spine fractures in the geriatric fracture center: early ambulation leads to good results on short term and is a successful and safe alternative compared to immobilization in elderly patients with two-column vertebral fractures.

INTRODUCTION: Thoracolumbar spine fractures are common osteoporotic fractures among elderly patients. Several studies suggest that these fra … ts the level of functioning 3 months after admission. On comparison, the level of functioning after early ambulation is equal to the level of functioning after immobilization. Where immobilization may lead to complications, early ambulation was not associated with new complications or neurological damage. Based on these advantages, the treatment of elderly patient …

Early mobilisation after abdominal surgery: a concept analysis.

OBJECTIVE: To clarify and define the clinical practice concept of early mobilisation after abdominal surgery. DESIGN: A concept analysis guided by Walker and Avant's method. DATA SOURCES: MEDLINE (Ovid), AMED-(Ovid), Embase (Elsevier) and CINAHL (EBSCO) were searched through 5 December 2024. ELIGIBILITY CRITERIA: Relevant studies that included combinations of the terms 'early mobilisation', 'early ambulation', 'early acceleration', 'abdominal surgery' and 'surgical procedures' were selected. We restricted the search to English full-text publications involving adult patients, limited to the …

Exploring a Culture of Nurse-Led Mobility to Advance Hospitalized Patients on the Recovery Continuum.

Patients with intensive care unit-acquired weakness often fail to reach preadmission baseline values of functional ability at the time of hospital discharge. Progressive mobilization is the use of mobility early in the inpatient stay with intent to maintain muscle mass and strength. The nurse is uniquely positioned to encourage mobilization as a primary caregiver who can oversee patient mobility outside of scheduled rehabilitation sessions. The adverse effects of immobilization involve several complex mechanisms that contribute to protein imbalance, muscle deterioration, and progressive weakne

Patient-Specific Mobilization: A Structured, Systemwide Model of Mobility Care.

Hospital-associated functional decline resulting from immobility and prolonged bed rest is a widespread yet underrecognized complication in acute care. Despite strong evidence linking immobility directly to severe in-hospital complications and significant postdischarge impairments - including reduced independence, diminished quality of life, and impaired ability to perform activities of daily living (ADLs) - many U.S. hospitals lack standardized mobility care programs. Mobility practices vary widely and are typically driven by individual clinician knowledge, institutional culture, and informal

Preventive effects of early mobilisation on delirium incidence in critically ill patients: systematic review and meta-analysis.

OBJECTIVE: To evaluate whether early mobilisation can reduce the incidence of delirium in critically ill patients and to assess the methodological quality of published studies. METHODS: Three electronic databases, PubMed, Embase and the Cochrane Library, were searched for relevant studies published up to 2 March 2024. Articles were screened independently by two reviewers, based on inclusion and exclusion criteria, and a meta-analysis was performed using RevMan 5.3 software with a random-effects model. RESULTS: A total of 18 studies (intervention group: 1794 participants, control group: 2129

Summary of the best evidence for early mobilization after lumbar spinal fusion surgery: a systematic review.

PURPOSE: This study aimed to systematically identify, critically appraise, and synthesize the highest-quality evidence on early mobilization following lumbar spinal fusion surgery (LSF), thereby establishing an evidence-based framework to guide clinical nursing practice. METHODS: Following the 6 S evidence model, we performed a comprehensive literature search across multiple databases and gray literature sources up to December 2024. Studies investigating early postoperative mobilization in LSF patients were screened for eligibility and rigorously assessed for methodological quality. Relevant e

Roller traction: mobilizing patients with acetabular fractures.

Patients with acetabular fractures unamenable to open reduction and internal fixation are subject to 6-12 weeks of traction and, traditionally, bed rest. Using roller traction allows the patient to be mobilized to a bedside chair and to stand without disrupting the traction. This is accomplished by slightly modifying a standard lower extremity skeletal traction setup. Roller pulleys and stopper clamps are substituted for the usual pulleys. These are attached to extra-long horizontal bars that expand the lateral space that the patient can now use around the bed. Roller traction helps prevent th

Understanding kinesiophobia in older postoperative patients admitted to the ICU: A narrative review of conceptual models, assessment tools, and clinical implications.

BACKGROUND: Early mobilization is a key component of recovery for older patients in intensive care units (ICUs). However, psychological resistance to movement-particularly fear-based avoidance-remains poorly understood and frequently overlooked in critical care settings. OBJECTIVE: This narrative review aims to explore the conceptual relevance of kinesiophobia (fear of movement) in older postoperative patients admitted to the ICU, examine its potential assessment challenges, and outline nursing-sensitive strategies for identification and intervention. METHODS: A purposive literature search w

Effectiveness of structured early mobilization protocol on mobility status of patients in medical intensive care unit.

INTRODUCTION: Patients admitted to the intensive care units (ICU) have limited mobility due to their illness and its management and are at a risk for immobility-related complications. Early mobilization has been suggested to prevent or limit physical dysfunction due to these complications. Effectiveness of early mobilization protocols is studied using various outcomes. OBJECTIVE: To study the effectiveness of an early mobilization protocol on mobility status of patients in Medical ICU. METHODS: Patients admitted to Medical ICU were screened for eligibility and allotted into two groups. Inter

Mobilisation of epistaxis patients--a prospective, randomised study documenting a safe patient care regime.

OBJECTIVE: The project aimed at determining whether patients admitted to hospital for the treatment of epistaxis can be mobilised instead of the conventional regime of bed rest without increasing the risks of new bleeding episodes. BACKGROUND: A survey of nursing regimes for epistaxis patients has shown that patients were prescribed bed rest in 11 of 16 hospital departments. This prescription has been based on the assumption that bed rest reduces the risk of renewed bleeding. There is no scientific documentation of the need for bed rest demonstrating any reduced risks of new bleeding episodes

Orthopaedic Out of Bed Project (OOBP): improving early mobilisation following femoral fracture using a therapy-led education programme.

Delayed mobilisation following hip fracture surgery is detrimental to patients and health systems. Prolonged hospital stay additionally results in decreased function and increased mortality. Our hospital was underperforming against the national metric for mobilisation by the day after surgery and physiotherapists were the primary healthcare professionals expected to do this. The therapy team therefore undertook a service improvement to increase the number of patients mobilised by the day after their femoral fracture surgery. This was through a ward-based education programme aimed at increasing

The pneumatic post-amputation mobility aid in geriatric rehabilitation.

Several disadvantages of rigid temporary dressings for geriatric amputees led to the trial of a relatively new kind of pylon--the Pneumatic Post-Amputation Mobility Aid (PPAM). Thirty-five geriatric amputees who were referred to a geriatric rehabilitation hospital participated in the study. Thirty-three of these patients learned to walk with the device easily and safety. The below-knee amputees achieved independent gait with a walker as early as their second treatment. The above-knee and the double below-knee amputees required closer supervision. The inexpensive price of the PPAM, its handines

Early mobilization after anatomical lung resection with thoracotomy.

In the past, patients who underwent thoracic surgery were advised to rest, recover, and save energy, avoiding engaging in tiring physical activity. Postoperative rest-centered management of patients following anatomical resection can cause pulmonary and cardiovascular complications. Inability to cough, not deep breathing, dysfunctional diaphragm, pain and lying down cause lung atelectasis, pneumonia, and respiratory failure. Early postoperative mobilization's effects on mental or physical recovery and morbidity rate are unclear. Although advanced technological developments, thoracotomy is stil

Early mobilisation after hip fracture surgery is associated with improved patient outcomes: A systematic review and meta-analysis.

INTRODUCTION: The aims of this systematic review and meta-analysis were to determine if after hip fracture surgery (1) early mobilisation is associated with improved clinical outcomes, and if so (2) are benefits directly proportional to how soon after surgery the patient mobilises. METHODS: A Preferred Reporting Items for Systematic Reviews and Meta-Analyses systematic review was conducted using four databases to identify all studies that compared postoperative early mobilisation with delayed mobilisation, in patients after hip fracture surgery.
