PURPOSE: To evaluate the results from the correction of ectropion of the punctum lacrimale in lower eyelids with a new surgical clamp.
DESIGN: Prospective study.
METHODS: Eighty eight eyelids in 55 patients with mild and moderate ectropion were included in the study. An excision of a diamond of tarso-conjunctiva with retractor reattachment and concomitant correction of horizontal lid laxity, if present, was performed.
RESULTS: Resolution of tearing was obtained in 77 eyes. In 11 eyes, persistent tearing was reported.

Left upper eyelid eversion with chemosis was present, passive to mechanic reduction. Compressive occlusion was done with ectropion regression in 48 hours. We report a case of transient ectropion associated with corneal ulcer. The corneal ulcer responded well to treatment with fortified gentamicin and cefazolin. As the corneal ulcer healed, the eyelid simultaneously returned to its original position. We suggest that in the presence of involutional eyelid changes, even a minimal additional factor, such as conjunctival edema and blepharospasm, may cause transient ectropion.

We present nine patients with unilateral congenital ectropion uveae (hyperplasia of the iris pigment border), iris hypoplasia, characteristic gonioscopic findings, and glaucoma. A majority of these patients also demonstrated mild ptosis. In none of these patients were there any associated systemic abnormalities. Pityriasis rubra pilaris is an idiopathic, papulosquamous dermatological disease. It is clinically and histologically distinct from, but may resemble, psoriasis. Pityriasis rubra pilaris can be self-limiting but may also run a protracted, relapsing course. A 6-month-old infant with congenital icthyosis was brought to the authors with bilateral cicatricial ectropion of the upper and lower eyelid with resultant exposure keratopathy. Skin autograft was not possible due to generalized skin involvement, leading to unavailability of a donor site. A maternal skin allograft was performed for both eyelids of each eye. Prior human leukocyte antigen typing was not done, and postoperative immunosuppressive agents were not given. At 10 months' follow-up, the graft was well accepted, and the ectropion was corrected.

Eleven patients with cutaneous anthrax of the eyelids are presented. The complications were cicatricial ectropion (eight patients), resulting in corneal scarring (three patients). The ectropion was corrected by full thickness postauricular skin grafts, with good results. The predilection of this infection for the eyelids of young children and a seasonal variation suggest that a vector may be involved in transmission.

A very rare combination of congenital malformations of the eyelids and eyes in a 6-month-old boy is reported. The main symptoms are a bilateral microblepharon associated with ectropion and bilateral aphakic microphthalmus. The pathogenesis of these changes is discussed.

BACKGROUND: Amniotic band syndrome (ABS) or amniotic deformity, adhesions and mutilations (" ADAM" ) is a spectrum of rare congenital malformations related to an early phase of organogenesis.
