Pediatric Elbow Injuries.

Elbow trauma in children is one of the most commonly encountered musculoskeletal injuries in pediatric radiology. However, elbow injuries in children can be misdiagnosed due to secondary ossification centers unique to pediatric patients. Familiarity of the normal elbow anatomy in children is crucial for an accurate diagnosis. This article seeks to improve diagnostic accuracy of these elbow injuries by reviewing the secondary ossification centers in the elbow in the pediatric population, followed by a discussion of commonly encountered acute and chronic fractures, dislocations or other injuries…

Medial elbow pain.

Medial elbow pain is uncommon when compared with lateral elbow pain.Medial epicondylitis is an uncommon diagnosis and can be confused with other sources of pain.Overhead throwers and workers lifting heavy objects are at increased risk of medial elbow pain.Differential diagnosis includes ulnar nerve disorders, cervical radiculopathy, injured ulnar collateral ligament, altered distal triceps anatomy or joint disorders.Children with medial elbow pain have to be assessed for 'Little League elbow' and fractures of the medial epicondyle following a traumatic event. This paper is primarily focused on …

Posttraumatic elbow stiffness.

Early posttraumatic elbow contractures may be treated with a combination of manipulation with the patient under anesthesia followed by bracing.Extrinsic contractures of the elbow may be treated with open or arthroscopic release, whereas intrinsic and combined contractures may require tissue release as well as partial or total arthroplasty.

[Elbow: plain radiographs].

Plain radiographs usually allow accurate diagnosis. In this paper, the main radiographic projections will be reviewed along with their normal imaging features. In spite of significant advances in imaging of the elbow (multiplanar capabilities of MRI, volumetric CT imaging), conventional radiographs of the elbow remain a firstline imaging technique in the evaluation of disorders of the elbow.

Unstable elbow dislocations.

Seventeen patients with a posterior dislocation of the elbow and either no fracture or a minimal capsuloligamentous avulsion fracture were treated operatively for persistent redislocation after manipulative reduction. Fifteen had open reduction and ligament repair (3 with ancillary hinged external fixation), and 2 infirm patients had closed reduction and cross pinning of the elbow joint. Among the 12 patients treated with ligament repair alone, 1 redislocated (treated with a second surgery for hinged external fixation) and 4 had residual subluxation (treated with temporary casting of protected…

Posttraumatic elbow stiffness.

This article outlines the nonoperative and operative treatment of posttraumatic elbow contractures. Elbow stiffness is a significant cause of disability because it results in the inability to position the hand in space. Initial treatment should be nonoperative, consisting of splinting and physical therapy. Established elbow contractures that interfere with a patient's vocation or avocations can be treated operatively by either open or arthroscopic release. The surgical release of posttraumatic elbow contractures is rewarding, with a high incidence of success and a reasonable risk of complicati…

Pediatric elbow injury.

The increasing involvement of children in sport and fitness related activity has been parallel by an increase in the frequency of sports-related childhood injury. Immature, open epiphyses, as well as soft articular cartilage predispose children to age-specific injury. An understanding of osseous development, normal radiologic variants, and the diagnosis and treatment of common elbow injuries in the pediatric population will assist the clinician who treats younger patients.

Congenital elbow dislocations.

Three categories of congenital elbow dislocations are described in the literature: humeroulnar, humeroradial, and dislocation or joint laxity with other congenital anomalies. Congenital humeroulnar dislocation is a well-defined problem that occurs predominantly in small breeds of dogs and causes severe limb deformity and dysfunction. Early reduction and stabilization are simple to achieve and have provided good results. Radial head dislocations occur most often as a result of growth disturbance of the radius and ulna. There is a lack of conclusive information on congenital radial head dislocat…

Intermediate-term outcomes for floating elbow and floating elbow variant injuries.

BACKGROUND: There are limited outcome studies describing floating elbow injuries. The purpose of this report is to describe patient outcomes after floating elbow injury using the American Shoulder and Elbow Surgeons (ASES) elbow assessment form. MATERIALS AND METHODS: From 1994 to 2004, 19 patients were retrospectively identified at 3 Level I trauma centers and returned for follow-up evaluation after treatment of their floating elbow injury. Patient outcomes were assessed based on physical examination and the ASES elbow form. RESULTS: The 19 patients were evaluated at a mean of 6.7 years (ra…

Development and validation of a new elbow-specific scoring system for patients with elbow stiffness: the Shanghai Elbow Dysfunction Score.

BACKGROUND: Clinical scoring systems are increasingly important and popular for the evaluation of orthopedic patients. Elbow stiffness commonly causes functional impairment and upper-limb disability. The purpose of this study was to develop and validate a new elbow-specific assessment score to evaluate joint function in patients with elbow stiffness. METHODS: The new system, the Shanghai Elbow Dysfunction Score (SHEDS), was developed in 3 portions: elbow motion capacities, elbow-related symptoms, and patient satisfaction level. A total of 73 patients with elbow stiffness were prospectively in…

Elbow Resection for Deep Infection After Total Elbow Arthroplasty: Surgical Technique.

INTRODUCTION: Deep infection at the site of a total elbow arthroplasty is best managed with definitive removal of the components with resection arthroplasty in selected patients. STEP 1 SKIN INCISION AND FLAP MANAGEMENT: Use a previous skin incision when possible, keep the subcutaneous flaps as thick as possible, and avoid inadvertent iatrogenic injury to the ulnar nerve. STEP 2 IDENTIFY ULNAR AND RADIAL NERVES: The location of the ulnar nerve may be unpredictable, and the radial nerve may be at risk in two different locations. STEP 3 DEAL WITH EXTENSOR MECHANISM: Access the implants throug…

Residual elbow instability in children with posterior or postero-lateral elbow dislocation.

The purpose of this study is to evaluate the incidence and the severity of residual elbow instability in children treated for posterior (PED) or postero-lateral (PLED) elbow dislocation. This retrospective study included all children younger than 14 years of age with a confirmed diagnosis of acute post-traumatic elbow dislocation (ED). Subjective data such as perceived pain, stiffness and impact of daily activities, and objective data such as skin lesions, surgical-site infections, and range of motion were recorded. All patients underwent the milking test, the chair sign test (CST) and the Dra…

[Exploring the mechanism of children elbow joint injury led to elbow varus].

OBJECTIVE: To explore the causes and mechanisms of children elbow joint injury led to elbow varus. METHODS: One hundred children with elbow varus causded by elbow joint injuries were retrospectively studied during January 2005 to April 2008 in department of child orthopedics involving 54 males and 46 females with an average age of 9.7 years old ranging from 7 to 16 years. The anterior-posterior and lateral position X-ray films of elbow joint continuous 12 to 18 months (means 15 months) of all the cases were collected. The postoperative X-ray films were observed to analyze the cause of elbow v…

Systematic review of primary total elbow prostheses used for the rheumatoid elbow.

Total elbow prosthesis (TEP) has been shown to be a viable option for treatment of the rheumatoid elbow. Many types of TEP have been studied, but the heterogeneity of the studies makes most conclusions subject to discussion. The aim of this systematic review is to show the differences between the most commonly used TEP for the destroyed rheumatoid elbow. After a search in Pubmed (NLM, Bethesda, USA) the senior author selected eight frequently used TEP: the Capitellocondylar, Coonrad-Morrey, GSB III, Kudo, Liverpool, Norway, Roper-Tuke and Souter-Strathclyde. For inclusion studies we arbitraril…

Role of Elbow Incongruity in Canine Elbow Dysplasia: Advances in Diagnostics and Biomechanics.

Elbow incongruity plays an important role in the development of elbow dysplasia and its diagnosis can be challenging. The objectives of this systematic review were to discuss elbow biomechanics and recent advances in the assessment of incongruity, to revise the diagnostic protocols suggested, debate advantages and disadvantages of measurements proposed to characterize the joint and improve screening. In spite of advances in the understanding of elbow incongruity, the exact mechanisms involved are not fully elucidated. The elbow is a complex joint and structures in contact differ in different ph…
