Functional outcomes of "floating elbow" injuries in adult patients.

OBJECTIVES: To assess elbow function, complications, and problems of floating elbow fractures in adults receiving surgical treatment. DESIGN: Retrospective clinical review. SETTING: Level I trauma center in Kanagawa, Japan. PATIENTS: Fourteen patients with fifteen floating elbow injuries, excluding one immediate amputation, seen at the Kitasato University Hospital from January 1, 1984, to April 30, 1995. INTERVENTION: All fractures were managed surgically by various methods. In ten cases, the humeral and forearm fractures were treated simultaneously with immediate fixation. In three cases,…

Bony injuries about the elbow in the throwing athlete.

Tremendous valgus stress during the acceleration phase of pitching is the primary cause of elbow injury in the pitcher. Lateral compression injuries are usually found in pitchers in their early teens, and osteochondritis dissecans is the primary underlying cause. Severe flexion contractures and even partial ankylosis of the elbow may occur. Prognosis after surgery is generally poor. Medial tension injuries with either soft-tissue damage or bony damage usually require surgery. The medial collateral ligament should be considered the primary stabilizer against the valgus-vector overload. The medi

[Ligamentous elbow injuries-When is surgical treatment still indicated?].

The elbow joint is a hinge-type synovial joint and is the second most frequently dislocated joint in adults and the most frequently dislocated joint in children. To find the right treatment options a precise understanding of the injury pattern, anatomy, biomechanics and pathology (simple vs. complex dislocation) is essential for a good reconstruction of the function and to prevent chronic instability and pain. The differentiation between acute and chronic instability gives another variation in the treatment plan. According to the latest literature there are clear indications for surgery of a c

Elbow Ulnar Collateral Ligament Injuries: Indications, Management, and Outcomes.

Ulnar collateral ligament (UCL) injuries continue to be a major source of morbidity in baseball players. The throwing motion creates nearly supraphysiological levels of valgus stress on the medial elbow, placing these athletes at high risk of UCL injury. The incidence of injury continues to rise at an alarming rate, especially among adolescent baseball pitchers. Certain risk factors for UCL injury have been identified, including pitch velocity, fewer days between outings, and overall workload. Treatment of UCL injuries depends on the type of tear. Low- to medium-grade partial UCL tears (i.e.,

Risk factors for elbow injuries among young baseball players.

HYPOTHESIS: There are some risk factors that could predispose a young baseball player to elbow injuries. MATERIALS AND METHODS: Study participants were 294 baseball players aged 9 to 12 years old. A questionnaire, physical examination, and ultrasound imaging to investigate elbow injuries were performed. Data for the groups with and without elbow injuries were analyzed statistically using multivariate logistic regression models. RESULTS: Ultrasound imaging showed that 60 participants had elbow injuries, including medial epicondylar fragmentation in 58 and osteochondritis dissecans of the capi…

Distal radioulnar joint dislocation in association with elbow injuries.

Traumatic distal radioulnar joint (DRUJ) dislocation with or without an associated fracture is a rare injury. When coupled with a radial head fracture this is commonly known as the Essex-Lopresti injury. We report two cases of elbow dislocation with ipsilateral radial neck fractures and associated true DRUJ dislocations. This has not been previously described in the literature. In elbow injuries with wrist involvement, symptoms in the latter may be subtle. Due to inadequate examination of the affected joint, poor initial radiographic views, and general rarity of this injury, distal radioulnar …

Shoulder and elbow injuries in the skeletally immature athlete.

The intensity of training and competition among young athletes can place them at increased risk of acute and chronic injuries, which occur in patterns unique to the skeletally immature athlete. Prompt recognition and treatment of these injuries are critical to prevent long-term functional disability and deformity. Children and adolescents participating in recreational and organized sports are particularly susceptible to a broad spectrum of shoulder and elbow injuries involving both osseous and soft-tissue structures. Understanding the relevant functional anatomy, biomechanics of throwing, and …

Magnetic resonance imaging of sports injuries of the elbow.

Many abnormalities seen in the elbow result from trauma, often from sports such as baseball and tennis. Elbow problems are frequently related to the medial tension-lateral compression phenomenon, where repeated valgus stress produces flexor-pronator strain, ulnar collateral ligament sprain, ulnar traction spurring, and ulnar neuropathy. Lateral compression causes osteochondral lesions of the capitellum and radial head, degenerative arthritis, and loose bodies. Other elbow abnormalities seen on magnetic resonance imaging include radial collateral ligament injuries, biceps and triceps tendon inj

Rehabilitation of shoulder and elbow injuries in tennis players.

Optimization of treatment of the tennis player with an upper extremity overuse injury requires a thorough understanding and analysis of the biomechanical and physiologic stresses inherent in the game of tennis. Anatomic adaptations found in the upper extremity of elite tennis players provide the framework for evaluation and treatment of overuse injury through both an anatomically based total-arm strength program and efforts to normalize joint arthrokinematics. Integration of these key factors allows the clinician to rehabilitate the patient and design preventive conditioning programs in a scie

Prevention and rehabilitation of overuse injuries of the elbow.

Rehabilitation from injury and prevention of injury are terms that describe differing aspects of the same challenge. The clinician must be continually aware of the principles of rehabilitation and their order of priority (prevent further damage, restore motion, restore strength, reestablish coordination) while supervising patients' progress toward their goals. If patients are made aware of these principles at an early stage, and taught to take as much responsibility as possible for their own rehabilitative course, they will be more likely to reach their own ideal compromise of safety and speed

Chronic stress injuries of the elbow in young gymnasts.

The radiological changes of chronic stress injuries of the elbow in 19 adolescent elite gymnasts are reported. The principal abnormalities were avascular necrosis of the capitellar epiphysis (Panner's disease) (one patient), post-traumatic subarticular necrosis (osteochondritis dissecans) of the capitellum (six patients) and the medial articular eminence of the distal humerus (one patient). Flattening and anterior depression of the radial head epiphysis with an underlying metaphyseal notch associated with an epiphyseal cleft (three patients), which, in one patient who continued training, devel

Radiographic Study of Elbow Injuries in Professional Rodeo Cowboys.

In brief: A radiographic study of 21 bull riders and 18 bareback riders was conducted to determine the bony changes that result over time from elbow injuries in professional cowboys. X-rays revealed 39 bony abnormalities in the bull riders and 37 in the bareback riders. More than 60% of these changes occurred in the medial epicondyle and the posterior aspect of the olecranon process. Surprisingly, 30 of the injuries affected the free arm rather than the gripping arm. The authors recommend that riders tape both elbows to prevent hyperextension and that they wear a pad for protection from an ani…

Ipsilateral injuries of the elbow and forearm in children.

Fracture of the forearm with ipsilateral injury of the elbow joint is a rare traumatic entity in children. Twenty-four such cases were treated in our clinic during the last 4 years (1981-1984). The treatment was surgical in six cases and related to the elbow injuries, while the forearm underwent closed treatment. The results of these cases, regardless of treatment and severity of the injury, did not differ appreciably from the results of common forearm or elbow injuries.

Management of Geriatric Elbow Injury.

Approximately 4.1% of all fractures in the elderly involve the elbow. Most elbow injuries in geriatric patients occur as the result of low-energy mechanisms such as falls from standing height. Elbow injuries in elderly patients present complex challenges because of insufficient bone quality, comminution, articular fragmentation, and preexisting conditions, such as arthritis. Medical comorbidities and baseline level of function must be heavily considered in surgical decision making.

Elbow injury in women's gymnastics.

This investigation reports on 30 female gymnasts with 32 elbow injuries acquired in gymnastics. Thirty of the injuries were acute fractures and dislocations, and two developed Panner's disease of the humeral capitellum. Primary treatment included 14 closed reductions and 11 open surgical procedures. Those who returned to competition lost an average of 4.1 months. Spotters were not present during nearly 60% of these injuries. The mechanism which led to a variety of elbow injuries was possibility singular: a fall on an outstretched arm. More than twice as many injuries occurred on this mats or t…
