Obturator hernia: anatomy, embryology, diagnosis, and treatment.

An obturator hernia is an infrequent but significant cause of intestinal obstruction. The incidence of obturator hernias is between 0.05 and … his article is to review the diagnosis and treatment of obturator hernia by describing the anatomy, embryology, clinical presentation, diagnosis, and management currently in use. Increased awareness and proper management of this condition will result in decreased morbidity and mortality.

[The hand: embryology and main malformative mechanisms].

Upper limb bud appears in the cervical region of the embryo during the fifth week of development. It is made of epithelia and underlying mesenchyme. Diffusible growth factors, expressed by the apical ectodermal ridge, direct the proximal-distal growth. Other factors are expressed by zone of polarizing activity and ectoderm. They induce together anterior-posterior growth and dorsal-ventral polarity of the limb bud. The development of axial skeleton pattern is controlled by transcription factors from the HOX family, which are expressed in a stripe along the proximal and distal edges of the limb

Spinal lipomas: clinical spectrum, embryology, and treatment.

Spinal lipomas, particularly lipomas of the conus medullaris and terminal filum, are the most common form of occult spinal dysraphism and represent a wide spectrum of disease with regard to anatomy, clinical presentation, and treatment options. These lesions, however, are united by a similar embryology and pathological mechanism by which symptoms arise. Recently, the treatment of these lesions has generated much controversy, with some physicians advocating surgical treatment for all patients regardless of symptoms and others proposing that surgery be withheld until symptoms develop. The author…

Congenital abnormalities of body patterning: embryology revisited.

Many of the developmental mechanisms and molecular pathways that underlie fundamental features of body patterning are shared by all vertebrates, and some have even been conserved across evolution from invertebrates to vertebrates. Defects in such processes are a common cause of congenital malformation syndromes, and rapid progress is being made in elucidating their embryological and genetic basis. Here, I focus on three examples, each of which has been the subject of recent advances, and which together illustrate many of the most interesting and important aspects of these disorders. The first

Persistent sciatic artery: embryology, pathology, and treatment.

PURPOSE: The purpose of this article is to describe the embryologic development and anomalous persistence of the sciatic artery, pathologic changes that may occur in the persistent sciatic artery, and management of complications related to these pathologic changes. METHODS: Two patients with persistence of the sciatic artery treated in the authors' experience are reported. In addition, the computerized data base of the University of Mississippi Medical Center was searched for patients with other lower extremity arterial aneurysms, and their records were reviewed for possible aneurysm of a per

Embryology of the eye and its adnexae.

The embryonal and fetal development of the human eye includes a series of sequential events starting with the fertilization of the ovum and culminating in the birth of a normal baby. Three main periods can be distinguished in the prenatal development of the eye. The first period called embryogenesis is characterized by the establishment of the primary organ rudiments and ends at the end of the 3rd week with the appearance of the optic sulci on both sides of the midline at the expanded cranial end of the still open neural folds. The second period called organogenesis includes the development of

[Choanal atresia: review of embryology and pathogenesis].

In the paper a study on the embryology and pathogeny of choanal atresia is presented, with particular reference to its association with other congenital malformations. The experience of the AA., gained through 10 of those cases seen at the ENT-department of the Servasa Hospital, Alicante, is the low incidence of coexisting malformations. They emphasize one case of familiar presentation (mother-daughter), other with eye coloboma and bifid uvula coexistence and a last one with a sensorineural deafness of moderate degree.

The embryology and surgical management of gastroschisis.

Thirteen cases of gastroschisis are presented and a reduction in the operative mortality to 15 per cent is recorded. The aetiology of the condition is discussed and the opinion expressed that it represents the result of an intra-uterine rupture of an incarcerated "hernia into the cord" rather than any specific failure of development of the abdominal wall musculature. Early skin closure and the creation of a ventral hernia remain the surgical treatment of choice, but it is recognized that the use of a prosthetic sac may occasionally be necessary. Infection is not the most important single facto

Cleft lip∕palate. From embryology to developmental pathogenesis.

Cleft of the lip associated with or unassociated with cleft of the palate and cleft palate (CLP) represent frequent birth defects, and the mechanisms by which they occur are not completely clear. On the other hand, knowledge of the normal mechanisms underlying lip development and palatogenesis is still incomplete, and therefore a better understanding and consolidation of our knowledge in this field could help to study these deformities. Upper lip and secondary palate formation are complex processes, involving the subtle integration of several biological events, the disruption of which causes t

Otto Grosser: doyen of embryology and national socialist.

INTRODUCTION: Otto Grosser (1873-1951) was a prominent embryologist in the first half of the twentieth century. This paper summarizes his scientific contributions and controversial politics. RESULTS: Grosser developed a classification of placental types, based on the tissues in the interhaemal barrier, published as influential monographs in 1909 and 1927. He was professor of anatomy at the German University in Prague for 36 years, first under the Habsburg empire, then in Czechoslovakia, and finally during the German occupation. Grosser was a German nationalist and strove to promote German sci

Neural crest cells: bridging embryology and regenerative medicine.

Neural crest cells are an embryonic stem cell population unique to vertebrates. Neural crest cells, which arise from the embryonic ectoderm germ layer, are thought to give rise to neurons and glia of the peripheral nervous system, as well as melanocytes throughout the body. In addition, neural crest cells give rise to many ectomesenchymal derivatives in the head, including cells typically derived from the mesoderm (cartilage, bone, and adipose tissue). Neural crest cells are a group of multipotent and migratory cells found throughout the developing embryo. These cells migrate to many different

Intestinal atresia and necrotizing enterocolitis: Embryology and anatomy.

The primitive gut originates at week 3 of gestation from the endoderm, with posterior incorporation of the remaining embryo layers. Wnt, Notch and TLR4 pathways have been shown to play central roles in the correct development of the intestine. The classical hypothesis for intestinal atresia development consists of failure in bowel recanalization or a vascular accident with secondary bowel reabsorption. These have been challenged due to the high frequency of associated malformations, and furthermore, with the discovery of molecular pathways and genes involved in bowel formation and correlated d

Embryology and physiology of testicular development and descent.

Sexual differentiation starts with the development of bipotential gonads that further differentiate into testes or ovaries. The fetal testis secretes hormones that guide the differentiation of internal and external sex organs, whereas the fetal ovary remains rather inactive hormonally. Defects in gonadal differentiation or hormone secretion and action result in disorders of sex development (DSD). Testicular descent is a continuum that has often been described to occur in two main phases: the transabdominal phase and the inguinoscrotal phase. The first phase is according to animal studies depen

Embryology, diagnosis, and evaluation of congenital hand anomalies.

Although congenital hand anomalies are rare, musculoskeletal clinicians should have a basic understanding of their clinical manifestations and the possibility of concurrent anomalies and syndromes. In this review, we provide a brief overview of the embryology of limb development and the molecular pathways involved. We also summarize the clinical manifestations, diagnostic evaluation, and principles of surgical treatment for radial longitudinal deficiency, thumb hypoplasia, ulnar longitudinal deficiency, central deficiency, syndactyly, polydactyly, and amniotic constriction band. Although one o…
