Empathy deficits in Asperger syndrome: a cognitive profile.

Although lack of empathy has been considered a central characteristic of Asperger syndrome, quantitative and qualitative assessments of empathy in this syndrome are lacking. We present two cases of adolescents with Asperger syndrome who show extreme deficits on measures of both cognitive and affective empathy. Analysis of their performance on tasks assessing cognitive and affective processing did not reveal significant impairment in executive functions, nor in their ability to recognize emotions or the ability to create a mental representation of another person's knowledge. However, both patie…

Hostility toward women and victim empathy in rapists.

The present study examined empathy and hostility toward women among 32 rapists, 28 nonsex offenders, and 40 nonoffender males. Results indicated that rapists were significantly less empathic than either of the other two groups toward women who had been sexually assaulted by an unknown assailant. They were also significantly less empathic toward their own victims than toward any other women, and they were markedly more hostile toward women than were the other subjects. Finally, among the rapists, hostility toward women was significantly negatively related to their empathy toward their own victi…

Empathy and cognition in high-functioning children with autism.

This study compares the ability of nonretarded autistic children (9-16 years of age) with the ability of normally developing children (9-14 years of age) to discriminate between various emotional states, to take the perspective of another regarding emotional states, and to respond affectively. The children's understanding of conservation was also assessed. While the children with autism did surprisingly well on the empathy-related measures, they performed less well than the normal children on these measures and on conservation. There was a closer association between cognitive abilities and aff…

Proxy patients' perceptions of genetic counselor empathy responses.

Empathy, the ability to understand and communicate another's experience to them, positively affects genetic counseling processes and outcomes. Genetic counseling, a specialty healthcare profession, provides education and support for those impacted by genetic disease; thus, empathy is critical. Limited studies have explored patients' perceptions of genetic counselor (GC) empathy responses. This study investigated the types of GC responses proxy patients perceived as most empathic and why. Female MTurk workers (n = 198) completed a survey containing two hypothetical genetic counseling scenarios …

Empathy Throughout the Clinical Years in Iranian Generation Z Medical Students: Validity and Reliability Properties of the Persian Version of the Jefferson Scale of Empathy.

BACKGROUND: The Jefferson Scale of Empathy-Student Version (JSE-S) is among the most widely used instruments for assessing empathy in medical students. This study aimed to evaluate the validity and reliability of the Persian version of the JSE-S among Iranian undergraduate clinical trainees, specifically clerks and interns. METHODS: A cross-sectional study was conducted involving 531 medical students. Content validity was assessed using the CVR, CVI, and kappa coefficient, while face validity was evaluated through student ratings. A PLS-based confirmatory measurement model was implemented in …

Safeguarding medical empathy and cultivating compassion in contemporary medicine.

Empathy and compassion are both fundamental to patient-centred care, enhancing satisfaction, adherence and outcomes while improving clinician well-being. However, certain aspects of medical culture may put these emotional qualities at risk. The preclinical-to-clinical transition exposes students to hierarchical structures, time constraints and performance expectations prioritising technical competence over interpersonal skills. Negative role modelling, rushed consultations and inadequate emotional support normalise emotional detachment. The COVID-19 pandemic intensified burnout and moral injur…

Therapeutic modulation of empathy: pharmacological, neurostimulation, and behavioral approaches.

Empathy is a core neurobiological capacity that enables humans to perceive, understand, and respond to others' experiences, yet its deliberate modulation as a therapeutic target is still in its early stages of scientific development. This manuscript presents a hybrid narrative-scoping review synthesizing evidence across pharmacological, neurostimulation, and behavioral approaches aimed at enhancing or restoring empathic functioning. A structured literature search covering 2014-2025 was conducted across PubMed, PsycINFO, Web of Science, and Scopus, with emphasis on post-2020 research. Evidence …

Do pigs have altruistic behaviour based on cognitive empathy?

BACKGROUND: Altruistic behaviour is commonly defined as actions that benefit others at a potential cost to oneself and, in animal research, is typically inferred when individuals assist conspecifics without immediate personal gain. In pigs, altruistic behaviour associated with cognitive empathy remains an understudied area. In the present study, altruistic behaviour was operationally defined as a pig voluntarily sacrificing its own access to a food reward to alleviate a peer’s distress. We investigated whether pigs adjust their own behaviour in relation to the emotional state of others in a ma…

Pain Overexposure Affects Self-Pain and Empathy in Athletes.

Experiencing pain repeatedly or over prolonged periods has been shown to affect the pain experience itself. Similarly, people overexposed to others' pain respond differently to expressions of it. Given that pain experience and empathy toward others' pain share cognitive and cerebral mechanisms, examining how both are affected by overexposure to self-pain and others' pain within the same population is of particular interest. Athletes represent one such population. This study thus aimed to compare pain perception and empathy in athletes and non-athletes. Thirty athletes and 28 non-athletes were …

Understanding Empathy Toward Dissimilar Others in Challenging Everyday Interactions.

Empathy is essential for human social interaction; however, extending empathy toward individuals with dissimilar characteristics facing daily challenges may be difficult. This study investigated how people without disabilities empathize with individuals with disabilities, specifically those with stroke-induced hemiplegia, during manual interactions with objects or other people. Using functional magnetic resonance imaging and multivoxel pattern analysis (MVPA), we examined the neural and behavioral mechanisms underlying empathy in these contexts. Participants observed video stimuli featuring in…

Grey-matter correlates of empathy in 4-Repeat Tauopathies.

Loss of empathy is an early and central symptom of frontotemporal lobar degeneration spectrum diseases. We aimed to investigate the topographical distribution of morphometric brain changes associated with empathy in Progressive Supranuclear Palsy (PSP) and Corticobasal Syndrome (CBS) patients. Twenty-seven participants with CBS and 31 with PSP were evaluated using Interpersonal Reactivity Index scales in correlation with gray matter atrophy using a voxel-based morphometry approach. Lower levels of empathy were associated with an increased atrophy in fronto-temporal cortical structures. At subc…

Physician Assistant Student Attitudes and Empathy Following Virtual Immersion.

BACKGROUND: Addressing provider attitudes associated with diabetes and social determinants of health (SDH) can help improve a patient's overall health and wellbeing. Empathy can improve patient care and outcomes. Cinematic virtual reality (Cine-VR) has been shown to positively affect student empathy. The purpose of this study was to measure and compare empathy and diabetes attitudes among physician assistant students following immersion in 12 virtual-reality video modules. METHODS: Students (n=57) completed the Diabetes Attitude Scale-version 3 (DAS-3) and Jefferson Empathy Scale (JES) before…

Incidental fear reduces empathy for an out-group's pain.

Humans generally fear those different to them (i.e., an out-group) in the same way they fear natural predators. But fear pushes us to derogate others, whether they constitute a threat or not. Research has examined how fear associated with specific intergroup relations interferes with how individuals relate to in-group and out-group members. However, we know relatively little about how intergroup relations might be affected by incidental emotions. We tested how incidental fear affects empathy toward in-group and out-group members. We found that exposing participants to fearful imagery was suffi…

Therapist empathy and client outcome: An updated meta-analysis.

Put simply, empathy refers to understanding what another person is experiencing or trying to express. Therapist empathy has a long history as a hypothesized key change process in psychotherapy. We begin by discussing definitional issues and presenting an integrative definition. We then review measures of therapist empathy, including the conceptual problem of separating empathy from other relationship variables. We follow this with clinical examples illustrating different forms of therapist empathy and empathic response modes. The core of our review is a meta-analysis of research on the relatio…

Alexithymia, emotional empathy, and self-regulation in anorexia nervosa.

BACKGROUND: Individuals with anorexia nervosa (AN) who are starved have poor awareness (alexithymia), reduced understanding of others' mental states (cognitive empathy), and difficulty regulating personal emotions (self-regulation). Despite its important role in social interaction, sympathy for others (emotional empathy) has not been measured in AN. Furthermore, it is unknown how restoring weight affects the relationship among alexithymia, empathy, and self-regulation in AN. METHODS: Women with AN were tested longitudinally during their starvation period (N = 26) and after weight was restored…

Marital quality, forgiveness, empathy, and rumination: a longitudinal analysis.

McCullough, Rachal, et al.'s (1998) social-psychological framework of forgiveness informed a longitudinal study that examined the extent to which marital forgiveness is determined by social-cognitive (the offended spouse's rumination and emotional empathy) and relationship variables (the quality of the relationship in which the offense took place). In the study, 119 husbands and 124 wives from long- and medium-term marriages in north Italy provided data at two time points separated by a 6-month interval. Structural equation models showed that rumination and empathy independently predicted conc…

Intention to Stop Bullying following a Condemning, Empathy-Raising, or Combined Message from a Teacher - Do Students' Empathy and Callous-Unemotional Traits Matter?

Knowing which intervention strategies work best and for which student is essential for teachers when they intervene in cases of bullying. The effects of teachers' (1) condemning, (2) empathy-raising, and (3) combined (including elements of both) messages on students' intention to stop bullying were tested in a between-subject experimental design. A total of 277 seventh grade students (Mage = 12.93, SD = 0.49; 47% female) were asked to imagine they had bullied a peer and were invited to a discussion with a teacher. They saw a video vignette with one of the above messages. Hierarchical regressio…
