Chronic kidney disease of nontraditional etiology in Central America: a provisional epidemiologic case definition for surveillance and epidemiologic studies.

SYNOPSIS Over the last two decades, experts have reported a rising number of deaths caused by chronic kidney disease (CKD) along the Pacific coast of Central America, from southern Mexico to Costa Rica. However, this specific disease is not associated with traditional causes of CKD, such as aging, diabetes, or hypertension. Rather, this disease is a chronic interstitial nephritis termed chronic kidney disease of nontraditional etiology (CKDnT). According to the Pan American Health Organization (PAHO) mortality database, there are elevated rates of deaths related to kidney disease in many of th

Allostatic load and cancer risk, progression, and mortality in epidemiologic studies.

The current body of evidence from both preclinical and epidemiologic studies suggests that cumulative stress may play a role in carcinogenesis; however, few studies on human populations have examined allostatic load (AL) as it relates to cancer risk, progression, and mortality. In this commentary, we discuss the use and value of AL as a measure to assess exposure to stress for cancer epidemiologic studies, describe the existing evidence between AL and sources of cancer-related environmental stressors, and summarize the existing epidemiologic studies on AL and cancer risk, progression, and mort…

Why we need epidemiologic studies of polycystic ovary syndrome in Africa.

The primary objective of the Ghana Polycystic Ovary Syndrome Epidemiology and Phenotype (Ghana-PEP) study will be to assess the relevance and phenotypic distribution of polycystic ovarian syndrome (PCOS) in a medically unbiased population of reproductive-aged women. In addition, the study will also attempt to identify sociodemographic, environmental, and psychological factors that may play a role in the development of PCOS phenotype. The study aims to recruit 990 randomly selected women aged 18-45 years living in Nsawam, the district capital of the Nsawam-Adoagyiri Municipality, in the Eastern

Gestational age and childhood leukemia: A meta-analysis of epidemiologic studies.

OBJECTIVE: An increasing amount of evidence shows that childhood leukemia is initiated in utero. Birth characteristics initiated in utero, such as gestational age, may play a role in leukemogenesis. The purpose of our meta-analysis is to explore the association between gestational age and childhood leukemia. METHODS: Relevant studies up to 21 April 2017 were collected by searching PubMed and EMBASE databases. Subgroup analysis, sensitivity analysis and publication bias assessment were conducted. RESULTS: A total of 13 studies were included. Pooled odds ratios (ORs) with 95% confidence interv

Large epidemiologic studies of gout: challenges in diagnosis and diagnostic criteria.

Large epidemiologic studies of gout can improve insight into the etiology, pathology, impact, and management of the disease. Identification of monosodium urate monohydrate crystals is considered the gold standard for diagnosis, but its application is often not possible in large studies. Therefore, under such circumstances, several proxy approaches are used to classify patients as having gout, including ICD coding in several types of databases or questionnaires that are usually based on the existing classification criteria. However, agreement among these methods is disappointing. Moreover, stud…

Prenatal infection and schizophrenia: a review of epidemiologic and translational studies.

An emerging literature from epidemiologic, clinical, and preclinical investigations has provided evidence that gestational exposure to infection contributes to the etiology of schizophrenia. In recent years, these studies have moved from ecologic designs, which ascertain infection based on epidemics in populations, to investigations that have capitalized on reliable biomarkers in individual pregnancies. These studies have documented specific candidate infections that appear to be associated with an elevated risk of schizophrenia. Animal models of maternal immune activation inspired by this wor

Ecstasy and other club drugs: a review of recent epidemiologic studies.

PURPOSE OF REVIEW: This review highlights the epidemiologic research on club drug use in the past year, with a focus on clinical epidemiology, social epidemiology, new methodological approaches, and alternative explanations for drug use behaviors. RECENT FINDINGS: Although 3,4-methylenedioxy-N-methylamphetamine (MDMA) or ecstasy is currently classified as a type of hallucinogen and its withdrawal is not recognized in the Diagnostic and Statistical Manual of Mental Disorders, there is evidence for the association of withdrawal symptoms with MDMA abstinence. Findings from latent class analysis

Contact lens-related microbial keratitis: what can epidemiologic studies tell us?

Since the publication of large-scale population based studies to estimate the risks of contact lens-related microbial keratitis, the lens types and modes of wear have changed considerably, with the introduction of silicone hydrogel contact lenses for continuous wear of up to 30 nights and daily disposable contact lenses. It is time to consider epidemiologic studies to estimate the absolute and relative risk of microbial keratitis associated with these new exposures. This article summarizes current knowledge of the estimates and determinants of risk associated with re-useable hydrogel lenses an…

Comorbidity and social phobia: evidence from clinical, epidemiologic, and genetic studies.

This paper reviews evidence from clinical, epidemiologic, and family studies regarding the association between social phobia and other syndromes. Social phobia is strongly associated with other anxiety disorders, substance abuse, and affective disorders in both clinical and community samples. An average of 80% of social phobics identified in community samples meet diagnostic criteria for another lifetime condition. Social phobia is most strongly associated with other subtypes of anxiety disorders, with an average of 50% of social phobics in the community reporting a concomitant anxiety disorde

Results of oral contraceptive epidemiologic studies regarding neoplastic and cardiovascular effects.

Over the past three decades, much of the research on oral contraceptives has focused on cardiovascular and neoplastic effects. Results of recent United States studies have shown no increased risk of death among users of oral contraceptives, although an increased risk of idiopathic venous thromboembolism has been a consistent finding. There is compelling evidence that oral contraceptive use provides significant protection against endometrial and ovarian cancer, and that this protection lasts at least 15 years. In addition, no clear association has been found between oral contraceptive use and b

Pelvic inflammatory disease, intrauterine contraception, and the conduct of epidemiologic studies.

Pelvic inflammatory disease (PID) has been described in the medical literature for more than a century as a specific entity. Neisseria gonorrhoeae, Chlamydia trachomatis, and Mycoplasma hominis, along with Bacteroides fragilis and other anaerobic bacteria, have been most frequently associated with PID. Factors affecting the occurrence of PID have been extensively studied during the past two decades and include number of sexual partners, age, race, socioeconomic status, education, and contraceptive method. As knowledge concerning factors that contribute to PID increases, epidemiologic studies a…

A semiquantitative score system for epidemiologic studies of fetal alcohol syndrome.

A total of 464 children of 323 women registered for alcoholism treatment in Budapest, Hungary, were studied in 1977-1979. A complex epidemiologic investigation was carried out using medical, psychological and anthropological data. The data were evaluated on the basis of a semiquantitative diagnostic scoring system for fetal alcohol syndrome. The score distribution curve for 301 children whose mothers imbibed during pregnancy was statistically significantly different from the score distribution curve for 163 children born to alcoholic mothers who remained abstinent during pregnancy and from the
