Autochthonous dengue transmission in Europe: epidemiology, mechanisms, and modelling insights.

BACKGROUND: Dengue is the most rapidly expanding arboviral infection globally, with rising incidence and widening geographic distribution driven by urbanisation, climate change, vector expansion, and global mobility. Although traditionally confined to tropical and subtropical regions, dengue is increasingly emerging in temperate settings, including Europe. Recent outbreaks in Italy, Spain, and France, together with evidence of under-recognised transmission, indicate a transition from sporadic importation to recurrent autochthonous infection. This shift is facilitated by the establishment of Ae

An immune memory-structured SIS epidemiological model for hyperdiverse pathogens.

A hyperdiverse class of pathogens of humans and wildlife, including the malaria parasite Plasmodium falciparum, relies on multigene families to encode antigenic variation. As a result, high (asymptomatic) prevalence is observed despite high immunity in local populations under high-transmission settings. The vast diversity of "strains" and genes encoding this variation challenges the application of established models for the population dynamics of such infectious diseases. Agent-based models have been formulated to address theory on strain coexistence and structure, but their complexity can lim

Mitochondrial Cardiomyopathy: Molecular Epidemiology, Diagnosis, Models, and Therapeutic Management.

Mitochondrial cardiomyopathy (MCM) is characterized by abnormal heart-muscle structure and function, caused by mutations in the nuclear genome or mitochondrial DNA. The heterogeneity of gene mutations and various clinical presentations in patients with cardiomyopathy make its diagnosis, molecular mechanism, and therapeutics great challenges. This review describes the molecular epidemiology of MCM and its clinical features, reviews the promising diagnostic tests applied for mitochondrial diseases and cardiomyopathies, and details the animal and cellular models used for modeling cardiomyopathy a

Measuring differences between phenomenological growth models applied to epidemiology.

Phenomenological growth models (PGMs) provide a framework for characterizing epidemic trajectories, estimating key transmission parameters, gaining insight into the contribution of various transmission pathways, and providing long-term and short-term forecasts. Such models only require a small number of parameters to describe epidemic growth patterns. They can be expressed by an ordinary differential equation (ODE) of the type C'(t)=f(t,C;Θ) for t>0, C(0)=C0, where t is time, C(t) is the total size of the epidemic (the cumulative number of cases) at time t, C0 is the initial number of cases, f

Modeling the potential impact of vaccination on the epidemiology of congenital cytomegalovirus infection.

BACKGROUND: Understanding the potential for vaccination to change cytomegalovirus (CMV) epidemiology is important for developing CMV vaccines and designing clinical trials. METHODS: We constructed a deterministic, age-specific and time-dependent mathematical model of pathogen transmission, parameterized using CMV seroprevalence from the United States and Brazil, to predict the impact of vaccination on congenital CMV infection. FINDINGS: Concurrent vaccination of young children and adolescents would result in the greatest reductions in congenital CMV infections in populations with moderate an

Creating a process for incorporating epidemiological modelling into outbreak management decisions.

Modern computational models of infectious diseases greatly enhance our ability to understand new infectious threats and assess the effects of different interventions. The recently-released CDC Framework for Preventing Infectious Diseases calls for increased use of predictive modelling of epidemic emergence for public health preparedness. Currently, the utility of these technologies in preparedness and response to outbreaks is limited by gaps between modelling output and information requirements for incident management. The authors propose an operational structure that will facilitate integrati

Studies on Prn variation in the mouse model and comparison with epidemiological data.

The virulence factor pertactin (Prn) is a component of pertussis vaccines and one of the most polymorphic Bordetella pertussis antigens. After the introduction of vaccination shifts in predominant Prn types were observed and strains with the Prn vaccine type (Prn1) were replaced by strains carrying non-vaccine types (Prn2 and Prn3), suggesting vaccine-driven selection. The aim of this study was to elucidate the shifts observed in Prn variants. We show that, although Prn2 and Prn3 circulated in similar frequencies in the 1970s and 1980s, in the 1990s Prn2 strains expanded and Prn3 strains disap

Monitoring epidemiologic surveillance data using hidden Markov models.

The analysis of routinely collected surveillance data is an important challenge in public health practice. We present a method based on a hidden Markov model for monitoring such time series. The model characterizes the sequence of measurements by assuming that its probability density function depends on the state of an underlying Markov chain. The parameter vector includes distribution parameters and transition probabilities between the states. Maximum likelihood estimates are obtained with a modified EM algorithm. Extensions are provided to take into account trend and seasonality in the data.

A random effects model for some epidemiological features of dental caries.

We describe a random effects model for caries lesion development and progression based on considering the effects of the pH fluctuations over time in microbial dental plaque as a Wiener process with a single absorptive barrier. The model predicts that the period of greatest risk to developing caries occurs shortly after eruption, but thereafter the longer a surface survives without developing a lesion, the less likely will it be that a lesion will subsequently develop. The model is able to anticipate why the effect of water fluoridation on caries prevalence is most pronounced when caries is di

Utility of epidemiologic simulation models in the planning of trypanosomiasis control programs.

To understand the behavior of complex epidemiologic systems, it is useful to devise computer based simulation models by approximating the interactions via biomathematical expressions. Without doubt, these models could be over simplifications of complex interactions but they could be useful in comparison to classical laboratory and/or field experimental approaches which may not be practical or feasible. In the past obtaining solutions to biomathematical equations with any degree of complexity has been impossible. However, the availability of powerful computers has now made the quantitative anal

A Dual-age Structured Epidemiological Model with Waning Immunity and Reinfection.

Waning immunity and reinfection are critical features of many infectious diseases, but epidemiological models often fail to capture the interaction between an individual's immunity history and their current infection status, or do so only simplistically. We develop a dual-age structured model tracking immunity age (time since last recovery) and infection age (time since infection) to analyze epidemic dynamics under waning immunity and reinfection. The model is formulated as a system of age-structured partial differential equations describing susceptible and infected populations stratified by b…

Modelling assessment of epidemiological strategies based on self-screening tools.

BACKGROUND: To reach 2030 targets, the World Health Organization specifically recommends test-and-treat strategies to eliminate HIV and bacterial STI. Mathematical models identified the required coverages for the success of these interventions. However, they appear out of reach for current testing protocols based on medical recommendations. Still, promoting voluntary testing can effectively increase testing rates if individuals perceive high benefits associated to frequent testing. Design of multi-disease voluntary testing kits remains a key strategic tool to reach various epidemiological goal

Multi-Study Factor Regression Model: An Application in Nutritional Epidemiology.

Diet is a risk factor for many diseases. In nutritional epidemiology, studying reproducible dietary patterns is critical to reveal important associations with health. However, this task is challenging: diverse cultural and ethnic backgrounds may critically impact eating patterns by showing heterogeneity, leading to incorrect dietary patterns and obscuring the components shared across different groups or populations. Moreover, covariate effects generated from observed variables, such as demographics and other confounders, can further bias these dietary patterns. Identifying the shared and group

Congruity of genomic and epidemiological data in modelling of local cholera outbreaks.

Cholera continues to be a global health threat. Understanding how cholera spreads between locations is fundamental to the rational, evidence-based design of intervention and control efforts. Traditionally, cholera transmission models have used cholera case-count data. More recently, whole-genome sequence data have qualitatively described cholera transmission. Integrating these data streams may provide much more accurate models of cholera spread; however, no systematic analyses have been performed so far to compare traditional case-count models to the phylodynamic models from genomic data for c

A description of the epidemiological dynamics of Chagas disease via mathematical modeling.

Chagas disease is caused by the protozoan Trypanosoma cruzi, which parasitizes many mammals, including humans. Its vectors are blood-feeding hematophagous triatomine insects of different species, which vary according to the geographical area. One of the 17 neglected diseases targeted by the World Health Organization, Chagas disease is endemic to the Americas, but has spread to other countries due to human migratory movements. In this study, we describe the epidemiological dynamics of Chagas disease in an endemic area, considering the main transmission mechanisms and the demographic effects of

Rich dynamics of a bidirectionally linked immuno-epidemiological model for cholera.

Cholera is an environmentally driven disease where the human hosts both ingest the pathogen from polluted environment and shed the pathogen to the environment, generating a two-way feedback cycle. In this paper, we propose a bidirectionally linked immuno-epidemiological model to study the interaction of within- and between-host cholera dynamics. We conduct a rigorous analysis for this multiscale model, with a focus on the stability and bifurcation properties of each feasible equilibrium. We find that the parameter that represents the bidirectional connection is a key factor in shaping the rich
