Reflections on the knowledge base for obstetric fistula.

This article presents the reflections of an experienced fistula surgeon and an epidemiologist on the current knowledge base for obstetric fistula. The incidence, prevention, and management of vesico-vaginal and recto-vaginal fistula are discussed. The authors call for more randomized controlled trials to determine the effectiveness of surgical interventions for fistula repair.

Oscar Costa-Mandry and posthurricane bacillary dysentery.

After the recent hurricanes Katrina and Rita, outbreaks of waterborne viral and bacterial diseases were expected. Dr. Oscar Costa-Mandry, a Puerto Rican bacteriologist and epidemiologist in the 1930s, identified the relationship between hurricanes and epidemic bacillary dysentery in townships that were directly affected by hurricanes.

[Ageing and cancer: association or causality?].

Epidemiologists are interested in differences among populations, and in their composition with regard age distribution. The definition of elderly is strongly dependent upon the period and the social context. In this contribution the main epidemiological aspects of cancer in the elderly are discussed, keeping in mind that the ageing process is associated with cancer but does not cause it. double dagger.

Evaluation of knowledge resources for public health reporting logic: Implications for knowledge authoring and management.

To control disease, laboratories and providers are required to report conditions to public health authorities. Reporting logic is defined in … c hepatitis B (64%), acute hepatitis C (96%), and past and present hepatitis C (96%). Public health epidemiologists have the need to communicate parameters other than just the name of a disease or organism that should be reported, such as the status and specimen sources. Existing knowledge resources should be integrated, harmonized and made computable. Our findin …

The Museum of the History of Health Care in Croatia, as the first such museum in the southeastern part of Europe, was established by the Croatian Medical Association in Zagreb in 1944. Beside Vladimir Ćepulić (1891 - 1964) the head of the Croatian Medical Association, epidemiologist Stanko Sielski (1891 - 1958), was one of the most prominent personalities to be credited for realizing this project. He was born in Gracanica into a family of Polish origin. After his graduation in Vienna in 1919, he worked as an epidemiologist in Konjic, Prozor, Glamoc and other places in the area of Bosnia, mostl

A listeriosis patient infected with two different Listeria monocytogenes strains.

Normally, only one isolate of Listeria monocytogenes from a case of listeriosis is subjected to characterization. Here we show that two isolates from different sites of the body were not the same strain. Such a phenomenon may not have any clinical relevance, although it may confuse the epidemiologist trying to match infection source with infection target.

A perspective on respiratory disease in feedlot cattle.

The etiological factors causing acute pneumonia in feedlot cattle are complex in terms of their interactions with the host to cause clinical disease or death. The most significant agent is Pasteurella hemolytica, but it may be acting relatively late in the overall sequence of events. It is necessary to develop an overall strategy to determine the pathogenesis of the disease in the field. Approaches to this objective are discussed, with the emphasis on the role of the epidemiologist.

Epidemiology and epidemiologic methods: an introduction for respiratory care practitioners.

Epidemiology is the study of the distribution of disease in a population and of the factors that determine that distribution. Respiratory care practitioners encounter many aspects of epidemiology in training and practice and should become aware that most initial epidemiologic observations are made by clinicians. The determinations that excessive oxygen administration caused retrolental fibroplasia and that exposure to cotton bracts in the textile industry caused byssinosis resulted from the collaborative efforts of epidemiologists, laboratory workers, and clinicians. The three types of study d…

Strengthening global science: The role of the International Network for Epidemiology in Policy (INEP) in bridging science and policy.

Gaps between epidemiological evidence and public policy remain a persistent challenge in public health. Despite increasing data availability, and public awareness, evidence often fails to translate into timely or equitable policy action. In this Commentary, we argue that epidemiologists must come together to support the integrity of evidence-informed public health policy making. As an example, we describe the International Network for Epidemiology in Policy (INEP). INEP was established in 2006 to address these gaps by promoting the more ethical and effective use of epidemiological evidence in …

Scientific standards of criticism: a reaction to "Scientific standards in epidemiologic studies of the menace of daily life," by A.R. Feinstein.

Feinstein's criticism of epidemiology are summarized and evaluated. His major premises appear to be that methodologic errors inevitably result in false positive associations, that epidemiologists are complacent about methodology, and that epidemiologists encourage overinterpretation of epidemiologic data by the public. We challenge these premises and discuss a number of technical errors that underlie Feinstein's writings: he generally ignores the tendency of nondifferential exposure misclassification to dilute associations; he inappropriately interchanges measure of disease occurrence, which l…

Dynamic changes in the epidemiology of canicola fever in Israel. Natural adaptation of an established serotype to a new reservoir host.

Investigation of human leptospirosis caused by the serotype canicola revealed a significant shift in the chain of infection of this disease. The adaptation of an old and well-established serotype to a new and abundant host, the Norway rat, provides a new means for spreading and transferring canicola fever to man and farm animals. The changes in the epidemiology of leptospirosis described should serve as a warning to epidemiologists responsible for suggesting proper prophylactic measures.

Controversies in cancer epidemiology.

The histories of three controversies are described: fluoridation of water supplies and the risk of cancer; whether oral contraceptives cause breast cancer; and the role of self-examination in the prevention of cancer. For the future, some new controversies are already predictable. As well as the scientific debate around these topics, the roles of politics, the media, and pressure groups are discussed. The epidemiologist's responsibility to ensure that the correct interpretation of data is made clear to the public is considered.
