The self-management support needs of people diagnosed with psoriatic arthritis: a realist review protocol.

INTRODUCTION: Psoriatic arthritis (PsA) is a form of inflammatory arthritis linked to psoriasis. Previous research from the UK has found that many people feel unsupported when diagnosed with PsA and lack confidence in managing their condition. This realist review aims to understand what works and does not work for whom and in what circumstances, in relation to healthcare professionals engaging with people to support them in developing self-management skills. METHODS AND ANALYSIS: This protocol was developed by defining the scope of the review, using a brief directed literature review to suppo

Health Research Ethics: Between Ethics Codes and Culture.

This article is meant to describe and analyze some of the ethical difficulties encountered in a pilot research on treatment decisions of patients with chronic viral hepatitis C infection in Romania. It departs from an overview of the main ethics codes, and it shows that social health research on patients falls in between institutional codes of ethics. Furthermore, the article moves on to analyze so-called "important moments" of empirical research, such as the implementation of the ethical protocol, dealing with informal payments and with information on shady actions, as well as requests of inf

Epigenetics, Structural Determinants, and Race: An Integrated Conceptual Framework for Embodied Stress, Resilience, and Black Women's Cardiometabolic Health.

By integrating epigenetic science using a multidisciplinary approach, grounded in nursing theory, this concept paper offers a transformative frontier for understanding and addressing cardiometabolic health inequities experienced by Black women. Building on the legacy and insights of the Black Women's Health Study and drawing on the Social Ecological Model, the Neuman Systems Model, and the weathering hypothesis, we present a conceptual framework that elucidates how systemic drivers of health shape cardiometabolic health outcomes through psychosocial pathways that mediate epigenetic changes. When Subtle Neurologic Changes Speak: A Reflective Narrative of Central Cord Syndrome After Anterior Cervical Discectomy and Fusion.

This reflective narrative explores the lived experience of developing central cord syndrome (CCS) following a postoperative spinal epidural hematoma after anterior cervical discectomy and fusion and examines implications for holistic nursing practice and education. Grounded in Watson's Theory of Human Caring, this reflective narrative examines lived experience through a caring science lens. Findings indicate that early postoperative neurologic symptoms were initially minimized, contributing to delayed escalation and emergent surgical intervention for hematoma evacuation. The subsequent course

The virtues (and vices) of the four principles.

Despite tendencies to compete for a prime place in moral theory, neither virtue ethics nor the four principles approach should claim to be superior to, or logically prior to, the other. Together they provide a more adequate account of the moral life than either can offer on its own. The virtues of principlism are clarity, simplicity and (to some extent) universality. These are well illustrated by Ranaan Gillon's masterly analysis of the cases he has provided. But the vices of this approach are the converse of its virtues: neglect of emotional and personal factors, oversimplification of the iss

The use of anencephalic organs: historical and ethical dimensions.

The condition of newborn infants with anencephaly, a neural tube defect, is incurable and uniformly fatal. Although physicians reached a consensus two decades ago on the appropriateness of using these infants' organs, ethical and legal questioning has since challenged the grounds on which medical authorities justified transplantation. Advocates have proposed three conceptual strategies to warrant procuring anencephalics' organs: redefining death, excluding the infants from possessing personhood, and intubating and ventilating them while keeping a vigil for brain death. Each of these conceptual

Artificial Intelligence Connectedness: Theoretical Reconstruction of Connectedness and Its Impacts on Adolescent Mental Health.

The widespread penetration of generative artificial intelligence is reshaping adolescents' social ecosystems and emotional experiences, while challenging the interpretive boundaries of traditional connectedness theories. Following the logical path of "connotation reconstruction-extension transformation-concept construction", this study integrates the ethics of care and neo-ecological theory to systematically construct a theoretical framework of artificial intelligence connectedness. First, tracing theories across philosophy, sociology, and psychology, this research reconstructs the core connot

Instrumental harm and impartial beneficence distinctively frame cognitive representations of moral decision problems.

Utilitarian ethical theories argue that the morality of actions depends on their consequences for impartially maximizing overall welfare. Recent research suggests that individual differences in utilitarian tendencies fall along two dimensions: a permissive attitude toward harming others for greater good (instrumental harm [IH]) and an impartial concern for others' welfare (impartial beneficence [IB]). We hypothesize that these dimensions operate as intuitive theories in the moral domain, framing distinctive patterns of moral judgments and behavior. Using intersubject representational similarit

Androgenic morality? Associations of sex, oral contraceptive use and basal testosterone levels with moral decision making.

Aside from cultural, psychological, or situational factors, differences in moral judgements might also be influenced by biological variables. Since previous studies have reported stronger utilitarian tendencies in men, the relationship between testosterone and moral judgments has gained interest. Utilitarian judgements focus on the consequences of an action in terms of a cost-benefit analysis while deontological judgements are based on rules that are independent of an action's outcome or of situational features. We investigated decisions in moral dilemma situations in N = 157 young adults usin

'To arrive where we started, and know the place for the first time': Heidegger, phenomenology, the way human beings first appear in the world, and fresh perspectives on the abortion debate.

Intellectual stalemate in the abortion debate can be traced in part to its being framed as a standoff between religion and secular philosophy. While the former is thought to generate a broadly 'pro-life' position, the latter is associated with more 'pro-choice' thinking. This essay attempts to break free of this framing by criticising the philosophy informing 'pro-choice' positions, but not by resorting immediately to religious arguments but rather by drawing upon a rival philosophical tradition--the movement within twentieth and twenty-first Continental philosophy which was and is phenomenolo

How medical ethical principles are applied in treatment with artificial insemination by donors (AID) in Hunan, China: effective practice at the Reproductive and Genetic Hospital of CITIC-Xiangya.

This paper investigates the efficiency of application of medical ethics principles in the practice of artificial insemination by donors (AID) in China, in a culture characterised by traditional ethical values and disapproval of AID. The paper presents the ethical approach to AID treatment as established by the Reproduction and Genetics Hospital of CITIC-Xiangya (CITIC Hunan-Yale Approach) in the central southern area of China against the social ethical background of China and describes its general features. The CITIC-Xiangya Approach facilitates the implementation of ethical relations between

Bridging Theory and Practice in Emergency Nursing: A Discussion Paper on Peripheral intravenous catheter Management Using Tanner's and Chinn & Kramer's Models.

INTRODUCTION: Nursing management of peripheral intravenous catheters remains suboptimal and often lacks integration with underpinning theory. Therefore, it is essential to reflect on the persistent gap between theory and practice to enhance patients' outcomes in this area. OBJECTIVES: To conceptually evaluate the integration of Tanner's Clinical Judgment Model using Chinn and Kramer's criteria, and to explore the integration of these two complementary frameworks with Kramer's Ways of Knowing for peripheral intravenous catheter decision-making and patient outcomes improvement in the emergency

Principle-based structured case discussions: do they foster moral competence in medical students? - A pilot study.

BACKGROUND: Recent findings suggest that medical students' moral competence decreases throughout medical school. This pilot study gives preliminary insights into the effects of two educational interventions in ethics classes on moral competence among medical students in Munich, Germany. METHODS: Between 2012 and 2013, medical students were tested using Lind's Moral Competence Test (MCT) prior to and after completing different ethics classes. The experimental group (EG, N = 76) participated in principle-based structured case discussions (PBSCDs) and was compared with a control group with theor

On genies and bottles: scientists' moral responsibility and dangerous technology R&D.

The age-old maxim of scientists whose work has resulted in deadly or dangerous technologies is: scientists are not to blame, but rather technologists and politicians must be morally culpable for the uses of science. As new technologies threaten not just populations but species and biospheres, scientists should reassess their moral culpability when researching fields whose impact may be catastrophic. Looking at real-world examples such as smallpox research and the Australian "mousepox trick", and considering fictional or future technologies like Kurt Vonnegut's "ice-nine" from Cat's Cradle, and

Anencephalics as organ sources.

In recent years, the need for infant organs for transplantation has increased. There is a growing recognition of the potential use of anencephalics as sources of organs. Prevalent arguments defending the use of live anencephalics for organ sources are identified and criticized. I argue that attempts to deny the applicability of the "dead-donor rule" are either question-begging or based on false premises and that attempts to skirt the Kantian dictum against treating others as a means only are not successful. I contend that the apparent utilitarian justification for live anencephalics as organ s

A theory of international bioethics: the negotiable and the non-negotiable.

The preceding article in this issue of the Kennedy Institute of Ethics Journal presents the argument that "moral fundamentalism," the position that international bioethics rests on "basic" or "fundamental" moral prinicples that are universally accepted in all eras and cultures, collapses under a variety of multicultural and postmodern critiques. The present article looks to the contractarian tradition of Hobbes and Locke -- as reinterpreted by David Gauthier, Robert Nozick, and John Rawls -- for an alternative justification for international bioethics. Drawing on the central themes of this tra

Developing middle-range theories for the implementation of clinical practice guideline recommendations for pre-eclampsia prevention and management in eastern, central and southern Africa: a realist synthesis protocol.

INTRODUCTION: Hypertensive disorders of pregnancy, such as pre-eclampsia (PE), are one of the leading causes of maternal and perinatal deaths in low- and middle-income countries (LMICs). Although clinical practice guidelines (CPGs) for PE prevention and management are available, there is limited information on their implementation in LMIC contexts. This realist synthesis therefore aims to uncover the causal explanations underpinning the implementation of CPG recommendations for PE prevention and management in Eastern, Central and Southern African contexts. By developing and refining initial pr

[Nursing ethics and the access to nursing care].

The increasing number of ethical issues highlighted in everyday nursing care demonstrates the connectedness between nursing ethics and nursing practice. However, what is the role of ethical theories in this context? This question will be examined in this article by analysing the contribution made by the ethics of care, in particular in understandings of gender roles, asymmetries of power, professional knowledge and experience. The adoption and criticism of an emergent nursing ethics is discussed and stated from different viewpoints. The actuality of the caring approach is affirmed by a new rea

Beyond Principles: A Reflective-Cognitive Framework for Ethical Decision-Making in Anorexia Nervosa.

Anorexia nervosa is a clinically complex and ethically challenging psychiatric disorder. Clinicians are frequently confronted with ethical dilemmas arising from conflicts between core ethical principles in everyday clinical practice. Professional codes of ethics and legal frameworks often fail to provide a stable basis for resolving these dilemmas due to the fluctuating medical risk and the ego-syntonic nature of anorexia nervosa. Under conditions of heightened responsibility and medical risk, clinicians' cognitive and emotional responses may be activated and may mediate ethical decision-makin
