Factors Associated with Previable Delivery following Second Trimester Rupture of Membranes.

OBJECTIVE: To identify factors associated with previable delivery in second trimester preterm rupture of membranes (PROM). STUDY DESIGN: We conducted a single-center retrospective cohort study of women with pregnancies complicated by second trimester PROM (14.0-21.9 weeks' gestation) from 2000 to 2015 who elected expectant pregnancy management and achieved at least 24 hours latency. Maternal characteristics and clinical factors were compared among pregnancies that reached viability (≥ 23.0 weeks) and pregnancies delivered before viability (< 23.0 weeks) using appropriate statistical methods.

Ethics: "life before birth" and moral complexity in maternal-fetal surgery for spina bifida.

This article considers the ethical significance of a moral belief common among pregnant women (and their partners) who seek open uterine repair for fetal spina, namely that their fetuses are already "babies." The need to recognize and interact sensitively with a pregnant woman's vulnerability to her own beliefs and concerns regarding potential disabilities, the fetal intervention, and its potential outcome is emphasized. Such recognition and explicit discussions are ethically important for informed consent and to safeguard against the judgments, enthusiasms, and biases of surgeons and other te

Managing Monoamniotic Twin Pregnancies.

Monoamniotic twins comprise a rare subset of twins at risk of unique and serious complications. In addition to the risks faced by all twins (premature birth, growth restriction), all monochorionic twins (twin-to-twin transfusion syndrome), and all monozygotic twins (congenital anomalies), monoamniotic twins face the unique risk of cord entanglement, in addition to a markedly increased risk of congenital anomalies. Early diagnosis, screening for fetal anomalies and surveillance for twin-twin transfusion syndrome are critical. After fetal viability, frequent fetal monitoring reduces the risk of

Supraventricular tachycardia in one of the twins: The ethical dilemmas involved in treatment.

The pediatric cardiologist now has an important role to play in fetal medicine. They are often called upon to manage fetal cardiac problems such as arrhythmias or perform fetal cardiac interventions such as balloon valvuloplasty or atrial septostomy. In these scenarios, it becomes very important for the pediatric cardiologist to understand the concepts of "fetus as a patient," "viability," etc., and their implications in management. We try to shed light on these principles through our case scenario of managing supraventricular tachycardia in one of the fetuses of a twin pregnancy.

The definition of life: a survey of obstetricians and neonatologists in New York City hospitals regarding extremely premature births.

Among obstetricians and neonatologists in administrative roles in New York City hospitals, a survey was initiated to compare the physicians' definitions of live birth and fetal death, the gestational age at which they consider infants viable, and the resuscitation practices of the neonatologists. The target survey population was 34 neonatologists, and 39 obstetricians representing 41 of the City's 43 maternity hospitals. A telephone survey was used to gather qualitative data from the physicians regarding their definitions of live birth, fetal death, and viability, and their practices regarding

ZIKV Strains Differentially Affect Survival of Human Fetal Astrocytes versus Neurons and Traffic of ZIKV-Laden Endocytotic Compartments.

Malformations of the fetal CNS, known as microcephaly, have been linked to Zika virus (ZIKV) infection. Here, the responses of mammalian and mosquito cell lines, in addition to primary human fetal astrocytes and neurons were studied following infection by ZIKV strains Brazil 2016 (ZIKV-BR), French Polynesia 2013 (ZIKV-FP), and Uganda #976 1947 (ZIKV-UG). Viral production, cell viability, infectivity rate, and mobility of endocytotic ZIKV-laden vesicles were compared. All cell types (SK-N-SH, Vero E6, C6/36, human fetal astrocytes and human fetal neurons) released productive virus. Among primar

The use of pulsatile perfusion during aortic valve replacement in pregnancy.

Cardiac operations are occasionally required during pregnancy. Despite a low maternal mortality, fetal mortality remains high. Previous reports have suggested maintenance of high perfusion pressure and flow rate as protective measures to maintain fetal viability. Recent experimental data suggest pulsatile perfusion may help preserve placental hemodynamic function. The successful use of pulsatile bypass to replace the aortic valve in a 25-year-old female at 14 weeks gestation, with both maternal and fetal survival, is presented.

Surgical resolution of an ectopic pregnancy in a captive gerenuk (Litocranius walleri walleri).

A full-term ectopic pregnancy was diagnosed in a multiparous, captive gerenuk (Litocranius walleri walleri) following prolonged gestation without parturition. Until 213 days of gestation, fetal viability was determined by transabdominal ultrasound of fetal heartbeat, physical palpation, and visual observations of fetal movements. Survey abdominal radiographs obtained at day 229 of gestation revealed a fully mineralized fetal skeleton in an inappropriate position for parturition. Surgical exploration revealed a full-term, nonviable, male calf contained within fetal membranes attached to the ome

Chloroacetonitrile induces intrauterine growth restriction and musculoskeletal toxicity in fetal mouse.

Chloroacetonitrile (CAN) is a by-product of chlorination of drinking water. Epidemiological studies indicate that it might present a hazard to human health. The present study was designed to investigate the potential adverse effects of intrauterine exposure to CAN on fetal body weight and development of the musculoskeletal system in mice. At gestation day 6, pregnant mice were given CAN (12.5, 25, or 50 mg/kg/day) till gestation day 18. Uteri were then examined and live fetuses were collected, weighed, and evaluated for any malformations. High doses of CAN (50 mg/kg) significantly elevated fet

Assessment of adult skeletons to detect prenatal exposure to trypan blue in mice.

A Skeletal Variant Assay System (SVAS) consisting of a group of 88 spontaneously occurring qualitative variations of the adult mouse skeleton was studied in CD-1 mice which had been exposed in utero by way of three daily ip injections of their dams on days 7-9 of gestation with trypan blue. Treatment groups received daily doses of 0.25 cc of 0, .037, .075, .15, or .30% trypan blue dissolved in 0.9% NaCl. Two separate series of experiments were performed, and skeletons were examined at 62 +/- 2 days postnatal. Sixty-six and 58 of the variants occurred in the two series, respectively. Frequencie

Early embryo-maternal communication in the oviduct: A review.

An intact embryo-maternal communication is critical for the establishment of a successful pregnancy. To date, a huge number of studies have been performed describing the complex process of embryo-maternal signaling within the uterus. However, recent studies indicate that the early embryo communicates with the oviductal cells shortly after fertilizationand that this is important for the successful establishment of pregnancy. Only if the early embryo is capable to signal the mother within a precise timeframe and to garner a response, will the embryo be able to survive and reach the uterus. This

Protecting Future Children from In-Utero Harm.

The actions of pregnant women can cause harm to their future children. However, even if the possible harm is serious and likely to occur, the law will generally not intervene. A pregnant woman is an autonomous person who is entitled to make her own decisions. A fetus in-utero has no legal right to protection. In striking contrast, the child, if born alive, may sue for injury in-utero; and the child is entitled to be protected by being removed from her parents if necessary for her protection. Indeed, there is a legal obligation for health professionals to report suspected harm, and for authorit

[Risk following intrauterine death of one of the fetuses in twin pregnancy].

Eleven twin pregnancies in which one of the two foetuses had died more than 24 hours before birth were evaluated for this report. Intra-uterine death of one foetus did not necessarily entail death of the other. The vitality of a newborn and the neonatal period were not affected by death of one of the twins. Improvement of foetal maturation and induction of labour in case of risk due to placental insufficiency were found to be proper approaches to reducing perinatal mortality and postnatal morbidity of the surviving foetus of twin pregnancy.

Jefferson v. Griffin Spalding County Hospital Authority: court-ordered surgery to protect the life of an unborn child.

In Jefferson v. Griffin Spalding County Hospital Authority, the Supreme Court of Georgia affirmed a lower court order requiring a pregnant woman to submit to a cesarian section and other medical procedures necessary to save her unborn child's life. The court found that the state's interest in protecting the viable fetus outweighed the pregnant mother's right to religious practice, right to refuse medical treatment, and parental autonomy. Jefferson appears to stand for the proposition that fetuses have rights that attach at viability and that mothers have a corresponding duty to ensure live bir

Considerations in the management of maxillofacial infections in the pregnant patient.

The pregnant patient who presents with a maxillofacial infection requires additional management considerations. The maternal-fetal unit requires particular attention to maintain fetal viability, assure a normal pregnancy, and obtain a desirable outcome. The purpose of this article is to inform the oral and maxillofacial surgeon of the clinically relevant material that one needs to understand and consider when treating maxillofacial infections in this particular patient population.

Maternal and neonatal outcomes following expectant management of previable preterm premature rupture of membranes: a retrospective cohort study.

OBJECTIVES: Previable Preterm Premature Rupture of Membranes (PPPROM), defined as rupture of membrane (ROM) before fetal viability. Currently, it is recommended to counsel the family regarding expectant management (EM) vs. termination of pregnancy (TOP) given the increased maternal morbidity associated with EM. Additionally, there is limited knowledge of outcomes following ROM ≤20 weeks. Given Mississippi's high-risk obstetric population and all mothers undergoing EM, we aimed to evaluate outcomes following PPPROM at our center. METHODS: A single-center retrospective cohort study conducted of

Genome-wide epigenetic alterations in cloned bovine fetuses.

To gain a better understanding of global methylation differences associated with development of nuclear transfer (NT)-generated cattle, we analyzed the genome-wide methylation status of spontaneously aborted cloned fetuses, cloned fetuses, and adult clones that were derived from transgenic and nontransgenic cumulus, genital ridge, and body cell lines. Cloned fetuses were recovered from ongoing normal pregnancies and were morphologically normal. Fetuses generated by artificial insemination (AI) were used as controls. In vitro fertilization (IVF) fetuses were compared with AI controls to assess

Influence of Coxiella burnetii infection of male mice on their offspring.

The dramatic spread of Q fever in Poland among cattle kept in isolation from natural environment (ticks, wild animals) has suggested the possibility that the infection may also be transmitted sexually. To test this hypothesis series of experiments have been performed in controlled laboratory conditions. Male mice infected with C. burnetii were allowed to mate with healthy female mice. On day 18 of pregnancy serum IgM antibodies to C. burnetii antigens and bacteria in spleen, liver and placenta were detected. The influence of C. burnetii transmission between parents of their offspring was inves

Form and development of the umbilical cord in pigs and their association with delivery of viable pigs.

Length, extensibility, and absolute strength of umbilical cords were determined in litters of pigs at term. Length was also determined in fetuses earlier in gestation and in hypophysectomized fetuses. Mean cord length at term was 34.6 cm (range, 17 to 50 cm) and its length was correlated with length and birth weight of pigs. Cords were significantly (P less than 0.01) shorter in hypophysectomized fetuses, but cord length was not influenced by sex, position in the uterus, or twisting of the cord. Although the umbilical cord was capable of being stretched considerably, the degree varied among pi
