Patients with evidence of fluid overload or heart failure (HF) without clinical symptoms of pneumonia are often treated with antimicrobial therapy for pneumonia. We conducted a retrospective study to evaluate the use of antimicrobial therapy in critically ill patients with fluid overload or heart failure diagnosed as pneumonia. A retrospective chart review of patients on antimicrobial therapy treated for pneumonia in the intensive care unit was conducted. The study's primary outcome was the number of cases with no evidence of pneumonia, including fluid overload or heart failure, managed with a

BACKGROUND: Intravenous fluid therapy is required for most surgical patients, but inappropriate regimens are commonly prescribed. The aim of this narrative review was to provide evidence-based guidance on appropriate perioperative fluid management.
METHOD: We did a systematic literature search of the literature to identify relevant studies and meta-analyses to develop recommendations.
RESULTS: Of 275 retrieved articles, we identified 25 articles to inform this review. "Normal" saline (0.9% sodium chloride) is not physiological and can result in sodium overload and hyperchloremic acidosis. St…

PURPOSE OF REVIEW: The ultimate purpose of fluid administration in states of hypovolemia is to correct cardiac output to improve microcirculatory perfusion and tissue oxygenation. Observation of the microcirculation using handheld microscopes gives insight into the nature of convective and diffusive defect in hypovolemia. The purpose of this article is to introduce a new platform for hemodynamic-targeted fluid therapy based on the correction of tissue and microcirculatory perfusion assumed to be at risk during hypovolemia.
RECENT FINDINGS: Targeting systemic hemodynamic targets and/or clinica…

Diarrhea remains the leading cause of mortality in beef and dairy calves. Calves that have diarrhea frequently develop dehydration, strong ion acidosis, and electrolyte abnormalities, and are in a state of negative energy balance. Oral electrolyte therapy is a simple and economical method of addressing all of these potential complications. This article gives an overview of oral electrolyte therapy of calves, including indications, guidelines for administration, and how to choose an electrolyte product.

Fluid therapy in patients with pulmonary disease is challenging. Although a single set of rules cannot be applied to every patient, the following guidelines can be used when managing patients with pulmonary disease. Euvolemic patients with adequate tissue perfusion should be given sufficient isotonic fluid to balance insensible losses. If severe pulmonary compromise is present, cessation of all fluid therapy may be considered if the patient is able to match its losses by voluntary intake. In hypovolemic or hypotensive patients, small boluses of isotonic crystalloids or colloids should be given…

Nocturia is a common and troublesome symptom in otherwise healthy elderly men and women. Nocturnal polyuria (an excessive nighttime urine output) has been documented to be a common finding in healthy men with lower urinary tract symptoms. It is also a presenting feature of various medical conditions, such as renal failure, hypercalcemia and diabetes. Fluid balance therapy is an option in those whose nocturia is secondary to nocturnal polyuria. If a reduction in fluid intake fails to reduce nocturnal frequency a variety of drug treatments may be beneficial. Several studies have confirmed the ef

Fluid therapy products for use in the critically ill patient are described. Various specific clinical syndromes are described in detail, including shock, hypoalbuminemia, heart failure, liver failure, diabetic keto-acidosis and pancreatitis. Pathophysiology and specific therapeutic recommendations are given for these clinical syndromes.

A computer model incorporating known behavior of the cardiovascular system and intravascular:interstitial fluid exchange was designed which allowed bleeding rate, IV infusion rate, and prehospital care times to be independently specified. All possible circumstances were examined. The model shows that IV's are potentially of benefit only when all of the following occur: 1) the bleeding rate is initially 25-100 ml/min, 2) the prehospital time exceeds 30 minutes, and 3) the IV infusion rate is approximately equal to the bleeding rate. IV infusions therefore appear of little benefit in the usual u

This article reviews fluid therapy principles for puppies and kittens, emphasizing their higher and distinct fluid and electrolyte requirements compared to adults. It discusses clinical signs, diagnostic tools, and routes of administration-preferably enteral, with intravenous and intraosseous for severe cases. Tailored maintenance rates and rehydration strategies are outlined, highlighting the importance of rapid, appropriate fluid replacement in dehydration, ongoing losses, and shock. The text underscores the necessity of close monitoring and individualized treatment plans, considering the un…

Mutations in the gap junction β2 (GJB2) gene, which encodes connexin 26, are the leading cause of genetic deafness. These mutations are characterized by the degeneration and fragmentation of gap junctions and gap junction plaques (GJPs) composed of connexin 26. Dominant-negative mutations of GJB2, such as R75W, cause syndromic hearing loss and palmoplantar keratoderma. We previously reported that the R75W mutation, a single-base substitution where C is replaced by T, causes fragmentation of GJPs. Therefore, an adenine base editor (ABE), which enables A-to-G base conversions, can potentially be

Sepsis is currently defined as life-threatening organ dysfunction caused by a dysregulated host response to infection. Sepsis may occur secondary to infection anywhere in the body, and its pathogenesis is complex and not yet fully understood. Variations in the host immune response result in diverse clinical manifestations, which complicates clinical recognition and fluid therapy both in humans and veterinary species. Septic shock is a subset of sepsis in which particularly profound circulatory, cellular, and metabolic abnormalities are associated with a greater risk of mortality than with seps…

This article is aimed to persuade most bovine practitioners that FET is a therapeutic tool that can easily be used under field conditions, and that by following the right protocol, one can provide a cost-effective treatment for the farmer, a rewarding experience for us, and a boost for our professional image. Based on the excellent current publications, the scheme of work to put FET into practice is presented. Controversial decisions must be made (such as oral vs. parenteral fluids, removing milk from diet vs. continued feeding, oral fluids with bicarbonate vs. those with metabolizable bases,

Intravenous fluid therapy is often the only effective treatment of severely dehydrated calves. In farm animal practice, application of IV fluid therapy is still comparatively rare. A protocol for treatment of dehydration and acidosis in the field is described, along with the rationale behind each fluid used. Even without the support of sophisticated laboratory analysis, an estimated 50% of calves that are expected to die of dehydration recover after IV fluid therapy is given.

Fluid management of the traumatized patient begins with assessment of volume status via palpation of pulses; evaluation of mental status; and measurement of urine output, arterial blood pressure, and central pressures. Intravascular line placement and choice of initial resuscitation fluids should be individualized to the clinical situation, although in most situations a crystalloid solution continues to be the initial fluid of choice. Following initial stabilization, the intravenous fluid administered can be tailored to a given situation, chosen only after the deranged fluid balance is sequent

BACKGROUND: Amniotic fluid embolism (AFE) is a catastrophic disease with significant mortality. Because the cardiopulmonary dysfunction associated with AFE is self-limited, the disease could be well suited to the use of extracorporeal therapies.
CASE: A woman progressed into cardiac arrest immediately after an elective cesarean delivery. Owing to severe hypoxemia and hypotension, AFE was suspected and peripheral venoarterial extracorporeal membrane oxygenation was quickly initiated. Subsequent evolution was complicated by intrabdominal bleeding, which required massive transfusion and multiple
