[Focal presentation of progressive multifocal leukoencephalopathy: first manifestation of human immunodeficiency virus infection].

INTRODUCTION: Progressive multifocal leukoencephalopathy (PML) is a subacute demyelinating disorder of central nervous system caused by an opportunistic papovavirus infection of oligodendroglial cells, the JC virus. This virus remains latent in the human host. Their antibodies are detectable in 70% of healthy adult subjects. It is reactivated by different diseases and medical therapies impairing cellular immunity. CLINICAL CASE: A 35 year-old man, smoker, allergic to penicillin and derivatives, with a history of endovenous drug abuse, was admitted to Neurology Department due to a progressive

[The clinical spectrum of focal infection due to nontyphoid Salmonella: 32 years' experience].

BACKGROUND: The aim of this study was to analyze the manifestations of the syndromes which constitute focal infection by Salmonella no typhi (SNT). METHODS: Ninety-one episodes of SNT infections studied over a period of 32 years in the Fundación Jiménez Díaz (Madrid, Spain) were retrospectively analyzed. RESULTS: Of the 1,892 patients with SNT infection studied during this period, 91 (5%) presented some focal form (57 males and 34 females) with a mean age of 49 years (SD +/- 21.6 years). Sixty percent of the episodes were acquired within the community. The localization of the focal infection…

[Natural focal infections in the north of the Far East. I. The detection of natural foci of infections in Chukot].

Results of a 3-year complex (zoo-parasitological, viral, bacteriological, and serological) studies of mammals and birds in Chukotka demonstrated the presence of natural foci of pseudo-tuberculosis, intestinal yersinosis, salmonellosis (heidelberg), and of tick-borne encephalitis. The existence of natural foci of tularemia and endemic rickettsioses is supposed on the basis of serological data.

Extraintestinal focal infections in adults with Salmonella enterica serotype Choleraesuis bacteremia.

BACKGROUND AND PURPOSE: Salmonella enterica serotype Choleraesuis, which is associated with severe human infections and multidrug resistance … apy for their underlying diseases were two high-risk groups. Twenty cases (47%) had extraintestinal focal infections, including 10 cases of mycotic aneurysm, six of osteomyelitis, and 4 pleuropulmonary infections. Univariate analysis revealed that age > or =50 years was associated with occurrence of endovascular infection (p=0.008), while immunosuppressive therap …

[Epidemiologic situation in the quarantine of natural focal infections in the Southern Federal District].

Materials on the situation in quarantine natural focal infections (anthrax, plague, tularemia, brucellosis, leptospirosis, etc.) in the Southern Federal District are presented. Experience in the provision of the epidemic safety in the above-mentioned diseases, based on the systematic surveillance on the activity of epizootological factors on the territory of the natural foci of infections, are described. The causes of the possible aggravation of the epidemiological situation in a number of nosological forms in connection with changes in natural and socio-economic factors are given. The results…

The uselessness of procalcitonin in the diagnosis of focal bacterial central nervous system infection.

We investigated the utility of procalcitonin in early diagnosis of bacterial central nervous system (CNS) infection. Serum procalcitonin level was markedly elevated in the patients with systemic meningitis but not in the patients with brain abscess and subdural empyema. Procalcitonin may be useless to diagnose focal bacterial CNS infection.

[Problems in the organization of epidemiological surveillance of natural focal infections and ways of their solution].

The spread of natural focal infections is determined by a set of interrelated and interdependent populations of warm-blooded animals, arthropods, and microorganisms. Real danger to humans makes their foci be included into environmental risk factors in this or that area.

Focal non-typhoidal Salmonella infections from a single center in Malaysia.

A retrospective review of patients with focal non-typhoidal Salmonella (NTS) infection was performed to determine its features and outcome. All patients with focal NTS infection admitted to the University of Malaya Medical Center, Malaysia, from 1993 to 2002 were studied. More than half (58%) of the 35 cases (54% male, median age 39 years, range 1.5 months to 79 years) were immunocompromized or had chronic medical conditions. One-third of the patients (34%) had superficial infections (lymphadenitis or subcutaneous tissue infection) and all recovered with antimicrobial therapy alone. Deep infec

Activation of focal adhesion kinase in adenovirus-infected human corneal fibroblasts.

PURPOSE: Focal adhesion kinase (FAK), a nonreceptor protein tyrosine kinase with protean downstream influences on cell cycle regulation, cytoskeletal dynamics, and cell attachment, is activated by integrin binding and aggregation. Adenoviruses, including those associated with human keratitis, enter permissive cells by an integrin-mediated mechanism. Hence, a possible relationship between adenovirus infection and tyrosine phosphorylation of FAK in human corneal cells was explored. METHODS: Human corneal fibroblasts (HCFs) derived from donor corneas were infected for various periods with adenov

The role of the National Institute of Public Health in the field of infections with natural focality.

In the post-war period the National Institute of Public Health, later Institutes of Epidemiology and Microbiology, headed by K. Raska, ranked among famous laboratories in the world due to its priority findings and original results. Research results of the Institute stimulated further research not only in Czechoslovakia but also abroad, in laboratories of Europe and America. The authors emphasize the significance of certain results in the epidemiology and ecology of infections characterized by natural focality. In the first place they discuss the isolation of TBE in 1948 and 1949 by Gallia et a

Enterococcal sepsis in neonates: features by age at onset and occurrence of focal infection.

Fifty-six neonates with enterococcal septicemia in a single hospital from 1977 through 1986 were studied. The incidence was low and constant … had a mild illness with respiratory distress typical of other etiologic agents or diarrhea without focal infection. By contrast, late-onset enterococcal sepsis was heralded by severe apnea, bradycardia, circulatory collapse, and increased ventilatory requirements. Focal infections, including scalp abscess or catheter-related infection (23% each), meningitis or p …

Multifocal multilobar focal cortical dysplasia type IIa in Febrile Infection Related Epilepsy Syndrome (FIRES).

A seven-year-old girl with history of type I diabetes and no history of seizures presented for altered mental status with convulsions nearly one week after a febrile illness. Serum and laboratory studies were normal with EEG showing biparietal fast activity and seizures originating from right occipital lobe consistent with FIRES. A collaborative decision was ultimately made to withdraw care. Post-mortem whole brain histopathological examination revealed diffuse abnormalities in multiple areas including both parietal lobes and the right parieto-occipital junction consistent with focal cortical

Dentistry and internal medicine: from the focal infection theory to the periodontal medicine concept.

During past decades the relationship between dentistry and internal medicine and especially the concept of the so-called focal infection theory have long been a matter of debate. The pathogenesis of focal diseases has been classically attributed to dental pulp pathologies and periapical infections. Nonetheless, in recent years, their role is being dismissed while increasing interest is being devoted to the possible associations between periodontal infection and systemic diseases. In fact, periodontal pathogens and their products, as well as inflammatory mediators produced in periodontal tissue…

Chronic Tonsillitis as a Focal Infection: A Decade-Long Case Involving Severe Systemic Symptoms.

Unlike acute tonsillitis, which is readily recognized as infectious, chronic tonsillitis, tonsilloliths, and tonsillar detritus are often considered non-infectious and benign, despite their potential to act as focal infections causing systemic inflammatory symptoms that are frequently overlooked when tonsillar compression is not performed. We report the case of a 29-year-old female with a decade-long history of progressive musculoskeletal pain, episodic low-grade fever, headaches, and exercise-induced inflammatory arthralgia affecting the feet, ankles, wrists, and spine. The initial otolaryngo…

Remission of pustular psoriasis after cholecystectomy: role of focal infection in pustularization of psoriasis.

We report an 81-year-old male with long lasting psoriasis with exacerbation who showed remission of skin lesions after cholecystectomy. He consulted our clinic for impetiginous skin lesions and was treated with oral antibiotics without improvement. Several days later, he developed right hypochondralgia and was diagnosed with acute cholecystitis with gallstones. After cholecystectomy, the skin lesions significantly improved and disappeared within three weeks. Laboratory examination data indicated that acute inflammatory parameters normalized simultaneously. Although the role of focal infection …
