Tetanus prophylaxis for diabetic foot ulcers.

The infected or ulcerated diabetic foot is a suitable environment for Clostridium tetani. Tetanus intoxication as a result of foot ulcer has been described in the literature. Immunopathy, vasculopathy, and ulceration place the diabetic patient at risk for developing tetanus. Of diabetic patients who contract generalized tetanus in the United States, foot ulcer or gangrene are responsible for 25% of cases. Patients who have diabetic wounds should receive tetanus prophylaxis. The prophylaxis should follow the "tetanus-prone" wound recommendations as set by the Centers for Disease Control and Pre…

Preventing foot ulcers in patients with diabetes.

CONTEXT: Among persons diagnosed as having diabetes mellitus, the prevalence of foot ulcers is 4% to 10%, the annual population-based incidence is 1.0% to 4.1%, and the lifetime incidence may be as high as 25%. These ulcers frequently become infected, cause great morbidity, engender considerable financial costs, and are the usual first step to lower extremity amputation. OBJECTIVE: To systematically review the evidence on the efficacy of methods advocated for preventing diabetic foot ulcers in the primary care setting. DATA SOURCES, STUDY SELECTION, AND DATA EXTRACTION: The EBSCO, MEDLINE, a…

Managing foot ulcers in patients with diabetes.

Over 1 million people in the UK have diabetes mellitus, of whom around 7-10% will, at some point, develop a foot ulcer. These ulcers, which in severe cases extend to muscle and bone, are prone to infection and can lead to gangrene and may necessitate amputation. Around 85% of amputations in people with diabetes are preceded by persistent foot ulcers. Here, we review the management of such ulcers.

Foot ulceration and infections in elderly diabetics.

Foot lesions occur commonly among patients with diabetes, particularly the elderly and those with sensory neuropathy. Because of serious or recurrent infections and impaired healing processes, initially trivial lesions may progress to chronic nonhealing wounds, gangrene, or untreatable infections that can lead to limb amputation. Strategies to prevent amputation depend on understanding the multifactorial nature of diabetic foot disease; providing effective ongoing preventive care, including patient education; and prompt and aggressive treatment of foot lesions when they occur. The approach to

Procalcitonin and Diabetic Foot Ulcer Infections: A Meta-Analysis.

In total, 532 patients were assigned to the IDFU group and 396 to the non-infected diabetic foot ulcers (NIDFU) group. The relationship between PCT and DFU was evaluated in ten studies, with significant heterogeneity among the included studies (x2 = 54.10, p = 0.00001; I2 = 83.6%). Therefore, a random effects model was used with a pooled standardised …

Mechanisms of lncRNA in Diabetic Foot Ulcer Wound Healing.

Diabetes mellitus (DM) is a chronic metabolic disease, and some diabetic patients develop diabetic foot ulcers (DFU), which are one of the most prevalent and serious complications among diabetic patients. These ulcers can lead to neuropathy, vasculopathy and severe infections. LncRNAs, a class of long-stranded noncoding RNAs that regulate gene expression, have been shown to play a critical regulatory role in diabetic foot ulcer wound healing. These molecules have been demonstrated to modulate numerous facets of the inflammatory response, cell proliferation and angiogenesis, which collectively …

Novel Supramolecular Hydrogel for Infected Diabetic Foot Ulcer Treatment.

Multifunctional responsive hydrogels hold significant promise for diabetic foot ulcer (DFU) treatment, though their complex design and manufacturing present challenges. This study introduces a novel supramolecular guanosine-phenylboronic-chlorogenic acid (GBC) hydrogel developed using a dynamic covalent strategy. The hydrogel forms through guanosine quadruplex assembly in the presence of potassium ions and chlorogenic acid (CA) linkage via dynamic borate bonds. GBC hydrogels exhibit pH and glucose responsiveness, releasing more chlorogenic acid under acidic and high glucose conditions due to b…

Diabetic Foot Ulcer Imaging: An Overview and Future Directions.

Diabetic foot ulcers (DFUs) affect one in every three people with diabetes. Imaging plays a vital role in objectively complementing the gold-standard visual yet subjective clinical assessments of DFUs during the wound treatment process. Herein, an overview of the various imaging techniques used to image DFUs is summarized. Conventional imaging modalities (e.g., computed tomography, magnetic resonance imaging, positron emission tomography, single-photon emitted computed tomography, and ultrasound) are used to diagnose infections, impact on the bones, foot deformities, and blood flow in patients…

Novel mediators regulating angiogenesis in diabetic foot ulcer healing.

A non-healing diabetic foot ulcer (DFU) is a debilitating clinical problem amounting to socioeconomic and psychosocial burdens. DFUs increase morbidity due to prolonged treatment and mortality in the case of non-treatable ulcers resulting in gangrene and septicemia. The overall amputation rate of the lower extremity with DFU ranges from 3.34% to 42.83%. Wound debridement, antibiotics, applying growth factors, negative pressure wound therapy, hyperbaric oxygen therapy, topical oxygen, and skin grafts are common therapies for DFU. However, recurrence and nonhealing ulcers are still major issues.…

Tissue Engineering-Based Strategies for Diabetic Foot Ulcer Management.

Significance: Diabetic foot ulcers (DFU) are a mounting problem with the increasingly frail population. Injuries that would otherwise heal are kept open by risk factors such as diabetes, obesity, and age-related conditions, which interferes with the natural wound healing processes. Recent Advances: This review summarizes recent advancements in the field of tissue engineering for the treatment of DFUs. FDA-approved approaches, including signaling-based therapies, stem cell therapies, and skin substitutes are summarized and cutting-edge experimental technologies that have the potential to manage…

A patient with foot ulcer and severe metabolic alkalosis.

We report a case of triple acid-base disorder with metabolic alkalosis as the primary disorder in a 65-year-old man due to ingestion and application to leg ulcers of baking soda (calcium bicarbonate). The blood pH was 7.65 with hypochloremia, hypokalemia, and prerenal azotemia. He was treated with isotonic saline with K replacement, and the patient improved without any adverse clinical consequences. We discuss the causes, mechanisms, and management of Cl-responsive (depletion) metabolic alkalosis.

Diabetic foot ulcer: a systematic review of risk factors.

BACKGROUND: This study used a systematic analysis to identify risk factors that may be used to predict which patients with diabetes will develop a foot ulcer. METHODS: A systematic review of the literature was conducted, which included observational cohort studies that followed patients with diabetes to see whether they developed a diabetic foot ulcer (DFU). A random-effect meta-analysis was performed, and odds ratios were used for nominal data or mean differences for continuous data. This study was registered at PROSPERO (CRD42020182527). RESULTS: The analysis included eight studies. From 4…
