A case of transvertebral anterior cervical foraminotomy for radiculopathy caused by cage subsidence.

BACKGROUND: Cage subsidence is a complication of anterior cervical discectomy and fusion (ACDF). Here, we present a case in which radiculopathy caused by cage subsidence was improved by transvertebral anterior cervical foraminotomy (TVACF). CASE DESCRIPTION: A 57-year-old male underwent an ACDF for cervical spondylotic myelopathy at the C5/ C6 level. Postoperatively, although the preoperative abnormal sensations improved, he developed increasing new paresthesia bilaterally in the C6 dermatome distribution. X-rays, 7 months postoperatively, revealed cage subsidence 8 mm into the superior and i…

Two-Level Minimally Invasive Lumbar Laminectomy and Foraminotomy: 2-Dimensional Operative Video.

Minimally invasive lumbar laminectomies utilize tubular retractors to minimize tissue disturbance, blood loss, and hospital length of stay compared to open operations. Here, we highlight the operative treatment of a 68-yr-old male entailing a 2-level minimally invasive lumbar laminectomy and foraminotomy at L2-L3 and L3-L4. The patient initially presented with symptoms of treatment-refractory lower extremity numbness and limited ambulation. His imaging demonstrated coronal scoliosis and severe lumbar central and foraminal stenosis at L2-L3 and L3-L4, with enlarged spinous processes, laminae, a…

Rates of anterior cervical discectomy and fusion after initial posterior cervical foraminotomy.

BACKGROUND CONTEXT: In select patients, posterior cervical foraminotomy (PCF) and anterior cervical discectomy and fusion (ACDF) result in similar clinical outcomes when used to treat cervical radiculopathy. Nonetheless, ACDF is performed more frequently, in part because of surgeon perception that PCF requires operative revisions more frequently. The present study investigates the rate of ACDF reoperation at the index level after initial PCF. PURPOSE: To determine the rate of ACDF after initial PCF and to further describe any patient characteristics or preoperative or operative data that incr…

Staged Full-endoscopic Foraminotomy for Multilevel Lumbar Stenosis with Scoliosis: A Case Report.

This report discusses the case of a 70-year-old male orthopedic surgeon with multilevel lumbar foraminal stenosis and degenerative scoliosis (Cobb angle 21°) who presented with right leg radiculopathy and foot drop. To accommodate his inability to take extended leave and his desire to avoid fusion, we performed a staged transforaminal full-endoscopic lumbar foraminotomy, consisting of one procedure per level, under local anesthesia. The procedures targeted the L5/S1, L4/5, and L3/4 levels sequentially at 1-month intervals. The patient's foot drop resolved immediately after the first surgery. A…

Enhancing Cervical Foraminotomy Outcomes With Patient-Specific 3D Anatomical Models: A Comparative Study.

INTRODUCTION: Cervical foraminotomy is a procedure used to treat patients with radiculopathy. While the procedure can be performed using a minimally invasive technique, achieving complete visualization of relevant anatomy can be challenging. This study explores the use of patient-specific three-dimensional (3D) printed anatomical models, created from advanced medical imaging data, for preoperative planning and intraoperative guidance in cervical foraminotomy by comparing fluoroscopy time, operative time, estimated blood loss volume, and functional improvement. METHODS: We conducted a retrospe…

Long-term outcomes following anterior foraminotomy for one- or two-level cervical radiculopathy.

PURPOSE: Anterior foraminotomy (AF) is a surgical treatment for unilateral cervical radiculopathy that avoids fusion-related complications, but its long-term outcome has yet to be investigated. To clarify the efficacy of AF, the author retrospectively collected long-term data regarding the results of this technique. METHODS: Of 50 patients who underwent AF between November 1999 and June 2005, those who were followed for more than 6 years (n = 44) were enrolled in this study. The parameters studied included the number of revisions, additional surgeries, VAS/NDI, and Odom's criteria. Plain radi…

The Top 100 Most Cited Articles on Posterior Cervical Foraminotomies: A Bibliometric Analysis.

BACKGROUND: Posterior cervical foraminotomy (PCF) is a procedure used for treating refractory cervical radiculopathy. Although anterior cervical discectomy and fusion (ACDF) remains the historical standard, growing evidence supports the noninferiority of PCF while preserving physiological cervical stability. While ACDF has extensive publications and associated bibliometric analyses, similar analyses are nonexistent for PCF. This study aims to identify the most influential studies related to PCF and investigate emerging trends and themes in the field. METHODS: The 100 most-cited articles relat…

Comparative Effectiveness Between Primary and Revision Foraminotomy for the Treatment of Lumbar Foraminal Stenosis.

BackgroundForaminotomy has demonstrated clinical benefit in patients with lumbar foraminal stenosis (LFS), as evidenced by several small retrospective investigations. However, there is a subset of patients who have recurrent symptoms following the operation and therefore require revision surgery. Yet, despite this phenomenon, the relative efficacy of revision foraminotomy (RF) is not well elucidated due to limited literature on the quality of life (QOL) outcomes and cost associated with primary foraminotomy (PF) and RF.PurposeTo compare the effectiveness of PF and RF in terms of QOL outcomes a…
