Explainable time-to-progression predictions in multiple sclerosis.

BACKGROUND: Prognostic machine learning research in multiple sclerosis has been mainly focusing on black-box models predicting whether a patients' disability will progress in a fixed number of years. However, as this is a binary yes/no question, it cannot take individual disease severity into account. Therefore, in this work we propose to model the time to disease progression instead. Additionally, we use explainable machine learning techniques to make the model outputs more interpretable. METHODS: A preprocessed subset of 29,201 patients of the international data registry MSBase was used. Di

[Tick-borne encephalitis in the Netherlands].

Tick-borne encephalitis (TBE) is endemic in South-Scandinavia, Central Europe and Eastern Europe. In 2016 the first patient was reported with TBE virus infection contracted in the Netherlands, in a forested area between Driebergen and Maarn (near Utrecht). At the time, field research did not identify any TBE-positive ticks at the supposed infection site. In the last two years, two patients have been diagnosed with TBE in the Diakonessenhuis Hospital in Utrecht; one patient was bitten by a tick in the Netherlands. This time round, tests on ticks from a different area near Utrecht (the forests a

Machine learning-based prediction of treatment response in comorbid hepatitis C patients receiving DAA therapy: a real-world study from Pakistan.

INTRODUCTION: Hepatitis C virus (HCV) infection remains highly prevalent in Pakistan, particularly among patients with multiple comorbid conditions. Despite the widespread availability of direct-acting antivirals (DAAs), practical machine learning approaches to predict sustained virological response (SVR) are still lacking in resource-limited settings. METHODS: This retrospective cohort study analyzed 221 comorbid HCV patients treated with Sofosbuvir + Daclatasvir ± Ribavirin combination therapy. Baseline demographic and laboratory parameters were preprocessed using standard scaling methods.

An epigenetic regulator synergizes with alphavirus-mediated gene therapy via biomimetic delivery for enhanced cancer therapy.

Gene therapy is promising for treating genetic disorders, but faces challenges in treating cancer due to the intricate genetic and immunosuppressive landscape of this disease. Here, we describe a technology combining alphavirus-based gene therapy with an epigenetic regulator via pyroptosis and immune checkpoints to address these challenges. A filamentous actin-mimicking liposomal delivery system, with high fusion efficiency, was developed that encapsulates the Semliki Forest virus (pSFV) DNA vector to deliver p53 and PDL1 scFv DNA, bypassing traditional endocytic barriers to deliver genes with

Artificial Neural Network Prediction of Mortality in Cancer Patients Presenting for Radiation Therapy at a Multisite Institution.

IntroductionFor many decades, the management of cancer has utilized radiation therapy, which continues to evolve with technology to improve patient outcomes. However, despite the standardization of treatment plans and the establishment of best clinical practices based on prospective, randomized trials and adherence to National Comprehensive Cancer Network (NCCN) guidelines, the outcomes from radiation therapy are highly variable and dependent on a number of factors, including patient demographics, tumor characteristics/histology, and treatment parameters. In this study, we attempt to use avail

Development of an interpretable machine-learning model to predict insufficient functional improvement after aquatic therapy in stroke.

BACKGROUND: Early identification of stroke survivors who are unlikely to achieve clinically meaningful motor improvement after aquatic therapy may help optimize rehabilitation prescriptions and support precision rehabilitation. This study aimed to develop and validate machine-learning models based on pretreatment Fugl-Meyer Assessment (FMA) data to predict insufficient motor improvement after aquatic therapy, to examine whether actual aquatic-therapy exposure improves prediction, and to identify the most important predictors. METHODS: We conducted a single-center retrospective study of patien

Predicting Inhibitor Development in Hemophilia 'A' using Machine Learning: A Comprehensive Approach to Data Preprocessing, Balancing, and Biomarker Identification Using AI on the CHAMP Dataset.

BACKGROUND: Hemophilia 'A' (HA) is a genetic blood disorder characterized by a deficiency of Factor VIII (FVIII), with treatment often triggering the development of neutralizing antibodies (inhibitors) to FVIII. Predicting the development of these inhibitors is crucial for clinical applications but presents significant computational challenges due to data imbalance, skewed data, and inadequate data sanitization. OBJECTIVES: This study aimed to develop a machine-learning/AI approach to find biomarkers and predict the development of inhibitors to Factor VIII in patients with Hemophilia 'A,' add

Identification of Plasmodium relictum causing mortality in penguins (Spheniscus magellanicus) from São Paulo Zoo, Brazil.

This study reports avian malaria caused by Plasmodium relictum in Magellanic Penguins (Spheniscus magellanicus) from São Paulo Zoo. The disease was highly infective among the birds and was clinically characterized by its acute course and high mortality. The penguins of São Paulo Zoo were housed for at least 2 years without malaria; however, they had always been maintained in an enclosure protected from mosquito exposure during the night period. When they presented pododermatitis, they were freed at night for a short period. São Paulo Zoo is located in one of the last forest remnants of the cit

Second Trimester Maternal Serum microRNAs as Biomarkers for Congenital Diaphragmatic Hernia Severity.

INTRODUCTION: Fetoscopic endoluminal tracheal occlusion (FETO) is currently reserved for severe cases of congenital diaphragmatic hernia (CDH); however, prenatal identification of severity remains challenging given the limitations of prognostication with fetal ultrasound and magnetic resonance imaging. microRNAs (miRNAs), which are small noncoding RNAs circulating in serum that freely cross the placenta, may serve as potential biomarkers. Considering the limitations of fetal imaging in prognostication, we aimed to identify miRNA signatures in second trimester maternal serum (2TMS) that disting

Review of systemic therapy in HCC with an approach to response assessment in clinical practice.

Systemic therapy for hepatocellular carcinoma (HCC) has undergone rapid transformation over the past decade, significantly expanding treatment options and improving survival for patients with advanced disease. Guided by the Barcelona Clinic Liver Cancer staging system, systemic therapy is primarily indicated for advanced-stage HCC and select intermediate-stage cases with preserved liver function. Immune checkpoint inhibitor-based combination regimens have redefined first-line therapy, demonstrating substantial improvements in overall survival and response rates. As systemic and locoregional tr

The role of surface EMG in predicting responsiveness of muscles to FES therapy after cervical SCI.

INTRODUCTION: Cervical spinal cord injury (SCI) can severely impair upper extremity (UE) functions, limiting independence and quality of life. Prior clinical trials showed that functional electrical stimulation (FES) therapy can reduce UE impairment. However, the response to FES therapy is not consistent across all treated myotomes. Our objective was to predict the muscle response to FES therapy using electrophysiological biomarkers from baseline surface electromyography (sEMG) signals, in order to support treatment decisions at the point of care. METHODS: We recruited 17 participants with cer

Machine learning-based prediction of treatment outcomes and quantitative analysis of contributing factors in fertility induction therapy for adolescent male patients with congenital hypogonadotropic hypogonadism.

OBJECTIVE: Congenital Hypogonadotropic Hypogonadism (CHH) is a rare disease with an extremely low incidence, and the outcomes of fertility induction therapy in CHH patients exhibit significant interindividual heterogeneity. Given the context of scarce samples and heterogeneous phenotypes, conventional statistical methods struggle to integrate multidimensional data and quantify the contribution of influencing factors. In contrast, machine learning (ML) techniques offer unique advantages in integrating high-dimensional complex medical data and uncovering hidden relationships. To date, the applic
