A galling disease? Dieulafoy's lesion of the gallbladder.

INTRODUCTION: Dieulafoy's lesion is a rare vascular anomaly characterized by an abnormally large and tortuous submucosal arteriole leading to an area of mucosal defect with minimal inflammation. It is most often seen in the stomach but could occur anywhere along the gastrointestinal tract. Only five cases of gallbladder Dieulafoy's lesion have been published so far. PRESENTATION OF CASE: We report a case of Dieulafoy's lesion in the gallbladder in a 44 year-old patient who presented with calculous cholecystitis. DISCUSSION: The clinical, radiologic and histologic findings are discussed in li

Epidemiology of gallbladder disease: cholelithiasis and cancer.

Diseases of the gallbladder are common and costly. The best epidemiological screening method to accurately determine point prevalence of gallstone disease is ultrasonography. Many risk factors for cholesterol gallstone formation are not modifiable such as ethnic background, increasing age, female gender and family history or genetics. Conversely, the modifiable risks for cholesterol gallstones are obesity, rapid weight loss and a sedentary lifestyle. The rising epidemic of obesity and the metabolic syndrome predicts an escalation of cholesterol gallstone frequency. Risk factors for biliary slu

Managing gallbladder disease with prostaglandin inhibitors.

Experimental studies have shown that prostaglandins increase hepatic bile flow and gallbladder mucin production, cause gallbladder dysmotility, and inhibit intraluminal fluid absorption, thus promoting gallstone formation. By virtue of their positive actions on gallbladder fluid secretion, muscle contraction, and inflammatory properties, prostaglandins may also have a role in the pain of acute cholecystitis. Although only a few clinical studies of the role of prostaglandin inhibitors in gallbladder disease have been done, results show that use of drugs such as aspirin or other nonsteroidal ant

Gallbladder Disease in the Morbidly Obese Patient.

Obesity alone and rapid weight loss induced by bariatric surgical procedures are both recognized risk factors for the development of cholelithiasis. The advisability of performing routine concomitant cholecystectomy with bariatric surgery has been controversial. This prospective clinical study was performed to determine if preoperative ultrasonography, preoperative cholescintigraphy and intraoperative surgical findings could help determine which patients would benefit from concomitant cholecystectomy. Eighteen morbidly obese patients undergoing bariatric surgery were studied preoperatively wit

Cholecystokinin cholecystography in the diagnosis of gallbladder disease.

Twenty-six patients who had typical symptoms of biliary tract disease, e.g. postprandial right upper quadrant pain, nausea and vomiting, fatty food intolerance and flatulence and who had had two or more normal oral cholecystograms were subjected to cholecytokinin cholescystography. Ten patients showed a normal response to the intravenous administration of cholecystokinin, namely prompt and complete emptying of the gallbladder without producing any adverse reaction or symptoms. Sixteen patients demonstrated either no contraction or incomplete contraction of the gallbladder in response to cholec

Sex Disparities in the Presentation of Gallbladder Disease.

INTRODUCTION: Although gallbladder disease is more common in women, there is a trend toward more complicated cases in male patients. METHODS: All cholecystectomies captured by the National Surgical Quality Improvement Program database for the year 2016 were reviewed. This encompassed 38 736 records. Records were reviewed for age, sex, procedure performed, operative time, postoperative diagnosis, functional status, American Society of Anesthesiologists (ASA) class, preoperative lab values (total bilirubin, alkaline phosphatase, white blood cell count, and aspartate aminotransferase. Diagnosis and management of gallbladder calculus disease.

BACKGROUND: The number and rate of cholecystectomy are increasing worldwide, although indications for operative treatment remain empirical, and several issues in the understanding of the condition are not concisely outlined. Our intention is to summarize and interpret current opinion regarding the indications and timing of cholecystectomy in calculous gallbladder disease. METHODS: Publications concerned with gallstone disease and related topics were searched for in MEDLINE using PubMed and summarized according to clinical scenarios with an emphasis on recent research. RESULTS: Only one rando

Dietary intake and gallbladder disease: a review.

OBJECTIVE: Dietary intake has long been looked upon as a potentially modifiable risk factor for gallbladder disease (GBD), here defined as either having gallstones or having had surgery for gallstones. This paper reviews the epidemiological evidence for an association between dietary intake and GBD, focusing on six dietary factors that have received the most attention in studies in this area: energy intake, fatty acids, cholesterol, carbohydrates and fibre, calcium and alcohol. The objectives of this review are to evaluate the potential usefulness of altering the diet to prevent GBD and to con

Computed tomographic evaluation of gallbladder disease.

Evaluation of gallbladder diseases by computed tomography (CT) is limited compared with ultrasonographic evaluation. However, CT can bring about excellent information in advanced carcinoma of the gallbladder, severe and complicated cholecystitis, porcelain gallbladder, and indication for dissolution of gallbladder stones. Unique findings are also available in emphysematous cholecystitis, xanthogranulomatous cholecystitis, stones with gas (Mercedez-Benz sign), limy bile, and enhancement in acute cholecystitis. Combined use of CT and ultrasound apparently increase the accuracy of diagnosis in ca…

Coexistence of gallbladder disease and morbid obesity.

To further investigate the relationship between gallbladder disease and morbid obesity, 92 morbidly obese patients underwent routine cholecystectomy at the time of their bariatric procedures. The preoperative ultrasonographic findings were positive in only 20 patients. Of the 92 patients who underwent cholecystectomy, 87 (95 percent) had pathologic evidence of gallbladder disease. This included cholecystitis, cholesterolosis, cholelithiasis, or some combination of the three. The incidence of postoperative cholecystitis, the technical difficulty of reoperation, the unnecessary expense and expos

Hydrops of the gallbladder in an adult with Kawasaki disease.

An acute, febrile, self-limited disease process occurred in an otherwise healthy young adult that satisfies accepted criteria for the diagnosis of Kawasaki disease. Unusual features of this case include the patient's age and the documentation of notable, acute noncalculous distention of the gallbladder, a recognized complication of the condition. Evidence of cystic-duct obstruction during the acute phase of distention and subsequent establishment of patency during the convalescent stage is provided with the use of serial ultrasonographic and radioisotopic studies. Discussion of the possible me

Disease of the gallbladder in patients with normal cholecystograms.

In contrast with what has widely been assumed, disease of the gallbladder sometimes remains undertected by even repeated cholecystography or cholegraphy. Chronic cholecystitis, gallstones, and acalculous cholesterolosis can escape detection by roentgenography of the gallbladder. Of 4,000 patients subjected to cholecystectomy between 1962 and 1973, the cholecystogram had been normal in only two. The range of indications for the operation has since been widened. Patients with typical symptons of gallbladder disease or recurrent pancreatitis of unknown origin have been operated on despite a norma

Age, Predisposing Diseases, and Ultrasonographic Findings in Determining Clinical Outcome of Acute Acalculous Inflammatory Gallbladder Diseases in Children.

We evaluated clinical factors such as age, gender, predisposing diseases and ultrasonographic findings that determine clinical outcome of acute acalculous inflammatory gallbladder diseases in children. The patients were divided into the four age groups. From March 2004 through February 2014, clinical data from 131 children diagnosed as acute acalculous inflammatory gallbladder disease by ultrasonography were retrospectively reviewed. Systemic infectious diseases were the most common etiology of acute inflammatory gallbladder disease in children and were identified in 50 patients (38.2%). Kawas…

Endoscopic gallbladder drainage for symptomatic gallbladder disease: a cumulative systematic review meta-analysis.

BACKGROUND: Endoscopic ultrasound (EUS)-guided transmural or endoscopic retrograde cholangiography (ERC)-based transpapillary drainage may provide alternative treatment strategies for high-risk surgical candidates with symptomatic gallbladder (GB) disease. The primary aim of this study was to perform a systematic review and meta-analysis to investigate the efficacy and safety of endoscopic GB drainage for patients with symptomatic GB disease. METHODS: Searches of PubMed, EMBASE, Web of Science, and Cochrane Library databases were performed in accordance with PRISMA and MOOSE guidelines. Poole

Prevalence of Bactibilia in Patients Undergoing Cholecystectomy for Gallbladder Diseases: An Observational Study.

INTRODUCTION: Bactibilia, the presence of bacteria in bile, is a significant concern in patients undergoing cholecystectomy for gallbladder diseases. It may contribute to post-operative complications, influencing surgical outcomes. Despite its clinical relevance, data on the prevalence, microbial spectrum, and antibiotic resistance patterns of bactibilia in the Indian population remain limited. This study aimed to assess the prevalence of bactibilia, identify the predominant bacterial isolates, and evaluate their antimicrobial susceptibility patterns. METHODS: This cross-sectional study was c…
