A hydrophilic polyimidazolium antibiotic targeting the membranes of Gram-negative bacteria.

OBJECTIVES: The rise of MDR Gram-negative bacteria (GNB), especially those resistant to last-resort drugs such as carbapenems and colistin, is a global health risk and calls for increased efforts to discover new antimicrobial compounds. We previously reported that polyimidazolium (PIM) compounds exhibited significant antimicrobial activity and minimal mammalian cytotoxicity. However, their mechanism of action is relatively unknown. We examined the efficacy and mechanism of action of a hydrophilic PIM (PIM5) against colistin- and meropenem-resistant clinical isolates. METHODS: MIC and time-kil…

Sensitization of Gram-Negative Bacteria to Aminoglycosides with 2-Aminoimidazole Adjuvants.

In 2019, five million deaths associated with antimicrobial resistance were reported by The Centers for Disease Control and Prevention (CDC). Acinetobacter baumannii, a Gram-negative bacterial pathogen, is among the list of urgent threats. Previously, we reported 2-aminoimidazole (2-AI) adjuvants that potentiate macrolide activity against A. baumannii. In this study, we identify several of these adjuvants that sensitize A. baumannii to aminoglycoside antibiotics. Lead compounds 1 and 7 lower the tobramycin (TOB) minimum inhibitory concentration (MIC) against the TOB-resistant strain AB5075 from…

The role of Gram-negative bacteria in skin and soft tissue infections.

PURPOSE OF REVIEW: Skin and soft tissue infections (SSTIs) are a leading cause of morbidity, emergency department visits and hospitalization. In recent years, the spread of carbapenem-resistant gram-negative bacteria (GNB) is also increasing in SSTIs. However, the armamentarium of available drugs is recently expanding as well. In this review, we reported the most recent data and about management and treatment of SSTIs caused by GNB, mainly for the treatment of carbapenem-resistant Enterobacterales (CRE), Pseudomonas spp and Acinetobacter spp. RECENT FINDINGS: The increasing incidence of carba…

Impact of Gram-negative bacteria on the treatment of venous leg ulcers.

Gram-negative germs with and without multi-resistance are garnering more and more importance. The aim of this study was to investigate the frequency and rate of resistance against antibiotics and to clarify the impact of Gram-negative bacteria, especially with high rates of resistance, for the treatment of venous leg ulcers. This is a retrospective, monocentric, non-randomised open study. Included were all data within 1 year of bacterial swabs of venous leg ulcers. We performed summarization, pooling, and descriptive analysis for frequencies and crossover. We analysed 679 swabs of 285 patients…

The "other" gram-negative bacteria in mastitis: Klebsiella, serratia, and more.

Mastitis caused by gram-negative infections is of increasing importance on modern and well-managed dairy farms. Without a doubt, E coli tends to be the most important cause of these gram-negative infections when the data are tallied across farms.1 However, more precise investigation of individual farms often reveals a farm-specific infection pattern where a single gram-negative bacterial species predominates. Several farms with a predominance of “other” gram-negative IMIs may be observed. We have shown the presence of outbreaks on individual dairy farms with K pneumoniae, S marcescens, and Ent…

Gram negative bacteria are associated with the early stages of necrotizing enterocolitis.

INTRODUCTION: Necrotizing enterocolitis (NEC) affects 5-10% of infants born weighing less than 1500 g. Most models of NEC recapitulate late-stage disease with gut necrosis and elevated inflammatory mediators. Evaluation of NEC at earlier, less lethal stages of disease will allow investigation of initial disease triggers and may advance our understanding of temporal relationships between factors implicated in NEC pathogenesis. In this manuscript, we describe our investigation of early NEC and test the hypothesis that bacteria and inflammatory mediators differ between animals with early NEC and

[Fluoroquinolones and Gram-negative bacteria: antimicrobial activity and mechanisms of resistance].

Fluoroquinolones acts by interacting with type II topoisomerases (DNA gyrase and topoisomerases IV). Related to this mechanism of action, bacteria have developed resistance mechanisms consisting in some target mutations (GyrA/GyrB for DNA gyrase and ParC/ParE for topoisomerase IV) or in a reduced access to the target itself, by either decreased permeability or augmented expression of efflux pumps, such as AcrAB and MexAB. Along with these classical mechanisms of chromosomal resistance, the presence of fluoroquinolones resistant proteins (Qnr) has been recently evidenced, codified by transmissi

Resistance in nonfermenting gram-negative bacteria: multidrug resistance to the maximum.

Nonfermenting gram-negative bacteria pose a particular difficulty for the healthcare community because they represent the problem of multidrug resistance to the maximum. Important members of the group in the United States include Pseudomonas aeruginosa, Acinetobacter baumannii, Stenotrophomonas maltophilia, and Burkholderia cepacia. These organisms are niche pathogens that primarily cause opportunistic healthcare-associated infections in patients who are critically ill or immunocompromised. Multidrug resistance is common and increasing among gram-negative nonfermenters, and a number of strains…

Colonization of the cementum surface of teeth by oral Gram-negative bacteria.

By using in vitro assays, a group of related, filamentous gram-negative bacteria isolated from subgingival plaque deposits of patients with periodontal disease were found to colonize intact teeth. Tentatively identified as members of the genus Cytophaga, these isolates exhibited a preference for colonizing the cementum surface of the root. Examination of intact teeth after several weeks of colonization revealed that the root substructure had been extensively demineralized.

New strategies for the management of biofilms formed by Gram-negative bacteria.

INTRODUCTION: The global rise of multidrug-resistant (MDR) Gram-negative bacteria (GNB) has intensified the threat of chronic and hard-to-treat infections, many of which are associated with biofilm formation. These biofilms confer enhanced resistance to antimicrobials and immune responses, posing a major clinical challenge. AREAS COVERED: This review summarizes the biological mechanisms of biofilm formation in GNB and explores both traditional and novel strategies for their prevention and eradication. The literature search covered peer-reviewed articles from major databases, focusing on thera…

Resistance of Gram-Negative Bacteria to Cefepime-Enmetazobactam: A Systematic Review.

Cefepime-enmetazobactam is a novel β-lactam/β-lactamase inhibitor combination showing good activity against multidrug-resistant (MDR) Gram-negative bacteria producing a variety of β-lactamases. In this systematic review, we aimed to evaluate the available data on resistance to this drug. We performed a thorough search of four databases (Embase, PubMed, Scopus, and Web of Science), as well as backward citation searching, to identify studies containing data on resistance to cefepime-enmetazobactam. The data were extracted and analyzed according to the breakpoints established by the European Comm…

Lipopolysaccharide biogenesis and transport at the outer membrane of Gram-negative bacteria.

The outer membrane (OM) of Gram-negative bacteria is an asymmetric lipid bilayer containing a unique glycolipid, lipopolysaccharide (LPS) in its outer leaflet. LPS molecules confer to the OM peculiar permeability barrier properties enabling Gram-negative bacteria to exclude many toxic compounds, including clinically useful antibiotics, and to survive harsh environments. Transport of LPS poses several problems to the cells due to the amphipatic nature of this molecule. In this review we summarize the current knowledge on the LPS transport machinery, discuss the challenges associated with this p…

Dissemination of carbapenemases producing Gram negative bacteria in the Middle East.

The emergence and spread of carbapenemase-producing bacteria, that hydolyze most β-lactams, including carbapenems, are a major concern of public health system worldwide, particularly in the Middle East area. Since the plasmids harboring resistance genes could be spread across other bacterial populations, detection of carbapenemase-producing organisms has become more problematic. These organisms produce different types of enzymes including the most prevalent types including KPC, VIM, IMP, NDM, and OXA-48. Carbapenemase producers are mostly identified among Escherichia coli, Klebsiella pneumonia

Penetration of beta-lactamase inhibitors into the periplasm of gram-negative bacteria.

The effectiveness of a beta-lactamase inhibitor/beta-lactam combination against Gram-negative pathogens depends on many interplaying factors, one of which is the penetration of the inhibitor across the outer membrane. In this work we have measured the relative penetrations of clavulanic acid, sulbactam, tazobactam and BRL 42715 into two strains of Escherichia coli producing TEM-1 beta-lactamase, two strains of Klebsiella pneumoniae producing either TEM-1 or K-1, and two strains of Enterobacter cloacae each producing a Class C beta-lactamase. It was shown that clavulanic acid penetrated the out

Immune responses to anaerobic gram-negative bacteria in health and disease.

In the serum of healthy and sick persons antibodies to Bacteroidaceae and Veillonella have been detected. The antibody titres to these anaerobic bacteria are commonly low, in adults, however, higher than in children. High antibody titres could be found in patients suffering from Crohn's disease and in cancer patients. Immune responses to anaerobic bacteria occur in periodontal processes, too. These antibodies belong mainly to the IgM class of immunoglobulins. On the contrary, antibodies observed in patients with sepsis belong to the IgG class.

Effects of clindamycin on derepression of beta-lactamases in gram-negative bacteria.

Depression of beta-lactamases in certain non-fastidious Gram-negative bacilli has been responsible for (i) the rapid development of resistance to a variety of beta-lactam antibiotics and (ii) antagonism between beta-lactam antibiotics. Therefore, the effects of a variety of inhibitors of macro-molecular synthesis on derepression of beta-lactamase were investigated with four strains each of enterobacter and Pseudomonas aeruginosa. When tested at concentrations that were not inhibitory to growth, clindamycin was the most effective inhibitor of derepression of beta-lactamases in some of the strai

[Characteristics and function of enterotoxins of gram negative bacteria (author's transl)].

Several factors are important for the pathogenesis of bacterial intestinal disease: 1. Colonisation of the intestine; 2. exo(= entero)toxin production without penetration into the intestinal wall; 3. invasion of the intestinal wall with or without (?) simultaneous formation of enterotoxin. These factors are of different importance for the various infective agents. Whereas cholera is exclusively caused by an enterotoxin, this is of minor importance for the pathogenesis of Shigella dysentery. The enterotoxins of Vibrio cholerae, Escherichia coli, and Shigella dysenteriae 1 have been intensively
