Postterm pregnancy: practice patterns of contemporary obstetricians and gynecologists.

The purpose of this study was to determine the management of postterm pregnancy by contemporary practicing obstetricians. A questionnaire investigating practice patterns pertaining to postterm pregnancy was mailed to 1000 randomly selected American College of Obstetricians and Gynecologists (ACOG) Fellows and Junior Fellows in March 2004. The response rate was 52.2% (522/1000). Statistical analysis included the answers from the 420 practicing obstetricians. Males comprised 55.7% (234/420) of the responding obstetricians. The majority of responding obstetricians (95.4%) rated their residency tr…

ACOG practice bulletin. Premature rupture of membranes. Clinical management guidelines for obstetrician-gynecologists. Number 1, June 1998. American College of Obstetricians and Gynecologists.

The following recommendations are based on good and consistent scientific evidence (Level A): With term PROM, labor may be induced at the time of presentation or patients may be observed for up to 24-72 hours for the onset of spontaneous labor. Antibiotics prolong the latency period and improve perinatal outcome in patients with preterm PROM and should be administered according to one of several published protocols if expectant management is to be pursued prior to 35 weeks of gestation. Antenatal corticosteroids should be administered to gravidas with PROM before 32 weeks of gestation to reduc

ACOG practice bulletin. Medical management of tubal pregnancy. Number 3, December 1998. Clinical management guidelines for obstetrician-gynecologists. American College of Obstetricians and Gynecologists.

Ectopic pregnancy is a major health problem for women of reproductive age and, in the United States, is the leading cause of pregnancy-related death during the first trimester. Diagnosis and treatment of tubal pregnancy before tubal rupture occurs decreases the risk of death. Early detection may make it possible for some patients to receive medical therapy instead of surgery. Methotrexate, a folinic acid antagonist, has been used to treat patients with small unruptured tubal pregnancies. The purpose of this document is to present evidence, including risks and benefits, about methotrexate as an

Pelvic venous disorders in women: Diagnosis and management for gynecologists.

Pelvic venous disorders (PeVD) are an underrecognized yet significant cause of chronic pelvic pain (CPP) in reproductive-aged women, characterized by venous reflux, obstruction, or both. This clinical opinion provides a practical overview of PeVD for gynecologists, outlining its pathophysiology, common presentations, diagnostic approach, and available treatments. Through a case-based example, it highlights how transdisciplinary collaboration-particularly with interventional radiology-can facilitate timely diagnosis and effective management. By improving awareness and clinical confidence, this …

Burnout in Obstetricians-Gynecologists: Its Prevalence, Identification, Prevention, and Reversal.

Studies indicate that burnout rates among obstetricians-gynecologists range from 40% to more than 75%, which is in the middle to upper one-third of medical specialties. Symptoms range from feelings of underappreciation and unresolved fatigue, to cynicism, depression, physical symptoms, and illness. Burnout is associated with poor job satisfaction, questioning career choices, and dropping out of practice, impacting workforce concerns and patient access. Awareness of the symptoms and some simple stress and fatigue reduction techniques can decrease the risk of being trapped in the downward spiral…

A Clinical Reference Guide on Sexual Devices for Obstetrician-Gynecologists.

In the United States, more than half of all women have used a vibrator, nearly one third of women have used a dildo, and more than three quarters of women who have sex with women have used a vibrator. Sexual devices can be used by patients with decreased libido, anorgasmia, conditions inhibiting vaginal penetration, partner erectile dysfunction, and motor or sensory disabilities. Basic knowledge of sexual devices can help obstetrician-gynecologists counsel patients about device safety, sharing, cleaning, disinfection, and material selection. Common sexual devices include vibrators, penetrative…

The ostrich syndrome: obstetrician gynecologists and human immunodeficiency virus exposure.

Obstetrician-gynecologists and midwives as well as surgical and labor and delivery staff are at risk for contact with human immunodeficiency virus (HIV), but measures can be taken to reduce the risk of exposure and transmission. Physician denial of exposure is common. Prompt identification of exposure, including rapid testing of index patients of unknown HIV status, combined with prompt initiation of prophylactic medication, may reduce transmission by 81%.

Eating disorders: recognition, evaluation, and implications for obstetrician/gynecologists(1).

Eating disorders are a common finding in adolescent females. The lifetime prevalence of anorexia nervosa and bulimia nervosa is as high as 3.7% and 4.2%, respectively. The disease is much more common in females than males and is often associated with depression and other mood disorders. Complications from eating disorders can include amenorrhea, osteoporosis with pathologic fractures, electrolyte disturbances, dehydration, cardiac arrhythmias, and even death. Eating disorders appear not only to increase the risk of miscarriage, but also are associated with preterm delivery and lower infant bir

Provision of primary-preventive health care services by obstetrician-gynecologists.

OBJECTIVE: To determine the extent to which obstetrician-gynecologists serve as primary care providers for women 15 years and older. METHODS: Three national data bases were analyzed to determine if differences exist in the content of care provided to women during a general medical examination by three medical specialties. Reasons for medical visits by women to obstetrician-gynecologists were compared with other medical specialties, and women who rely on obstetrician-gynecologists for their care were characterized demographically. Survey data on the self-perceived role of obstetrician-gynecolo…

What Gynecologists Need to Know About Patients With Physical Disabilities.

Using spinal cord injury as a framework, this paper reviews some of the care considerations that gynecologists may consider when providing care for patients with physical disabilities. It describes disability in the context of its impact on patients seeking care, and the responsibilities of health care systems and providers. Providing equitable care requires respectful communication, avoidance of common biases, an accessible environment, and knowledge of secondary conditions and diagnostic overshadowing.
