[Free nerve transplants in the reconstruction of secondary defects of the peripheral nerves of the hand].

BACKGROUND: Peripheral nerve of the hand significantly participate ub the physiological functions of the hand. A defect in the area of the peripheral nerve therefore represents a problem, the solution of which is in the field of microsurgical reconstruction. The study deals with the possibility of microsurgical reconstruction by use of free nerve draft. SUBJECTIVE: Clinical observation of patients after microsurgical reconstruction of the hand peripheral nerve defect. MATERIAL AND METHODS: 34 patients after lesions of the median nerve, ulnar nerve and mutual impairment of both nerves. The ev

Mycobacterium marinum Hand Infection in a Kidney Transplant Recipient: A Diagnostic Challenge.

BACKGROUND: Mycobacterium marinum is a slow-growing nontuberculous mycobacterium associated with aquatic environments and may cause chronic skin and soft-tissue infections following minor skin trauma. Diagnosis can be delayed because clinical manifestations may mimic conventional bacterial infections. We report an unusual case in a kidney transplant recipient with a positive QuantiFERON-TB Gold test and subsequent M. marinum infection of the hand. CASE PRESENTATION: A 53-year-old man with a history of kidney transplantation and long-term immunosuppressive therapy developed progressive swellin

Partial Hand Transplant: Lessons Learned From Cadaveric Dissection Studies.

As the field of vascularized composite allotransplantation continues to expand, new upper extremity transplant candidates are being considered. We recently evaluated a bilateral amputee who had a mid-forearm amputation and a contralateral metacarpal hand amputation. In the latter limb, a "partial" hand transplant that preserved the majority of the patient's existing hand, including a partially severed thumb with intact thenar muscle function, was proposed. The feasibility of this partial hand transplant was studied in fresh-frozen cadaver limbs. This report details the proposed approach, the c

Hand-assisted laparoscopic splenectomy in ABO-incompatible kidney transplant recipients.

OBJECTIVE: We perform living-related ABO-incompatible kidney transplantations to alleviate the organ shortage in our country. Splenectomy has been performed routinely in these recipients, although its clinical significance remains controversial. In this study, we have reported our experience with a hand-assisted laparoscopic splenectomy (HALS) technique. METHODS: Between April 2000 and December 2006, 50 patients (23 males) underwent ABO-incompatible kidney transplantation with HALS. The mean age and weight of the recipients were 44 +/- 13 years and 56 +/- 12 kg, respectively. All patients und

Hand-foot syndrome and risk factors for occurrence in hematopoietic stem cell transplantation recipients.

PURPOSE: Hand-foot syndrome (HFS) is a typical skin disorder caused by the use of cytotoxic anticancer drugs and molecular targets. Similarly, various anticancer drugs have been used as a conditioning regimen for hematopoietic stem cell transplantation (HSCT), and skin disorders such as HFS have been reported. The aim of this study was to determine retrospectively the frequency of HFS in recipients who have received a first allogeneic HSCT and the risk factors for HFS occurrence. METHODS: We retrospectively investigated the medical records of recipients who received their first allogeneic HSC

18-month outcomes of heterologous bilateral hand transplantation in a child: a case report.

BACKGROUND: Although heterologous vascular composite allotransplantation has become a burgeoning treatment option for adult amputees, there have been no successful cases previously reported in children. Here, we describe the surgical, immunological, and neurorehabilitation details with functional outcomes 18 months after heterologous bilateral hand and forearm transplantation in an 8-year-old child with quadrimembral amputations and a previous kidney transplant. METHODS: 2 years of extensive preparation by medical and surgical teams preceded the hand-forearm transplantation of this child. The

Neural activation for actual and imagined movement following unilateral hand transplantation: a case study.

Transplantation of a donor hand has been successful as a surgical treatment following amputation, but little is known regarding the brain mechanisms contributing to the recovery of motor function. We report functional magnetic resonance imaging (fMRI) findings for neural activation related to actual and imagined movement, for a 54-year-old male patient, who had received a donor hand transplant 50 years following amputation. Two assessments, conducted 3 months and 6 months post-operatively, demonstrate engagement of motor-control related brain regions for the transplanted hand, during both actu

Cutaneous Histoplasmosis in a Renal Transplant Patient Presenting With Hand and Forearm Necrotizing Myofasciitis.

Histoplasmosis rarely manifests as a primary infection of the extremities, but when it does, the damage inflicted can be severe. We present a case report of cutaneous histoplasmosis of the upper extremity due to direct inoculation in a renal transplant patient, presenting as myofasciitis. The patient initially had a salvaged upper extremity that was functional, but a persistent infection recurred with aggressive myofasciitis requiring a subsequent amputation.

Reconstruction of four damaged or destroyed ipsilateral fingers with free toe-to-hand transplantations.

Three patients with traumatic partial or complete loss of all ipsilateral digits, excluding the thumb, were reconstructed with free microvascular toe transplantations for all involved digits. In two cases, two pairs of combined second and third toe transplantations were utilized; in the third case, four separate toes were employed. Clinical follow-up was 8 1/2 years, 2 years, and 1 2/3 years. Detailed functional analysis was performed. All patients except the first had initially undergone reconstruction of only two digits and had requested a second procedure to transfer two additional toes. Wh

Painless erythema of the hands associated with non-Hodgkin's lymphoma in a lung transplant recipient.

Acral erythema limited to the fingers or the hands has been described in systemic disease. We report the case of a 17-year-old patient who had undergone lung transplantation for cystic fibrosis. Six weeks after transplantation, rapidly growing large-cell immunoblastic non-Hodgkin's lymphoma of the lungs and mediastinum was diagnosed. Sharply demarcated, painless, glovelike erythema was found on both hands. Therapy was reduction of immunosuppression and 12 cycles of extracorporeal photophoresis. After 4 months, lymphoma was in clinical and radiologic remission. Bilateral erythema of the hands a
