Sleep in heart failure.

Sleep plays a large role in patients with heart failure. In normal subjects, sleep is usually in a supine position with reduced sympathetic drive, elevated vagal tone and as such a relatively lower cardiac output and minute ventilation, allowing for recuperation. Patients with heart failure may not experience the same degree of autonomic activity change and the supine position may place a large strain on the pulmonary system. More than half of all heart failure patients have one of two types of sleep apnea: either obstructive or central sleep apnea. Some patients have both types. Obstructive s…

Tachycardia-induced heart failure.

Heart failure associated with tachyarrhythmias can very often be reversed by dealing with the underlying tachyarrhythmia. Typically characterized by left ventricular dilation and subsequent systolic dysfunction, this disorder can be caused by both atrial and ventricular arrhythmias, most commonly chronic atrial fibrillation. Whereas for most cardiomyopathies there is little that can be done to reverse the progression of the disease, in tachycardia-induced heart failure the patient's often debilitating symptoms can be ameliorated. This is particularly important in the primary care setting becau…

[Heart failure. Neurohormonal treatment].

Two hypothesis on the cause of heart failure (HF) have been postulated, one of hemodynamic character and another one of neurohormonal nature. According to the second one, HF is consequence of the overactivity of endogenous endocrine systems such as the renin-angiotensin aldosterone or the autonomic system. Angiotensin-II blockage is actually the first option in the medical treatment of HF. Since it reduces cardiac and vascular remodelation, synthesis of collagen and improves the endothelial function. Preceding to this option one can find the use of beta adrenergic blochers. Recently it was dis…

Heart failure in women.

The incidence of congestive heart failure in the US population is increasing as more women and men survive with chronic hypertensive and atherosclerotic heart disease. Almost half the patients in the US with heart failure are women. Hypertension, diabetes mellitus, and cigarette smoking are all more potent risk factors for the development of heart failure in women than in men. Important differences in presentation and mortality explain gender differences in the clinical course of heart failure. Overall mortality is lower for women than for men with symptomatic heart failure. Some but not all o…

Prevention of heart failure.

In light of the increasing prevalence, morbidity, and mortality of heart failure, preventative strategies are urgently needed. Risk factors include coronary artery disease, renal insufficiency, diabetes, and smoking. Essential strategies for prevention of heart failure are modification of risk factors for its development, and detection and treatment of asymptomatic left ventricular dysfunction (ALVD). In patients with ALVD, angiotensin-converting enzyme (ACE) inhibitor and beta-blocker therapy can prevent progression to symptomatic heart failure. Additional recently identified preventative str…

Primary diastolic heart failure.

Diastolic heart failure is defined clinically when signs and symptoms of heart failure are present in the presence of preserved left ventricular systolic function (ejection fraction >45%). The incidence and prevalence of primary diastolic heart failure increases with age and it may be as high as 50% in the elderly. Age, female gender, hypertension, coronary artery disease, diabetes, and increased body mass index are risk factors for diastolic heart failure. Hemodynamic consequences such as increased pulmonary venous pressure, post-capillary pulmonary hypertension, and secondary right heart fai…

Sleep and heart failure.

Sleep problems and symptoms of sleep disturbance are very prevalent in patients with heart failure (HF). Numerous contributing factors include sleep-related breathing disorders, increasing age, medications, anxiety and depression, and comorbidities. Thus, the cardiovascular nurse has an important role in the recognition and management of sleep-related problems in persons with HF. This article provides an overview of sleep disturbances in patients with HF, suggests evidence-based strategies for managing the sleep problems, and identifies pertinent areas for future nursing inquiry.

Endothelin and heart failure.

The availability of potent and orally active nonpeptide endothelin (ET) receptor antagonists has generated a host of information on the pathophysiological role of ET-1 in a number of preclinical models including hypertension, renal failure, heart failure and pulmonary hypertension. Convincing data are available to show that ET-1 receptor antagonists are beneficial in humans as far as reversal of deranged systemic and regional hemodynamics associated with CHF and pulmonary hypertension. As in other disease areas, the issue of whether ET(A)-selective or ET(A/B) antagonists are more suited for CH…

Arrhythmias in Heart Failure.

Cardiac arrhythmias are very common in the setting of heart failure, with atrial and ventricular arrhythmias often present in the same patient. The risk and the benefit of antiarrhythmic therapies are still a matter of debate. Class I antiarrhythmic drugs should be avoided in patients with heart failure, cardiac ischemia, or previous myocardial infarction. Beta-blocker agents reduce morbidity and decrease mortality in patients suffering from moderate to severe heart failure. Amiodarone may be beneficial in patients with advanced heart failure and increased resting heart rates. This class III d…

Aldosterone and heart failure.

In untreated congestive heart failure, aldosterone plasma concentrations are elevated in proportion to the severity of the disease and are further increased by the use of diuretic treatment. Angiotensin II, plasma potassium concentration, and corticotropin are the major stimulators of aldosterone synthesis. During angiotensin converting enzyme (ACE) inhibition, the role of alternative major or minor regulatory mechanisms may become significant. This may explain why during continuous ACE inhibition, after an initial reduction, plasma aldosterone measurements may subsequently increase to prether…

[Hypocalcemia causing heart failure].

A 39-year-old female patient with refractory heart failure has been studied. On February, 1982 she was submitted to right lobar thyroidectomy for remotion of the left thyroid lobe. Following the surgery, she had signs of hypocalcemia and the diagnosis of secondary hypoparathyroidism and heart failure had been made. Seven months after she had acute pulmonary edema, cardiomegaly III (cardiothoracic index = 0.58) with predominant left atrial and left ventricular hypertrophy, which were confirmed by echocardiogram (ECO). The ECO also demonstrated low contractility of the left ventricle. The QT int…

[Heart failure in the elderly].

The prevalence of heart failure substantially increases with advancing age. Nevertheless, heart failure in the elderly is commonly under-diagnosed, because dyspnoea and fatigue are often attributed to the natural process of aging. Age-related alterations of the cardiovascular system and the presence of multiple comorbidities not only change the clinical features of heart failure, but also have an impact on heart failure treatment in this population. Cautious uptitration of the individual drugs and vigorous clinical and laboratory monitoring is mandatory to avoid undesired side effects. Althoug…

Heart failure and the lung.

Heart failure (HF) is a highly prevalent disease that leads to significant morbidity and mortality. There is increasing evidence that the symptoms of HF are exacerbated by its deleterious effects on lung function. HF appears to cause airway obstruction acutely and leads to impaired gas diffusing capacity and pulmonary hypertension in the longer term. It is postulated that this is the result of recurrent episodes of elevated pulmonary capillary pressure leading to pulmonary oedema and pulmonary capillary stress fracture, which produces lung fibrosis. It is likely that impaired lung function imp…

Heart Failure Medication Discontinuation Among Hospice Patients with Concomitant Heart Failure and Dementia.

BACKGROUND: Heart failure (HF) and Alzheimer's disease and related dementias (AD/ADRD) frequently co-occur, complicating medication management at the end of life. METHODS: This secondary analysis included 1338 patients with HF and AD/ADRD across 12 hospices. HF was classified as a primary (n = 210) or secondary (n = 1128). HF-related medication use and discontinuation at hospice admission were characterized and compared by diagnosis primacy. RESULTS: Prehospice use was highest for beta-blockers (44.0%), loop diuretics (32.4%), and renin-angiotensin-aldosterone system (RAAS) inhibitors (17.7%…

The Stanford acute heart failure symptom score for patients hospitalized with heart failure.

BACKGROUND: Currently, there are no simple tools to evaluate the acute heart failure (HF) symptom severity in children hospitalized with acute decompensated HF (ADHF). We sought to develop an inpatient HF score (HFS) that could be used as a clinical tool and for clinical trials. METHODS: Pediatric HF clinicians at Stanford reviewed the limitations of existing HFSs, which include lack of calibration to the inpatient setting, omission of gastrointestinal symptoms, need for multiple age-based tools, and scores that prioritize treatment intensity over patient symptoms. To address these, we develo…
