Heart rate variability in dilated cardiomyopathy.

Chronic heart failure is associated with excessive neurohormonal activation. Analysis of heart rate variability is considered a valid technique for assessment of the autonomic balance of the heart. Twenty symptomatic patients of dilated cardiomyopathy in NYHA class II-IV symptomatic status and as many normal controls were subjected to 24 hours Holter monitoring to assess the heart rate variability with both time domain and frequency domain analysis. Age of the patients ranged from 12 to 67 years (mean +/- SD 38.6 +/- 7 years), the male-female ratio was 4:1. The left ventricular ejection fracti…

Clinical relevance of heart rate variability.

Heart rate variability (HRV) has recently become a popular noninvasive research tool in cardiology. Clinical assessment of HRV is frequently based on standard long-term ambulatory electrocardiograms, whereas physiologic studies employ spectral analysis of short-term recordings under controlled conditions. From a general point of view, HRV can be used in clinical practice to estimate (1) the integrity of cardiac autonomic innervation, (2) the physiologic status of cardiac autonomic activity, and (3) the vulnerability to various cardiac arrhythmias resulting from autonomic imbalance. Clinical re…

Dexamethasone and fetal heart rate variation.

OBJECTIVE: To determine the effect of maternal administration of dexamethasone on fetal heart rate and its variation. DESIGN: Retrospective analysis of computerised data derived from cases studied over three years. SETTING: High risk pregnancy unit, John Radcliffe Hospital, Oxford. SUBJECTS: Twenty-eight pregnant women, at 27 to 32 weeks of gestation, to whom dexamethasone was given to accelerate pulmonary maturation in the expectation of preterm delivery. METHODS: Dexamethasone (two doses of 12 mg intramuscularly, 12 h apart) was given on 51 occasions at weekly intervals (one to four occa…

Heart rate variability. Frequency domain analysis.

Experience with frequency domain analysis over the past two decades strongly suggests that it represents a unique, noninvasive tool for achi … ecause this parameter also is strongly influenced by the degree of coupling between respiration and heart rate, which, in turn, reflects the intensity of the respiratory effort as well as of parasympathetic activity. Respiratory pattern also can significantly influence HF power. The use of controlled breathing minimizes these problems, improves reproducibili …

Heart rate changes in diabetes mellitus.

Resting heart rates were measured lying, sitting, and standing in 61 diabetics with varying degrees of cardiovascular reflex abnormalities. Those with parasympathetic abnormalities alone had the highest heart rates, while those with both parasympathetic and sympathetic involvement had slightly less rapid heart rates, which were still faster than those in diabetics with normal cardiovascular reflexes. 38 other diabetics in whom autonomic function tests had been done at least three times had a similar pattern of resting heart rate. 25 had unchanged tests: those with parasympathetic involvement a…

Disorders of heart rate and rhythm.

The clinical approach to a child with a disorder of the heart beat is still that of a careful history, physical examination, and an ECG with a long rhythm strip. A diagnostic approach is presented which is based on rate, rhythm, and mechanism. Newer diagnostic methods are presented as well as advances in the basic cellular electrophysiology. Disorders of automaticity, triggerable cells, and re-entry are analyzed. Finally, therapy is revised for conventional drugs, and new and nonapproved drugs are listed. Indications and limitations for cardioversion, electrical pacing, and surgery are present

Heart rate variability following neonatal heart surgery for complex congenital heart disease.

BACKGROUND: Altered cardiac autonomic control may play a role in the morbidity and mortality suffered by neonates who undergo surgery for complex congenital heart disease (CHD). The purpose of this study was to evaluate cardiac autonomic activity, as measured by spectral indices of heart rate variability (HRV), prior to and early after infant surgery for CHD and attempt to correlate HRV indices with clinical outcome. In addition, we assessed the hypothesis that single-ventricle physiology and surgical interruption of the great arteries negatively affects HRV. METHODS: Sixty neonates prospecti…

Rate control in atrial fibrillation: looking beyond the average heart rate.

PURPOSE OF REVIEW: The aim of this article is to provide a perspective on rate control in atrial fibrillation which emphasizes patient wellbeing (exercise tolerance, symptoms, quality of life) over attempts to reduce resting or exercise heart rate to an arbitrary range. RECENT FINDINGS: Recent trials of rhythm versus rate control strategies of treatment in patients with atrial fibrillation suggest that rate control is a viable first line strategy in many patients. The adverse consequences of atrial fibrillation with rapid ventricular response are partly due to factors other than rate itself, …

Classical heart rate variability and nonlinear heart rate analysis in mice under Napentobarbital and ketamine/xylazine anesthesia.

BACKGROUND: Anesthetics are often used in animal experiments to achieve immobilization and relieve pain. However, many anesthetics can alter the dynamics of cardiovascular systems. We aimed to compare the effects of two frequently used anesthetics agents on heart rate variability (HRV) parameters in mice. METHODS: This observational study was performed between May and June 2014 in 21 male BALB/c mice aged 16-20 weeks. The animals were divided into three groups: pentobarbital (P), (n = 7); pentobarbital+fentanyl (P+F), (n = 7); and ketamine+xylazine (K+X), (n = 7). Surface electrocardiography …

Improving heart rate estimation in preterm infants with bivariate point process analysis of heart rate and respiration.

Accurate estimation of heart rate dynamics in preterm infants is important for predicting recurrent episodes of severe bradycardia. We hypothesize that estimation of heart rate can be improved by including respiration as a state variable, based on mechanisms that underlie cardio-respiratory coherence. For ten preterm infants, we demonstrate that including respiration as a covariate improves estimation accuracy by an average of 11% across bradycardia severity, and reduces the maximum error by 8%. We also find that cardio-respiratory coherence increases in low frequency content just prior to sev…

Heart rate and heart rate variability in pregnant dairy cows and their fetuses determined by fetomaternal electrocardiography.

In this study, fetomaternal electrocardiograms were recorded once weekly in cattle during the last 14 weeks of gestation. From the recorded beat-to-beat (RR) intervals, heart rate and heart rate variability (HRV) variables standard deviation of the RR interval (SDRR) and root mean square of successive RR differences (RMSSD) were calculated. To differentiate between effects of lactation and gestation, pregnant lactating (PL) cows (n = 7) and pregnant nonlactating (PNL) heifers (n = 8) were included. We hypothesized that lactation is associated with stress detectable by HRV analysis. We also fol…

Effect of intravenous magnesium on heart rate and heart rate variability in patients with chronic atrial fibrillation.

The present double-blind, placebo-controlled study investigated the effects of intravenous magnesium on heart rate and rate variability in 30 patients with chronic atrial fibrillation. During standardized conditions, intraindividual variation in heart rate and rate variability was low in patients with chronic atrial fibrillation and magnesium had no effect on heart rate or rate variability.

Heart Rate Variability, Hypnosis, and Psychotherapy.

Hypnotherapy is increasingly recognized as an effective treatment option for a wide range of psychological conditions and the psychological elements of physiologic health concerns. Despite its growing use, the mechanisms underlying hypnotherapy's therapeutic benefits remain unknown. Heart rate variability (HRV), a non-invasive biomarker of autonomic nervous system (ANS) activity and other processes relevant to the psychophysiological stress response, has been used to study the physiological effects of hypnosis. HRV is associated with adaptability to stress and overall mental and physical healt…
