Alcoholic Hepatitis.

Alcoholic hepatitis (AH) is a common disease associated with significant morbidity and mortality. Most often the diagnosis is suggested by a history of heavy alcohol excess in a patient with features of hepatic decompensation. In its purest form, AH is a histologic diagnosis of acute hepatic inflammation in response to alcohol. The primary objective of treatment for AH is to support long-term alcohol abstinence and to achieve adequate nutrition with lifestyle modification; goal setting and education are integral to long-term medical management. Severity at presentation (calculated by way of th…

[Autoimmune hepatitis].

Autoimmune hepatitis (AIH) is chronic, predominantly periportal hepatitis with hypergammaglobulinemia and tissue autoantibodies. It is a relative rare disorder, with a preponderance of female patients, that can present at any age. Its diagnosis relies on the exclusion of viral, metabolic, genetic and toxic etiologies of chronic hepatitis or hepatic injury. There are no pathognomonic features, but the presence of antinuclear and smooth muscle antibodies or liver-kidney microsomal antibodies together with typical histological features in liver biopsy allows the diagnosis with good confidence. Co…

[Autoimmune hepatitis].

Autoimmune hepatitis is an unresolving, hepatocellular inflammation of unknown cause that is characterized by the presence of periportal hepatitis on histologic examination, tissue autoantibodies in serum, and hypergammaglobulinemia. By international consensus, the designation autoimmune hepatitis has replaced alternative terms for the condition. Three types of autoimmune hepatitis have been proposed based on immunoserologic findings. Type 1 autoimmune hepatitis is characterized by the presence of antinuclear antibodies (ANA) or smooth muscle antibodies (SMA) (or both) in serum. Seventy percen…

Typhoid hepatitis.

Though typhoid fever is quite common, typhoid hepatitis is a very rare entity which may have a variety of presenting features similar to other more common conditions. One case of typhoid hepatitis is reported here because of its ratity.

Autoimmune hepatitis.

Autoimmune liver disease encompasses several disorders (Table 1). Autoimmune hepatitis (AIH) affects mainly women and is characterized histologically by a portal tract mononuclear cell infiltrate disrupting the limiting plate and invading the parenchyma ("interface hepatitis") and serologically by the presence of autoantibodies and increased levels of immunoglobulin G (IgG), in the absence of a known cause. AIH responds to immunosuppressive treatment. It can present insidiously or as an acute hepatitis. The previously accepted requirement of 6-month duration of symptoms before diagnosis can be…

Alcoholic Hepatitis.

Alcoholic hepatitis is a multisystem disease seen in individuals who chronically abuse alcohol. When severe, it is associated with a very high mortality rate, with nearly 50% of severely affected persons dying within 1 month of hospitalization. Primary therapy is complete alcohol abstinence and supportive care. Corticosteroids have been shown to be beneficial in a subset of severely ill patients with alcoholic hepatitis and concomitant hepatic encephalopathy. Pentoxifylline has been shown to improve short-term survival rates. Other pharmacologic interventions, including colchicine, propylthiou…

[Autoimmune hepatitis].

OBJECTIVE: To review an update of autoimmune hepatitis in children, based on classification, diagnostic and therapeutic criteria. METHODS: Articles on the theme were selected and updated, and the author's experience at the Liver Unit, Children's Hospital, School of Medicine, Universidade de São Paulo, was considered as well. RESULTS AND CONCLUSIONS: Autoimmune hepatitis has clinical features suggestive of acute viral hepatitis; however, other clinical features, laboratory tests, endoscopic and histological findings pointed to chronic liver disease, and the majority of the cases evolved to hepa…

Autoimmune hepatitis.

Autoimmune hepatitis (AIH) is a rare disease, characterized by female predominance, hypergammaglobulinemia, autoantibodies, association with HLA DR3 and HLA DR4 and a good response to immunosuppression. Different subtypes of AIH may be distinguished, based on differences in the autoantibody patterns. AIH type 1 is characterized by anti-nuclear (ANA) and/or anti-smooth muscular (SMA) autoantibodies. AIH type 2 is characterized by liver/kidney microsomal autoantibodies (LKM). AIH type 3 may be distinguished by autoantibodies to soluble liver proteins (SLA) or the liver pancreas antigen (LP). AIH…

Viral hepatitis.

Hepatitis A is still the most frequently reported vaccine preventable disease. A reduction in the incidence will only be achieved by routine childhood vaccination rather than by targeted vaccination of high-risk groups. A larger vaccine program is warranted. Hepatitis B remains a large public health problem. Vaccination targeted to high-risk adults failed to decrease the incidence of hepatitis B virus (HBV) infection. Sexual as well as nosocomial transmission remain serious problems. Vaccine escape variants have also been identified in newborns from infected mothers who had been vaccinated at …

Viral hepatitis.

Viral hepatitis is still one of the most common causes of acute and chronic liver disease worldwide. Major advances have been made in our knowledge of these diseases, many during the past year. Molecular biology and clinical studies have improved our understanding of the mechanisms of antiviral drugs, as well as viral resistance to therapy. The risks of hepatitis A in patients with chronic liver disease have been confirmed, and the efficacy of hepatitis A vaccines in these patients has been proven. Aggressive combination therapy has emerged as a promising strategy for chronic hepatitis B and C…

[Autoimmune hepatitis].

The morphology of autoimmune hepatitis is characterized by portal-periportal predilection of necroinflammatory lesions. In comparison to the viral type of hepatitis severe piece-meal-necroses, the collapse of periportal parenchyma, and to a higher degree acinar transformation of hepatocytes are more prominent. The autoimmune hepatitis may start with acute disease displaying unusual clinical und histopathologic features. The postinfantile giant cell hepatitis seems to constitute a variant of autoimmune hepatitis. Autoimmune hepatitis has been reproduced in animal models and it could be demonstr…

[Ischemic hepatitis].

In ischemic hepatitis, which is not rare, there is a marked but transient increase in serum aminotransferase activity to at least 20 times the upper limit of normal, without any other cause for hepatic necrosis. It is generally preceded by an acute cardiac complication, usually pulmonary edema or a tachyarrhythmia, causing decrease in cardiac output, or may follow acute respiratory failure. The typical histological lesion is centrilobular hepatic necrosis. We present 2 cases of ischemic hepatitis due to hypotension following rapid atrial fibrillation, sepsis and acute respiratory failure, high…
