Acute onset rhythmic hiccup-like respirations secondary to oral baclofen toxicity.

OBJECTIVE: Baclofen toxicity has been associated with seizures, coma, apnea, autonomic disturbances, and cardiac conduction abnormalities. It has not been associated with rhythmic hiccup-like respirations. METHOD: We report a patient with suspected baclofen toxicity. RESULTS: Our patient is a 19-year-old girl with cerebral palsy secondary to prematurity and repaired tetralogy of Fallot who had started oral baclofen 8 months before to diminish spasticity. Her main concern was the acute onset of rhythmic, deep, continual, hiccup-like breaths every few seconds, increasing in frequency with exha…

[Drug-induced hiccup: a review of the France pharmacologic vigilance database].

Hiccup is a sudden contraction of the inspiratory muscles, followed by an abrupt closure of the glottis, thus producing a characteristic sound. Some drugs have been reported to induce hiccup. Using the French Pharmacovigilance Database, the aim of the present study was to investigate reports of such an adverse drug reaction (ADR). Between 1985 and 1997, 53 cases were reported to the French pharmacovigilance network. The sex ratio was 16.6. Mean age was 52 years (range 18-83). Of the total number, 23 per cent were related to corticosteroids. 15 per cent to psychiatric (mainly non-imipraminic an…

Treatment of hiccup during general anaesthesia or sedation: a qualitative systematic review.

BACKGROUND AND OBJECTIVE: Acute hiccup is a minor complication that can occur during sedation or general anaesthesia. The disorder can disturb the surgical field, might interfere with lung ventilation or could hamper diagnostic procedures. The objective was to perform a systematic search for interventions aimed at treating hiccup occurring during anaesthesia or sedation. METHODS: A systematic search for reports describing interventions to treat hiccup in conjunction with anaesthesia was carried out (MEDLINE, EMBASE, Cochrane-Library, manual screening of reference lists and review articles, up…

Acupuncture for hiccups: Case reports and literature review.

RATIONALE: Persistent hiccups following a stroke are a common complication that can adversely affect the patient's condition and rehabilitation. Certain refractory cases fail to respond adequately to pharmacological treatment. We report 2 cases of successful treatment of persistent hiccups with acupuncture and a medical electromagnetic device (trade name, TDP, an abbreviation of the Chinese phrase "Te-ding Dian-ci-bo Pu"). PATIENT CONCERNS: The first patient was a 94-year-old male who had experienced continuous hiccups for 7 days. His comorbidities included Alzheimer disease, cardiac arrhythm…

Intractable hiccups as the harbinger of a diffuse pontine glioma.

Hiccups are experienced by people of all ages. While acute hiccups are benign and self-limited, persistent and intractable hiccups can sometimes signal a serious disease. We present a young previously healthy man who complained of only hiccups for 4 months and later developed a severe headache and projectile vomiting. His systemic examination was within normal limits. Brain imaging revealed a diffuse pontine glioma with mild hydrocephalus.

Systemic review: the pathogenesis and pharmacological treatment of hiccups.

BACKGROUND: Hiccups are familiar to everyone, but remain poorly understood. Acute hiccups can often be terminated by physical manoeuvres. In contrast, persistent and intractable hiccups that continue for days or months are rare, but can be distressing and difficult to treat. AIM: To review the management of hiccups, including a systematic review of reported efficacy and safety of pharmacological treatments. METHODS: Available articles were identified using three electronic databases in addition to hand searching of published articles. Inclusion criteria were any reports of pharmaceutical the…

Three intermittent doses of ropivacaine were administered during the operative period (total dose 2.29 mg/kg) followed by a continuous epidural (caudal) infusion (0.1% ropivacaine; initial dose 0.23 mg/kg/h plus fentanyl 0.46 μg/kg/h). The infant was extubated in the recovery area and transferred to the intensive care…

[Stopping persistent hiccups in the adult by endoscopic maneuver].

Thirty-four patients with persistent hiccups (median duration 10 days) were treated by gentle endoscopic massage of the region of the cardia. Hiccups stopped in all cases. Its early recurrence in one patient only required a second massage, which was also effective. This simple technique is recommended for the treatment of persistent hiccups.

A Medication Hiccup: Chlorpromazine-Induced Agranulocytosis in a 72-Year-Old Male.

INTRODUCTION: Agranulocytosis, a severe decrease or absence of neutrophils, is a side effect of several medications, including chlorpromazine. If not promptly recognized, it can lead to overwhelming infection, sepsis, and death. CASE PRESENTATION: A 72-year-old man with adenocarcinoma of the lung status-post recent lobectomy was admitted for postsurgical pain and electrolyte derangement. During his admission, he had intractable hiccups and was started on chlorpromazine 25 mg by mouth 3 times a day. Within a week, he developed pneumonia, type 1 respiratory failure, and a progressive neutropeni…

[A case of multiple sclerosis with intractable hiccup and acute respiratory arrest].

A 41-year-old woman with multiple sclerosis (MS) manifesting optic neuritis, cerebellar ataxia and myelopathy was admitted for intractable hiccup and aggravation of sensory disturbance in both lower limbs. Magnetic resonance imaging (MRI) revealed a Gd-DTPA enhanced lesion from the level of the medulla oblongata, involving the reticular formation, to that of the vertebral body C2. She became abruptly unable to breath following aggravation of bulbar palsy and tetraplegia and mechanical ventilation was immediately initiated. The next day, voluntary breathing re-appeared but automatic respiratory…
