Unresectable hepatocellular carcinoma (HCC) remains a global challenge, with limited effective treatment options for advanced-stage disease. The HIMALAYA trial (phase III randomized study that evaluated the STRIDE regimen) introduced the Single Tremelimumab Regular Interval Durvalumab (STRIDE) regimen, an immunotherapy-based approach that achieved a median overall survival (OS) of 16.43 months compared to 13.77 months with sorafenib. While statistically significant, this ~2.7 months OS gain warrants scrutiny in light of STRIDE's increased immune-related toxicity and cost. This commentary evalu

WHAT IS THIS SUMMARY ABOUT?: This is a summary of results from a phase 3 clinical study called HIMALAYA. HIMALAYA looked at treatment with one dose of a medication called tremelimumab combined with multiple doses of a medication called durvalumab (the STRIDE regimen) or multiple doses of durvalumab alone. These treatments were compared with a medication called sorafenib in participants with unresectable hepatocellular carcinoma (HCC). HCC is a type of liver cancer that is difficult to treat because it is often diagnosed when it is unresectable, meaning it can no longer be removed with surgery.

Cryoconite holes (cylindrical melt-holes on the glacier surface) are important hydrological and biological systems within glacial environments that support diverse microbial communities and biogeochemical processes. This study describes retrievable heterotrophic microbes in cryoconite hole water from three geographically distinct sites in Antarctica, and a Himalayan glacier, along with their potential to degrade organic compounds found in these environments. Microcosm experiments (22 days) show that 13-60% of the dissolved organic carbon in the water within cryoconite holes is bio-available to

Acute mountain sickness (AMS) is very common at altitudes above 2500 m. There are few treatment options in the field where electricity availability is limited, and medical assistance or oxygen is unavailable or difficult to access. Positive airway pressure has been used to treat AMS at 3800 m. We hypothesized that continuous positive airway pressure (CPAP) could be used under field conditions powered by small rechargeable batteries. Methods Part 1. 5 subjects trekked to 3500 m from 2800 m in one day and slept there for one night, ascending in the late afternoon to 3840 m, where they slept usin

BACKGROUND: In 1999, Basnyat et al. published preliminary data demonstrating an inverse correlation between hydration status and acute mountain sickness during an epidemiological study performed in the vicinity of Mount Everest. To expand on these findings, we have re turned to the Langtang area of the Nepal Himalaya to perform more specific studies of altitude illness related to dehydration and hypoxemia using urine studies, pulse oximetry, and physical examination.
HYPOTHESIS: Dehydration will incite physiological changes aimed at the preservation of vascular volume homeostasis characterize

Ground observations with diurnal variability over the Himalayas are crucial to validate climate mitigation efforts, build accurate emissions inventories, and better forecasting. This study bridges the acute scarcity of such measurements by making the first online observations of CO2 and CH4 with CO at a mountain site (Nainital, 29.4° N, 79.5° E, 1958 m a.m.s.l.) continuously for 5 years in the Central Himalayas. Observed levels of CO2 and CH4 are higher than those at other background sites. The boundary layer evolution and upslope winds determine the diurnal pattern of CH4 and CO, while CO2 di…

The Tibetan Plateau is an important area for persistent organic pollutants (POPs) research because of high-altitude condensation and global distillation. However, the sources of the POPs in remote regions have not been quantified, which is critical for global control of POPs. The Gyirong Valley in the Himalayas bordering Nepal serves as an important pathway for cross-border transport of POPs to the Tibetan Plateau. In this study, the occurrences of polychlorinated dibenzo-p-dioxin/furans (PCDD/Fs) and polychlorinated biphenyls (PCBs) as typical POPs were investigated in surface soil samples co…

Aspergillus fumigatus is a ubiquitous mold and a common human fungal pathogen. Recent molecular population genetic and epidemiological analyses have revealed evidence for long-distance gene flow and high genetic diversity within most local populations of A. fumigatus. However, little is known about the impact of regional landscape factors in shaping the population diversity patterns of this species. Here we sampled extensively and investigated the population structure of A. fumigatus from soils in the Three Parallel Rivers (TPR) region in Eastern Himalaya. This region is remote, undeveloped an

Scrub typhus has re-emerged as an important public health concern in Himachal Pradesh, India, and is caused by Orientia tsutsugamushi. Effective control efforts are limited by outdated data on ectoparasite vectors and infection rates among reservoir hosts in this endemic region of the Western Himalayas. We conducted a comprehensive field study across eight districts of Himachal Pradesh to examine ectoparasite diversity associated with rodents and shrews in an area endemic for scrub typhus. Small mammals such as rodents and shrews were trapped, and their ectoparasites were collected and morphol…

Reticulate evolution, driven by incomplete lineage sorting (ILS) and gene flow, is increasingly recognized as a fundamental process shaping lineage diversification, posing both challenges and opportunities for taxonomic treatment. During field investigations in Xizang, China, we discovered a morphologically distinct new species exhibiting certain similarities with the genera Microtoena yet clearly differing in corolla morphology. We sampled all 12 currently recognized genera within the tribe Pogostemoneae and conducted phylogenomic analyses integrating data from 539 low-copy orthologous nuclea

This study presents the first report on the volatile composition of Allium stracheyi Baker, evaluating the impact of seasonal variability and circadian rhythm on the yield and chemical composition of its essential oil. Seasonal analysis involved leaf collection during three distinct seasons (dry, transition, and rainy), while the circadian study examined oil composition at three-hour intervals from 6 AM to 6 PM. Essential oils were extracted via hydrodistillation and analyzed using GC-MS. The highest essential oil yield (1.02 %, v/w) was recorded in the dry season (May), while the lowest yield

Medicinal plants are global sources of herbal products, drugs, and cosmetics. They are disappearing rapidly due to anthropogenic pressure, overexploitation, unsustainable harvesting, lack of knowledge on cultivation, and the availability of quality plating materials. In this context, standardized in-vitro propagation protocol was followed to produce Valeriana jatamansi Jones, and transferred in two locations at Kosi-Katarmal (GBP) Almora (1200 masl) and Sri Narayan Ashram (SNA) Pithoragarh (Altitude 2750 masl), Uttarakhand. Information on reproductive biology and pollination ecology studies of threatened plants are essential to develop strategies for their sustainable utilization and effective conservation. As such, these studies were conducted on Rheum webbianum, a high-value "vulnerable" medicinal herb of the north-western Himalaya. This species presents a unique mode of reproductive behavior through the involvement of different floral events, including the movement of reproductive organs. The plants survive extremely cold conditions through underground perennating rhizomes that sprout into juvenile shoots with
