Total hip replacement following infection in the opposite hip.

Total hip replacements without ensuing infection were performed in 31 patients in the presence of infection in the contralateral hip. Infection in the opposite hip was proved bacteriologically in the follow-up in 19 cases averaged 5 years (2 years 3 months to 10 years). In another 12 hips in the same circumstances, without positive bacteriological evidence of active infection on the opposite side, healing occurred without infection.

Conversion of hip joint pseudarthrosis to total hip replacement.

Forth hips in 35 patients were converted from pseudarthrosis to low friction total hip arthroplasty (LFA). Pain, function and range of motion were improved in most cases. Gait was improved and number of walking aids required were reduced in most cases. Abduction against gravity and straight leg raising were also improved. Limb length was restored in all but 9 cases. Two cases dislocated postoperatively. One was reduced and did well; the other had a trochanter avulsion with recurrent subluxation. Improvement of function is a prime indication for converting a pseudarthrosis to LFA whereas pain i…

Iliopsoas Impingement and the Post-Total Hip Arthroplasty Hip.

Iliopsoas impingement is an increasingly recognized source of groin pain following total hip arthroplasty. It occurs from mechanical conflict between the iliopsoas tendon and prosthetic components, most commonly the acetabular rim or femoral collar. Diagnosis relies on a combination of history, physical examination, and imaging, with ultrasound offering unique diagnostic and therapeutic roles. While most patients improve with nonoperative care, persistent cases may require surgical intervention. Arthroscopic iliopsoas release has demonstrated excellent outcomes with minimal morbidity and low s…

The complications after open hip dislocation in hip surgery.

Our purpose is to evaluate the complications of open hip dislocation, which is used as a helpful technique in hip surgery. We have retrospectively reviewed 45 hips of 44 cases who applied open hip dislocation with various indications in our institute between the years 2006-2013. There were 27 males and 17 females whose mean age was 31,9 (range, 11-58) years with mean follow-up time of 56,9 months (range, 13-106). The number of cases with at least one complication related to open hip dislocation was 27. Within our series 14 hips have developed only 1 complication, 1 hip have 2, 10 hips have 3 a…

Total hip replacement for arthritis following tuberculosis of hip.

AIM: To present the results of total hip arthroplasty (THA) for post tubercular arthritis of the hip joint. METHODS: Sixty-five patients (45 male, 20 female) with previously treated tuberculosis of the hip joint underwent cementless THA for post tubercular arthritis. The average age at the time of THA was 48 years (range 29 to 65 years). Erythrocyte sedimentation rate, C reactive protein, chest X-ray and contrast enhanced magnetic resonance imaging were done preoperatively to confirm resolution of the disease and to rule out any residual disease. Intra-operative samples were taken for microbi…

Radiographic manifestation of hip dislocation after total hip arthroplasty.

OBJECTIVE: The authors hypothesized that a patient who has posterior hip dislocation after total hip replacement does not have the same clinical manifestations of malposition as with a natural hip. The present study aimed to study clinical manifestation of hip dislocation after total hip arthroplasty. MATERIAL AND METHOD: Thirty-five cases of posterior dislocation after total hip replacement were retrospectively studied by medical records and radiographic evaluation. The study included leg position after hip dislocation, leg length, and leg abduction/adduction angles. RESULTS: External rotat…

Periprosthetic fractures around hip hemiarthroplasty performed for hip fracture.

UNLABELLED: Hip fracture is associated with considerable morbidity and mortality and occurs in an elderly and infirm group of patients. Periprosthetic fracture after hip hemiarthroplasty is a serious complication. In this study, we have reviewed our experience of this injury. The outcome measures used were fracture union, mortality, infection and requirement for further surgery. METHOD: We identified a cohort of 79 patients who sustained periprosthetic fractures after hip hemiarthroplasty from a prospective hip fracture database of 8354 patients (3611 were treated with hemiarthroplasty). Seve…

Femoral considerations for total hip replacement in hip dysplasia.

THAin the dysplastic hip is a technically demanding procedure that requires careful preoperative planning. The common deformities associated with the dysplastic femur include hypoplasia, excessive neck anteversion, a valgus neck-shaft angle, metaphyseal-diaphyseal mismatch, and a posteriorly displaced greater trochanter. In selected cases, osteotomy of the femur may be performed to correct anteversion and/or avoid excessive leg lengthening and stretch of the sciatic nerve. All of these issues mandate careful preoperative planning. With the advent of modern surgical techniques and implants, the…

Dynamic hip screw blade fixation for intertrochanteric hip fractures.

PURPOSE: To assess the one-year outcome of a dynamic hip screw (DHS) blade in the treatment of AO/OTA 31-A1 and 31-A2 intertrochanteric hip fractures. METHODS: 35 men and 65 women aged 47 to 100 (mean, 83) years underwent fixation with a DHS blade for A1 (n=47) and A2 (n=53) intertrochanteric hip fractures after a low-energy injury. Patients were operated on within 48 hours of admission. Anteroposterior and lateral radiographs were examined for the tip-apex distance and femoral shortening. Potential complications were looked for, including implant migration, cut-out, loosening, or breakage. F…

Hip protectors for preventing hip fractures in older people.

BACKGROUND: Hip fracture in older people usually results from a fall on the hip. Hip protectors have been advocated as a means to reduce the risk of hip fracture. OBJECTIVES: To determine if external hip protectors reduce the incidence of hip fractures in older people following a fall. SEARCH STRATEGY: We searched the Cochrane Bone, Joint and Muscle Trauma Group Specialised Register (January 2010), The Cochrane Library 2010, Issue 2, MEDLINE (1950 to November 2009), MEDLINE in-process (30 December 2009), EMBASE (1988 to 2009 week 52), CINAHL (1982 to February 2009), BioMed Central (January 2…

Total hip arthroplasty for late hip dislocation in paraplegia.

Late hip dislocation is uncommon, particularly in the context of paraplegia. We report a case in which total hip arthroplasty with a semi-constrained acetabular component was a successful treatment for this condition. A review of the literature revealed that this method of treatment had not been previously described in paraplegics. For patients with late hip dislocation in spastic paraplegia, total hip arthroplasty with a semi-constrained acetabular component, combined with adequate adductor release and obturator neurectomy is recommended. (Hip International 2002; 12: 338-41).

[Conservative treatment of hip dysplasia/hip dislocation in infancy].

In the last two decades, considerable progress has been made in the conservative treatment of dislocation. As our ability increases to detect hips at risk due to instability or decentration in the course of screening newborns, it ought to be possible to make this treatment increasingly simpler and less problematic.

[Management of hip fractures using a dynamic hip screw].

Authors describe the history of the dynamic hip screw and analyse the hip fracture. The operative technique of the DHS is described in details. In an analysis of the material of the Wilhelminenspital it is stressed that in the last years the DHS became dominant in the treatment of hip fractures. The treatment of four different types of fractures with DHS is demonstrated. The method is compared with other methods suitable for the treatment of hip fractures and it is stated that the less complications can be expected from the use of the DHS. On the basis of this comparison it is shown that the D…

The Effect of Hip Arthroplasty on Gait Function: Comparison of Ceramic-On-Ceramic Hip Resurfacing, Metal-On-Metal Hip Resurfacing, and Total Hip Arthroplasty.

Ceramic-on-ceramic hip resurfacing arthroplasty (CoC-HRA) has been developed to eliminate metal ion concerns which have been associated with metal-on-metal hip resurfacing arthroplasty (MoM-HRA) while maintaining similar functionality. The aim of the study was to examine gait function pre- and postoperatively between CoC-HRA, MoM-HRA, and THA using subjective and objective measures with comparison to a healthy control group. Nineteen unilateral CoC-HRA, 19 unilateral MoM-HRA, and 18 unilateral THA gender, age, and BMI matched participants completed patient-reported outcome measures (PROMs) (Ox…

Analysis of posterior hip joint impingement about developmental dysplasia of the hip after total hip arthroplasty.

BACKGROUND: Total hip arthroplasty is the preferred treatment for advanced hip osteoarthritis, yet complications like hip dislocation (0.2 %-10 %) persist due to factors such as implant design, positioning, surgical technique, and patient-specific conditions. Impingement between prosthetic components or the acetabulum and proximal femur is a primary cause of instability. METHODS: A retrospective analysis of 120 patients (130 hips) undergoing total hip arthroplasty at XX Hospital (2011-2023) categorized patients by lateral center-edge angle: developmental dysplasia of the hip, borderline devel…

The relationship between MRI-detected hip abnormalities and hip pain in hip osteoarthritis: a systematic review.

Magnetic resonance imaging (MRI) is increasingly used in the classification and evaluation of osteoarthritis (OA). Many studies have focused on knee OA, investigating the association between MRI-detected knee structural abnormalities and knee pain. Hip OA differs from knee OA in many aspects, but little is known about the role of hip structural abnormalities in hip pain. This study aimed to systematically evaluate the association of hip abnormalities on MRI, such as cartilage defects, bone marrow lesions (BMLs), osteophytes, paralabral cysts, effusion-synovitis, and subchondral cysts, with hip…

Posterior hip pericapsular neurolysis (PHPN) for inoperable hip fracture: an adjunct to anterior hip pericapsular neurolysis.

BACKGROUND: The development of anterior hip neurolysis has made an appreciable impact on the management of patients with inoperable hip fracture. Nonetheless, suboptimal analgesic benefit was still observed in some patients. We therefore developed a novel posterior hip pericapsular neurolysis (PHPN) to complement anterior hip neurolysis in inoperable hip fracture. METHODS: In this retrospective review, we analyzed patients who responded suboptimally (defined as composite pain score moderate or higher on hip flexion 80°) to the anterior hip neurolysis from July 2019 to March 2021. The patients…

Does high hip centre affect dislocation after total hip arthroplasty for developmental dysplasia of the hip?

BACKGROUND: To achieve sufficient socket coverage by the native bone, high placement of cementless acetabular cup is often required. We previously reported, using computer simulation, that higher hip centre improved the bone coverage but decreased the range of motion in total hip arthroplasty (THA) for patients with hip dysplasia. However, in a clinical setting, the correlation between the hip centre height and dislocation after primary THA is still unclear. We examined whether a high hip centre affects dislocation after THA. METHODS: A total of 910 patients, with 1079 dysplastic hips, who un…
