Lipoid Proteinosis: A Rare Cause of Hoarseness.

Lipoid proteinosis is a rare cause of voice problems. Hoarseness is often the first clinical manifestation of this disorder and can present years before any other symptom. Therefore, it is very important as an otorhinolaryngologist to be familiar with the main characteristics of this disease. We present a case report and a review of current literature to provide a concise overview of this frequently missed diagnosis.

[A smoker with hoarseness and a swelling of his neck].

A 68-year-old man was referred to the Department of Otolaryngology because of a swelling of his neck and hoarseness. CT imaging of his neck revealed a cystic mass in the larynx as well as in the neck, with an air-fluid level. The diagnosis 'laryngopyocele' was made.

Dysphagia, hoarseness, and globus in a postoperative patient.

Sore throat, hoarseness, and dysphagia are known and recognized postoperative complications of laryngeal mask airway use during operative procedures. The patient's symptoms, present immediately after surgery, are thought related to airway manipulation. Airway foreign bodies, although low on the differential, can cause similar symptoms. We present a case of a single patient who presented to a tertiary care center after an elective outpatient procedure with postoperative sore throat, hoarseness, and dysphagia. A foreign body was found lodged in the patient's hypopharynx. The differential diagnos…

Hoarseness: a sign of self-induced vomiting?

Acid reflux, which is the entry of acidic gastric contents into the upper gastrointestinal tract, may manifest as two clinical syndromes-gastroesophageal reflux and/or laryngopharyngeal reflux. The latter syndrome, laryngopharyngeal reflux, is associated with the symptom of hoarseness. In the literature on eating disorders, according to general articles on medical complications, case reports, review articles on reflux, and empirical research, patients with self-induced vomiting (e.g., anorexia nervosa binge-eating purging type, bulimia nervosa purging type) are at risk for acid reflux, potenti…

Dysphagia and hoarse voice: an unusual cause.

We report an unusual presentation and successful treatment of an extracranial skull base carotid aneurysm. The patient presented with a combination of multiple cranial nerve dysfunction and local mass effect. After endovascular treatment of this rare condition her symptoms resolved. The radiological images submitted demonstrate this rare condition well.

Cardiovascular hoarseness: an unusual presentation to otolaryngologists.

OBJECTIVE: We discuss the case of a 73-year-old woman with a six-month history of hoarseness secondary to an aortic arch pseudoaneurysm. METHOD: We present the findings of an extensive review of the literature relating to cardiovascular disorders involving the recurrent laryngeal nerve (i.e. Ortner's syndrome). RESULTS: Ortner's syndrome, also known as cardiovocal syndrome, is a rare condition, with few reports in the literature. CONCLUSION: This is only the second documented case of Ortner's syndrome in Great Britain and Ireland, and the first demonstrating an aortic pseudoaneurysm.

[Laryngeal amyloidosis: a rare cause of hoarseness].

INTRODUCTION: The larynx is a rare site of deposition for amyloidosis. Diagnosis may be delayed and evoked in patients with prolonged hoarseness. We have reported two cases of laryngeal amyloidosis. EXEGESIS: One man and one woman suffered from hoarseness during one and three years, respectively. Laryngoscopic examination showed diffuse infiltration of the larynx. Amyloidosis was confirmed by the characteristic Congo-red staining of laryngeal biopsies. The search for other localizations of amyloidosis was negative. No monoclonal plasma cell proliferation was detected. Both patients received e…

[Hoarseness from the viewpoint of primary care].

Hoarseness is a symptom that is constantly encountered by doctors in primary care. It is the leading symptom in acute laryngitis and is often present as one of the symptoms in, for example, respiratory tract infections, influenza, and bronchitis. For the general practitioner, the infection factor as the cause of hoarseness is so dominant that other possible causes may fail to be taken into consideration. The general practitioner's possibilities for diagnosing the cause of this symptom are largely determined by habit, and by his ability to observe and assess the vocal chords. Examination of the…

Laryngeal amyloidosis causing hoarseness and airway obstruction.

Amyloidosis constitutes a fraction of 1% of benign localized laryngeal tumors and may occasionally be associated with systemic disease. A chronic, insidious, progressive, recurrent disease characterized by hoarseness, dyspnea, and stridor, it is caused by extracellular deposition of insoluble, abnormal tissue injurious fibrils. Submucosal lesions occur frequently in the vestibular folds and ventricles, less commonly in the subglottis and aryepiglottic folds and least on the vocal folds. Apple green birefrigence under polarized light after Congo red staining, electron microscopic fibrillar stru…

Evaluating hoarseness: keeping your patient's voice healthy.

Hoarseness is the term often used by patients to describe changes in their voice quality. The causes of hoarseness are determined after obtaining a detailed medical history of the circumstances preceding the onset of hoarseness and performing a thorough physical examination. The latter may include visualization of the vocal cords, possibly using indirect laryngoscopy, flexible nasolaryngoscopy or strobovideolaryngoscopy. In the absence of an upper respiratory tract infection, any patient with hoarseness persisting for more than two weeks requires a complete evaluation. When the patient has a h…

Hoarseness. From viral laryngitis to glottic cancer.

Hoarseness indicates an abnormality at the level of the glottis. This symptom may result from either structural or physiologic disorders. The differential diagnosis is lengthy and includes both benign and malignant diseases. History taking and physical examination, particularly laryngeal visualization, provide key clues. Careful and complete examination is always recommended, because airway obstruction can in some situations quickly follow hoarseness.

Pulmonary sarcoidosis presenting as hoarseness and mass lesion.

A 46-year male, ex-smoker presented with cough and hoarseness for 2-3 months. The CECT chest revealed a left upper lobe mass. BAL was negative for AFB, GeneXpert, and malignant cells. The bronchial biopsy showed granuloma with no caseation and infiltration of lymphoid cells. The ultrasound abdomen was normal. The patient was started on prednisolone 0.5 mg/kg. He improved clinic-radiologically. Ex-smoker, middle-aged man with hoarseness and mass lesion on CECT chest is highly suspicious of lung malignancy. Pulmonary sarcoidosis should be kept in the differential diagnosis of unexplained hoarsen…

[Laryngopharyngeal Reflux (LPR) in patients with persistent hoarseness].

INTRODUCTION: In 2006 The Global Consensus Group in Montreal pointed out that chronic laryngitis is highly associated with gastroesophageal reflux disease (GERD). AIM OF THE STUDY: To evaluate the frequency of LPR in a selected group of patients with chronic hoarseness. We were also interested in assessment of the relationship between Reflux Symptoms Index (RSI) scores, Ryan scores from the pharyngeal pH monitoring and the morphological changes in the larynx according to Reflux Findings Score (RFS). In addition, we wanted to assess the frequency of various clinical symptoms included in the RS…

[Prolonged hoarseness and arytenoid dislocation after endotracheal intubation].

We present a case of prolonged hoarseness after tracheal intubation in a 62-year-old woman with bronchial asthma who underwent a pylorogastrectomy under a combination of epidural and general anesthesia. After the induction of anesthesia, the patient was carefully intubated using a 7.5-mm cuffed endotracheal tube without a stylet to avoid causing an asthma attack; the patient was extubated approximately 6 hours after the tracheal intubation. On the first postoperative day, the patient complained of hoarseness, vocal fatigue, and dysphagia. A direct laryngoscopy performed by an otorhinolaryngolo…

Rare causes of voice hoarseness: a case report.

We report two contrasting and rare cases of voice hoarseness in young patients of tubercular aetiology. First case report is of isolated tubercular recurrent laryngeal nerve palsy in a patient who presented with hoarseness of voice. Chest radiograph showed a left hilar prominence and bronchial washings isolated acid-fast bacilli. Hoarseness of voice as an initial symptom due to isolated vocal cord palsy with no morphological lesions in the larynx and without obvious parenchymal infiltration often poses a diagnostic dilemma. Second case report highlights the possibilities of tuberculosis of the…
