[Home hospice care for the blood cancer patients].

In order to clarify the clinical characteristics along with the practical care that we provided, 21 terminally ill blood cancer patients who died at home from July 1 in 2003 to May 31 in 2010 were analyzed and compared with 733 non-blood cancer patients. The patients'characteristics such as age, gender and the home death frequency did not show a significant difference. The home care period for blood cancer patients showed that it tended to be a short duration, but not a statistically-significant difference. For the cases of longer period of more than 15 days at home, focus lists at each phase

Principles of wound care in hospice and palliative medicine.

The author presents a case report of an elderly patient who was extremely frail and debilitated. The patient was a resident of a skilled care facility because of advanced chronic illnesses that included end-stage Alzheimer, dementia, and Parkinson disease. This patient had acquired multiple stages III to IV pressure ulcers. Aggressive wound care treatment was initiated with partial success, but the patient passed away before the wound could completely heal. Medical literature was reviewed to offer the best possible wound care approach in providing care to patients who are terminally ill withou

Experience with a hospice-care program for the terminally ill.

A hospice-care program offers an opportunity to provide effective palliative care for patients terminally ill with malignant disease and to develop improved methods for coping with the problems of the dying patient. All patients for whom antitumor therapy does not offer a reasonable possibility of cure are eligible for Church Hospital's multidisciplinary program, the focus of which is on both the patient and his family. Acceptance by medical staff, patients and families has been enthusiastic. Both conventional and unconventional methods can be helpful in making terminally ill patients more com

Perspectives on hospice and palliative care in an academic setting: An exploratory interview study with patients and relatives to inform the development of an academic inpatient hospice.

BACKGROUND: Although palliative and hospice care are essential for individuals with life-limiting illnesses, academic inpatient hospices, which combine care with research and education, remain rare. This study explores the experiences, perceptions, and expectations of patients and their relatives regarding palliative and hospice care with the aim of informing the development of an academic inpatient hospice. METHODS: Semi-structured individual and dyadic interviews were conducted with patients and relatives who had experience with palliative and hospice care services in an academic setting in…

Developing person-centred care in hospices through the voice and leadership of nursing: lessons from the United Kingdom.

INTRODUCTION: Nursing leadership and the voice of nurses are crucial for developing person-centred care in hospices. Concerns have been raised that, as palliative care has evolved from its original vision and become more integrated into the mainstream healthcare system, it has increasingly become medicalised. This paper presents an emancipatory practice development programme aimed at enhancing the visibility and voice of nursing and nurse leadership to improve person-centred care in hospices across the United Kingdom. METHODS: The project was a 10-month collaborative education programme for n…

Assessing Clinical Reasoning in Hospice and Palliative Medicine Fellows.

INTRODUCTION: Clinical reasoning is essential for hospice and palliative medicine (HPM) fellows, yet structured assessment tools are lacking. The Assessment of Reasoning Tool (ART) has shown positive results for early learners but has not been applied to advanced learners. METHODS: We conducted an 18-month mixed-methods study at a single academic center. Seven fellows and seven faculty participated. Faculty used the ART every day during clinical teaching rounds on a fellow's inpatient rotation. Pre- and post-intervention surveys assessed the ART via 4-point Likert scale and free-response ques

Inpatient hospice admissions trend reversal: clinical audit.

OBJECTIVES: To evaluate admission reasons, referral sources, and patient outcomes among hospice inpatients, and to explore factors contributing to a reduced average length of stay observed locally. METHODS: A retrospective audit of all hospice inpatient admissions between September and December 2024 was undertaken. Data included referral source, diagnosis, reason for admission, length of stay and outcomes. Findings were compared with previous local and national data. A re-audit in March 2025 assessed whether trends persisted. RESULTS: A total of 130 patients were admitted during the initial

Palliative and hospice care in prostate cancer: A scoping review.

Advanced prostate cancer presents therapeutic and prognostic challenges at the end of life. Palliative and hospice care improve quality of life, reduce hospitalizations, and enhance patient-centered decision-making. This scoping review emphasizes therapy and prognosis by evaluating the utilization, timing, and clinical impact of palliative and hospice care in prostate cancer. We systematically searched MEDLINE (PubMed), CINAHL, Embase, APA PsycInfo, Scopus, and Web of Science for English-language studies published between January 2014 and March 2024. Eligible studies included adults with prost

Hypothalamic Hyperphagia in the Hospice Patient: Case Report and Discussion.

We present a case of insatiable appetite and harmful overeating in a hospice patient that showcases the burden of hypothalamic hyperphagia, a rare complication of brain cancer. While confounders exist such as progression of disease and prior tapering of dexamethasone, in this case the initiation of metformin was associated with substantial appetite reduction and resolution of our patient's debilitating food-seeking behaviors. We will explore metformin as a potential cost-effective option for palliation in a hospice setting and explore some of the physiologic mechanisms involved.

Understanding the home hospice experience of Puerto Rican caregivers.

Hospice use among Hispanic Medicare beneficiaries has declined in the last few years, and Hispanic caregivers have reported insufficient support around the emotional and spiritual aspects of care. Understanding the home hospice experience of Puerto Rican (PR) caregivers can yield insight into ways to improve hospice participation and quality of care for the Hispanic population. This exploratory study utilizes qualitative methods to identify PR caregivers' experience in the setting of home hospice care. Data from interviews with eight (n=8) bereaved PR caregivers of patients who received home h

Hospice agency characteristics associated with benzodiazepine and antipsychotic prescribing.

BACKGROUND: Benzodiazepine and antipsychotic medications are routinely prescribed for symptom management in hospice patients, but have significant risks for older adults. We explored the extent to which patient and hospice agency characteristics are associated with variations in their prescribing. METHODS: Cross-sectional analysis of hospice-enrolled Medicare beneficiaries aged ≥65 years in 2017 (N = 1,393,622 in 4219 hospice agencies). The main outcome was the hospice agency-level rate of enrollees with benzodiazepine and antipsychotic prescription fills divided into quintiles. Rate ratios w

Emergency Palliative Care: Hospice Patients in the Emergency Department.

In this segment of the emergency department (ED) palliative care (PC) case series, we discuss a patient on hospice who presented to the ED for the management of acute symptoms and potential procedural intervention. Hospice patients frequently visit the ED and can challenge typical ED paradigms of care that often include resuscitative efforts and broad workups. Our patient had a history of advanced liver cancer, and his ED presentation was prompted by worsening abdominal pain from ascites requiring serial paracentesis. PC consultation was requested to help address the patient's symptoms and cre

Variation in Benzodiazepine and Antipsychotic Prescribing Among Hospice Agencies.

BACKGROUND: Benzodiazepines and antipsychotics are routinely prescribed for symptom management in hospice. There is minimal evidence to guide prescribing in this population, and little is known about how prescribing varies across hospice agencies. OBJECTIVE: Examine patient- and hospice agency-level characteristics associated with incident prescribing of benzodiazepines and antipsychotics in hospice. DESIGN: Retrospective cohort study of a 20% sample of Medicare beneficiaries newly enrolled in hospice. PARTICIPANTS: Medicare hospice beneficiaries ≥ 65 years old between 2014 and 2016, restri

Palliative Ventilator Withdrawal Practices in an Inpatient Hospice Unit.

BACKGROUND: Palliative ventilator withdrawal (PVW) involves removal of mechanical ventilation in patients not expected to survive to allow a peaceful death. This process traditionally occurs in Intensive Care Units (ICU) and recently has evolved to occur in Inpatient Hospice and Palliative Care Units (IPU). OBJECTIVES: To describe the process and response of patients undergoing PVW in an IPU setting. METHODS: This is a longitudinal observational cohort study of adult patients who underwent PVW in an IPU from January 2021 through March 2022. RESULTS: Among 25 enrolled subjects, median age wa

Management of Wound Myiasis in the Hospice and Palliative Medicine Setting.

Complex wounds are common complications in hospice and palliative medicine (HPM), especially in patients with aggressive malignancies. Myiasis, or an infestation of maggots, is a rare but significant complication of such wounds. While uncommon in the United States, many HPM patients have multiple risk factors and comorbidities that increase their vulnerability to this condition. Currently, there are no standard diagnostic or treatment guidelines for wound myiasis. In addition, common management strategies may not be easily accessible in HPM settings. We present this case of a patient with mali

Dexmedetomidine as an Option for Opioid Refractory Pain in the Hospice Setting.

Background: Opioid refractory pain is a common problem in pain management. Dexmedetomidine is suggested to have opioid-sparing effects, with well-described use in surgical and intensive care unit settings. Some authors advocate its benefit in reducing delirium. Its effects are thought to be exhibited through agonism of pre- and postsynpatic α2-receptors in the central nervous system. It is more selective on α2-receptors than clonidine, accounting for its relatively lower incidence of hypotension. Its use in sedation is favored because it does not depress the respiratory system. The main side e
