Mineralisation patterns in the subchondral bone plate of the humeral head.

PURPOSE: Pathologic changes of the glenohumeral joint, like a long-standing overloading or an accident often lead to severe glenohumeral osteoarthritis, and a glenohumeral joint replacement could be necessary. Joint instability and glenoid loosening are the most common post-operative complications, which can be caused by eccentric loading of the glenoid, if the humeral head is malcentered. If these malcentered cases could be identified pre-operatively, the pathologic position of the humeral head could be fixed intra-operatively and complication may be prevented. Computed tomography osteoabsorp…

Intrathoracic fracture-dislocation of the humeral head: a case report.

Fracture-dislocation of the humeral head into the thoracic cavity is a rare injury. We present one such case in a 70-year-old woman. She presented with a 4-part fracture-dislocation of the proximal humerus, with displacement of the humeral head into the thoracic cavity. She had no signs of acute distress or hemodynamic instability. She underwent hemiarthroplasty of the right shoulder, but the humeral head fragment could not be removed. At 27-month follow-up, the patient had limited mobility of her right shoulder due to axillary nerve palsy but no pain or intrathoracic complications. In the abs…

[Neonatal traumatic epiphyseolysis of the humeral head. A rare occurrence].

Neonatal traumatic epiphyseolysis of the humeral head is rare, and only a few cases are reported in the literature. We present a case of a 13-day-old female newborn with malposition and relaxation of the left upper limb. The clinical examination showed distinct range of motion particularly for abduction. Magnetic resonance imaging (MRI) indicated epiphyseolysis of the humeral head. Closed reposition followed by immobilisation was done. The following MRI showed correct axis with adaption of the humeral head. Later, malposition of the axis with angulation in the ventromedial position was seen. T…

Vascularized scapular grafting for treatment of osteonecrosis of the humeral head.

Eight dissections in four embalmed cadavers were performed to investigate the possibility of vascularized scapular grafting for osteonecrosis of the humeral head. When the angular branch was used as the nutrient vessel, the mean length of the vascular pedicle was 12.4 cm, which was sufficient for transferring the scapula into the humeral head. Based on the anatomical study, a 27-year-old man with corticosteroid-induced osteonecrosis of the right humeral head was treated by vascularized scapular grafting. The pedicled vascularized scapula was successfully transferred into the humeral head using…

Shoulder arthroplasty in cases with avascular necrosis of the humeral head.

Avascular necrosis (AVN) is a relatively uncommon cause of glenohumeral arthritis. Previous retrospective reviews of shoulder arthroplasty for AVN have shown very good results in small numbers of patients. This study prospectively evaluated a consecutive series of 21 shoulders in 19 patients treated with the same modular prosthesis at a single institution. Of the shoulders, 8 developed AVN after a proximal humeral fracture, 1 was associated with a massive rotator cuff tear, 10 developed after corticosteroid therapy, and 2 were idiopathic. The patients, 14 women and 5 men, with a mean age of 54

Osteosarcoma of the humeral head associated with osteochondritis dissecans in a dog.

A 6-year-old, male Labrador retriever was presented for lameness of the right forelimb. The lameness occurred 5 years after the dog was surgically treated for osteochondritis dissecans (OCD) of the caudal right humeral head. Radiographs revealed an expansile, osteolytic lesion occupying the entire caudal half of the right humeral head, and biopsy confirmed the diagnosis of osteosarcoma. Forelimb amputation was performed. Histopathology of the lesion and associated articular cartilage revealed neoplastic osteoblasts extensively infiltrating the marrow space adjacent to scar tissue associated wi…

Posterior dislocation of the humeral head in association with obstetric paralysis.

Twelve children with obstetric paralysis were diagnosed as having a posterior dislocation of the humeral head. The diagnosis was suspected on clinical grounds and confirmed by computed tomography (CT) scans in all cases. All 12 patients were treated with open reduction via an anterior approach. The age range at the time of surgery was from 7 months to 7 years (average, 2 years and 3 months). All patients were immobilized in a shoulder spica for 6 weeks and a further 6 weeks in an orthosis. All patients were examined by CT scans in the postoperative period, which confirmed a satisfactory reduct…

Inferior subluxation of the humeral head after trauma or surgery.

Inferior subluxation of the humeral head can occur after shoulder trauma or surgery. One hundred consecutive patients were evaluated prospectively after shoulder surgery or injury. The radiographic incidence of inferior subluxation of the humeral head 2 weeks after rotator cuff repair was 10%. The radiographic incidence of inferior subluxation after fracture of the proximal humerus was 42%, and the incidence 2 weeks after prosthetic humeral head replacement was 60%. The immediate postoperative radiograph showed an inferior subluxation of the humeral head in 4% of patients after prosthesis inse…

Measurement of the anteroposterior translation of the humeral head using MRI.

Anteroposterior translation of the humeral head within the glenohumeral joint was investigated using MRI. Ten normal shoulders, 11 recurrent anterior dislocations, 10 stabilised shoulders after the Putti-Platt operation and one shoulder with multidirectional instability, were scanned. The arm was positioned in internal and external rotation and in an overhead position. Gradient echo volume acquisition scans were carried out, and 22 were suitable for evaluation. Consistent results were obtained in the normal shoulders. The unstable and unstable shoulders showed higher variance, but there was no…

[Osteochondral lesions of the humeral head: the usefulness of echographic study].

PURPOSE: In the cranial humeral end, osteochondral injuries localize in a circular crown including part of the humeral head and part of the major and minor tuberosities. Since this region is easy to depict with US, we investigated the potentials of this technique in detecting osteochondral injuries. MATERIALS AND METHODS: Seventy-five osteochondral injuries found at 492 US examinations performed in 12 months with a 7.5 MHz linear probe were retrospectively reviewed. Clinical history taking was focused on the following: a) previous trauma or b) luxation-instability and c) if the patient was a …

Superolateral erosions of the humeral head in chronic inflammatory arthropathies.

Erosive lesions on the superolateral aspect of the humeral head were studied in 127 patients with chronic inflammatory arthropathies including rheumatoid arthritis (RA), juvenile rheumatoid arthritis (JRA), ankylosing spondylitis (AS), and psoriatic arthropathy (PA), as well as in a control group of 53 patients with non-inflammatory shoulder joint disease. Superolateral erosions were found in 39 out of 127 patients (31%), comprising 11/56 RA cases (20%), 22/50 JRA cases (44%), 4/9 cases of AS (44%), and 2/12 cases of PA (17%), but were absent in non-inflammatory disorders. Two morphologically …

[Fractures of the head of the humeral condyle in children and characteristics of their treatment].

The author has examined 165 children with FHCH. A method of surgical treatment of FHCH has been worked out and applied in 37 children; its essence consists in the open reposition of the fractures and the fixation of the fragment of the head with an original construction compression device which provides the possibility of making early active movements preventing rigidity in the elbow joint. The method does not envisage the necessity of fixation of the extremity with a plaster bandage and reduces the period of clinical and general treatment (by 18 and 14 days respectively). The economic damage

Arthroscopic treatment of steroid-induced osteonecrosis of the humeral head.

Shoulder arthroscopy was performed on a 52-year-old man for the treatment of steroid-induced osteonecrosis of the humeral head. The removal of loose bodies and joint debridement has successfully improved the patient's functional status through relief of pain, improved range of motion, and elimination of locking. This case suggests another use of arthroscopy in the treatment of shoulder pathology.

Chronic locked posterior gleno-humeral dislocation: technical note on fibular grafting for restoration of humeral head sphericity.

BACKGROUND: Reconstruction of reverse Hill-Sachs defect using osteo-chondral allograft has the advantages of spherical re-contouring and provision of smooth biological articular surface of the reconstructed humeral head. However, worldwide availability and risk of disease transmission of osteo-chondral allograft remain points of increasing concerns. As an alternative to lacking osteo-chondral allograft, the current technical note describes a reconstructive technique of reverse Hill-Sachs defect using autologous fibular grafting. METHODS: Following open reduction of the dislocated humeral head…
