Recurrent Caffey's cortical hyperostosis and persistent deformity.

A boy, age 12 years, who had infantile cortical hyperostosis has continued to have occasional aches and new cortical thickenings in his arms and legs. His mandible is undergrown and his ribs have an abnormal slope. Recurrence of Caffey's cortical hyperostosis and persistent deformity have been observed in other children and young adults. Some unexplained cases of late cortical thickening and pain may be due to Caffey's disease.

[Hyperostosis frontalis interna : etiology and differential diagnosis].

Hyperostosis frontalis interna was first described in 1719 in association with obesity and hirsutism, forming Morgagni's syndrome. A high prevalence and a lack of studies demonstrating a strong correlation between these different signs currently question the existence of such a syndrome. Hyperostosis frontalis interna predominates in women. The anomaly exclusively involves the inner table and constantly spares the diploe and the external table. The main differential diagnosis of cranial hyperostosis is made between meningioma, osteoma, Paget's disease and fibrous dysplasia. The clinical implic…

Thoracolumbar Fracture in Disseminated Idiopathic Skeletal Hyperostosis.

Disseminated idiopathic skeletal hyperostosis (DISH) is a form of ankylosing spinal disorders, which is at high risk of fracture because of the rigidity of the spinal column and reduced bone quality. The patients with DISH are at higher risk of fall because of the poor muscle tone, rigid spine column, and positive sagittal balance. The management of spinal fractures in these patients proves to be challenging because of the altered biomechanics and alignment of the spine. Furthermore, most patients have multiple comorbidities with high intraoperative burden, and osteoporosis itself will impair …

Internal Acoustic Canal Stenosis Due to Hyperostosis.

Background  Exostoses and osteomas are benign, insidious lesions of the bone involving the internal acoustic canal (IAC). We present two cases of IAC exostoses managed with surgical decompression and review the clinical outcomes of previously reported cases in the literature. Methods  A comprehensive search was conducted using PubMed Central, Web of Science Core Collection, and Google Scholar databases to identify previous reports of IAC exostoses and osteomas. A total of 26 reported cases were identified, and patient presenting symptoms, management strategies, and response to surgery was obta

Otolaryngologic manifestations of diffuse idiopathic skeletal hyperostosis.

Diffuse idiopathic skeletal hyperostosis (DISH) is characterized by formation of large cervical osteophytes that may compress the posterior wall of the aerodigestive tract. It is a rare cause of dysphagia in the elderly. The aim of this study was to investigate the various otolaryngologic manifestations of DISH. Eleven elderly patients with DISH were included in the study. All patients presented with dysphagia that was graded on the swallowing screening tool (EAT-10), and the diagnosis of DISH was based on computed tomographic criteria. The patients were subjected to otolaryngologic examinatio…

Idiopathic hypoparathyroidism mimicking diffuse idiopathic skeletal hyperostosis.

Idiopathic hypoparathyroidism is an uncommon disease caused by insufficient secretion of parathyroid hormone. Idiopathic hypoparathyroidism may cause various musculoskeletal findings, including diffuse ligamentous and entheseal ossifications. Diffuse idiopathic skeletal hyperostosis is a disorder of the elderly characterized by ossification of the anterior longitudinal ligament of the spine and various extraspinal ligaments. We present a 50-year-old man with idiopathic hypoparathyroidism who had been diagnosed as having DISH at 40 years of age. Resistant neck, left shoulder, and left hip pain …

Infantile cortical hyperostosis (Caffey disease): a review.

PURPOSE: Face swelling in infants may have several causes including infantile cortical hyperostosis (Caffey disease), an inflammatory process with swelling of soft tissues and periosteal hyperostosis of some bones. New insights show that this self-limited condition is collagen I-related. PATIENTS AND METHODS: Collagen I is the most important component of bone and dentine. We reviewed literature to lighten this new collagenopathy, the first one with a self-regressive course. RESULTS: After describing a typical case and the clinical and radiologic features of the disease, we discuss the pathog…

Prenatal cortical hyperostosis with COL1A1 gene mutation.

Infantile cortical hyperostosis (Caffey disease) is benign and self-limiting when it presents near or after birth but it is usually lethal when it presents earlier. We present the clinical, ultrasonic, radiographic, and pathologic findings in an instructive case of early onset prenatal cortical hyperostosis. The pregnancy of a 21-year-old woman was medically terminated at 30 weeks of gestation after a diagnosis of severe osteogenesis imperfecta. Prenatal ultrasounds showed short long bones. Postmortem radiographs showed hyperostosis in long bones, ribs and mandible. The affected skeleton showe…

Severe prenatal infantile cortical hyperostosis (Caffey's disease).

We describe three cases of prenatal infantile cortical hyperostosis (Caffey's disease) from two families, all associated with maternal polyhydramnios. Case 1 (family 1) was an early early neonatal death after delivery at 27 weeks gestation, case 2 (family 2) an intrauterine death at 33 weeks. Case 3 (family 2) had limited skeletal involvement and followed a course typical for Caffey's disease. Only six cases of prenatal Caffey's disease with extensive skeletal involvement have previously been described. Polyhydramnios was reported in all but one and the condition was lethal unless pregnancy re…

Aspiration pneumonia due to diffuse cervical hyperostosis.

Diffuse idiopathic skeletal hyperostosis previously has been reported to cause a number of extraspinal manifestations including dysphagia, respiratory distress, dysphonia and cervical myelopathy. We report a case of cervical DISH so extensive as to interfere with the swallowing mechanism and lead to aspiration. Patients with DISH who have mechanical compression of the posterior pharynx may be at high risk for aspiration.

[Sternocostoclavicular hyperostosis (Köhler's disease). A new case].

The authors report a new observation of Köhler's disease. Since Köhler's description in 1975, 40 observations have been noted. It concerns a particular entity characterized by a hyperostosis usually bilateral sternocosto clavicular able to associate other osseous lesions and cutaneous eruptions. Its etiology remains unknown.

Endemic disease patterns in paleopathology: porotic hyperostosis.

Recent research has shown that constitutional factors can elicit a porotic skeletal lesion pattern related to iron-deficiency anemia, even when adequate dietary iron is available. This study considers the pattern of skeletal involvement under conditions of chronic or endemic dietary stress. Analysis focused on 54 subadults aged 0-10 years at death from the Arroyo Hondo site. Early age of onset is documented in the pattern of coincident active periosteal reactions and porotic lesions under 6 months. Endemically inadequate diets affecting pregnant females and their fetuses, acting synergisticall

[Peripheral localizations in diffuse idiopathic skeletal hyperostosis].

The extra-spinal manifestations of diffuse idiopathic skeletal hyperostosis (DISH) are described in 73 consecutive cases, detected during radiographic screening of the thorax for other problems. The characteristic spinal changes (calcification and ossification of the ligaments in the antero-lateral aspects of the vertebral bodies) are present in all individuals. Extra-spinal changes include "whiskering" at the site of the attachment of muscle in the pelvis and long bones, ligament and tendon ossification, periarticular osteophytes, and spurring of the calcaneous and the olecranon. The radiogra…
