STUDY OBJECTIVE: To demonstrate techniques of ureterolysis during complex laparoscopic hysterectomy.
DESIGN: Technical video demonstrating different approaches to ureterolysis for complex benign pathology during laparoscopic hysterectomy (Canadian Task Force classification III).
SETTING: Benign gynecology department at a university hospital.
INTERVENTION: Performance of ureterolysis during laparoscopic hysterectomy for benign pathology.
CONCLUSION: Ureteric injury has significant morbidity and is the most common reason for litigation following hysterectomy, with an estimated risk of 0.02% …

Although vaginal hysterectomy has long been championed by the American College of Obstetricians and Gynecologists as the preferred mode of uterine removal, nationwide vaginal hysterectomy utilization has steadily declined. This article reviews the evidence comparing vaginal with other modes of hysterectomy and highlights areas of ongoing controversy regarding contraindications to vaginal surgery, risk of subsequent prolapse development, and impacts of changing hysterectomy trends on resident education.

OBJECTIVE: Describe the epidemiology, diagnosis and treatment of vesicovaginal fistula (VVF).
PATIENTS AND METHODS: We conducted a retrospective descriptive study of all cases of VVF secondary to hysterectomy. The following parameters were studied: age, parity, indication for hysterectomy, risk factors, the consultation period, the anatomical type of VVF, the paraclinical, the surgical approach and results of the cure.
RESULTS: Fourteen cases were identified over 10 years. All hysterectomies were performed by laparotomia. The average age of patients was 54.3±13 years. Hysterectomy was perfor…

Endoscopic hysterectomy is increasingly selected as a current trend to minimize invasion, tissue trauma and early recovery. However it has disadvantages of the difficulty to learn and needs expensive equipments. So we developed a new minimally invasive method of vaginal hysterectomy-minilaparotomically assisted vaginal hysterectomy (MAVH) in order to complement the current laparoscopic surgery. The principle of MAVH is based on suprapubic minilaparotomical incision and uterine elevator that allows access and maximal exposure of the pelvic anatomy and an easy approach to the surrounding anatomy…

OBJECTIVES: The indications for hysterectomy after caesarean section, postoperative period, and pathological examination of women. In whom hysterectomy hampered operated delivery, are being hereby analysed.
MATERIALS AND METHODS: The study group consisted of 15 patients who gave birth in I Clinic Medical Academy in Warsaw in 1995-2001.
RESULTS: In 7 with 15 women (46.7%) the supravaginal amputation was carried on at the rest in 8 (53.3%), the corpus and cervix of uterine was resected. In pathomorphological examination a trophoblast growth into uterine muscle was found. In 8 women (53.3%). In…

OBJECTIVES: To determine the feasibility and acceptability of minilaparotomy-assisted vaginal hysterectomy.
METHODS: A prospective pilot study in a general hospital was conducted. Twenty patients who were on the waiting list for abdominal hysterectomy were included in the study. All these patients had one or more relative contraindications to vaginal hysterectomy. The hysterectomy procedure was started vaginally in all cases. A minilaparotomy incision was performed to complete the procedure if vaginal hysterectomy was not feasible. Results were analyzed on the intention to treat basis.

AIM OF STUDY: To review the cases of caesarean and post-partum hysterectomy performed over a 10-year period (1986-1996) in a teaching hospital, looking specifically at the associated morbidity.
METHOD: Retrospective review of case files.
RESULTS: During the study period, there were 14 cases of caesarean and post-partum hysterectomy. Caesarean hysterectomy was performed in 0.17% of caesarean sections and a hysterectomy was done in 0.02% of cases following a vaginal delivery. Uterine atony was associated with 43% of cases. Histological evidence of placenta accreta or increta was noted in 7 (50…

OBJECTIVES: To compare the clinical course and outcome of patients with post-partum tetanus (n = 21) to those of non-post-partum ones (n = 24) and to determine the effect of early hysterectomy on the survival of cases with severe post-partum tetanus (n = 20).
METHODS: In this prospective study we compared the clinical course and outcome of post-partum and non-post-partum cases of tetanus admitted to the intensive care unit. Moreover, early hysterectomy was done on every other patient with severe post-partum tetanus (n = 10). Chi-square and Student's t-tests were used for statistical purposes.…

OBJECTIVE: To report our experience with laparoscopically assisted vaginal hysterectomy (LAVH) and evaluate the advantages of LAVH.
STUDY DESIGN: From January 1991 to August 1992, 176 LAVHs were performed at the Department of Obstetrics and Gynecology, College of Medicine, Chung-Ang University, Seoul, Korea. The indications for LAVH, based on the preoperative diagnosis, were uterine myomas, dysmenorrhea, chronic pelvic inflammatory disease or pelvic pain, dysfunctional uterine bleeding and cervical intraepithelial neoplasia (in order of frequency).
RESULTS: Patients had concomitant procedure…

Eleven patients with stage IA2-IIA carcinoma of the cervix have been treated by combined laparoscopic-vaginal radical hysterectomy and bilateral pelvic lymphadenectomy (3-Stage IA2, 5-stage IB, 3-Stage IIA). The patients were unselected. Three patients had bulky (>e; 5 cms) tumors, one of whom weighed 239 lbs; one had prior anterior-posterior repair, was apareunic and had significant vaginal narrowing; two patients had extensive pelvic adhesions, one of whom also had a 480 gram uterus. Pelvic lymph node metastases were present in one patient and paracervical lymph node metastases in one. The…

The Authors carried out a retrospective examination of data relating to 241 patients undergoing radical hysterectomy with pelvic lymphadenectomy between 1970 and 1991. The basic aim of the study was to identify and quantify complications correlated to radical hysterectomy using the Wertheim-Meigs method. Surgery was performed in 223 cases due to cervical cancer, in 13 cases due to endometrial cancer, in 3 cases due to vaginal cancer and in 2 due to ovarian cancer with secondary extension to the uterus. Complications were subdivided into intraoperative and postoperative. Intraoperative complica…

OBJECTIVE: To evaluate complications of laparoscopic hysterectomy.
METHODS: A retrospective analysis of complications was tabulated from hospital and office records. Forty-five consecutive patients had laparoscopic hysterectomies from November 1991 to November 1992. Complications were divided into operative, anesthetic, postoperative, nursing, and equipment. Each complication was subjectively graded from mild to severe.
RESULTS: Complication rates by group were: operative 11%, anesthetic 7%, postoperative 16%, nursing 4%, and equipment 56%. Operative complications included one bladder perfor…

Efficacy data for single-dose cefazolin prophylaxis at hysterectomy are meager, and there are none evaluating the impact of route of administration on efficacy. For these reasons, 772 women undergoing elective abdominal or vaginal hysterectomy for benign diseases were given 1 g cefazolin either intramuscularly or intravenously in a randomized clinical trial. Preoperative diagnoses and clinical, surgical, and outcome variables were similar by route of administration for each surgical approach. Risk factors for infection after abdominal hysterectomy included younger age, lower postoperative hemo…

In most instances, patients with an acute abdomen and pneumoperitoneum have a perforated hollow viscus as the underlying cause. Clinicians must not forget, however, some of the less common causes of free air in the abdomen. We describe pneumoperitoneum from a vaginoperitoneal fistula, developing after hysterectomy, well known to gynecologists, but rarely noted in the medical literature.
