Gas embolism has been described as a complication of operative hysteroscopy. We present one of two non-fatal case stories where gas embolization was suspected due to symptoms observed by the anaesthesiologist and discuss how to prevent it.

A 32-year-old woman who had a history of abruptio placentae and cesarean section with her first pregnancy experienced vaginal hemorrhage 6 weeks after her last menstrual period. Congenital anomalies of the uterus have been estimated to affect 0.06-10% of women. Uterine septae account for approximately 80% to 90% of these anomalies and is also associated with the poorest reproductive outcome. Pregnancy loss in patients with a septate uterus has been reported to be as high as 90% after other causes for miscarriages have been excluded. We present here the first experience in Jamaica, of a successful pregnancy outcome after hysteroscopic metroplasty in a woman with a septate uterus and three previous miscarriages.

OBJECTIVE: A retrospective study of short and long term results of transcervical endomyometrial resection for menorrhagia.
MATERIAL AND METHODS: Patient data were collected from all 104 premenopausal women who had undergone a transcervical endomyometrial resection due to severe menorrhagia in 1990-95. Almost 40% had submucous fibromas that were resected together with the endometrium. A questionnaire about gynecological symptoms was mailed to all 104 women. Absorption of irrigating solution may involve serious complications during hysteroscopic surgery. This absorption occurs mainly into the vessels opened during the procedure. Careful perioperative monitoring of the deficit of collected irrigating medium during transcervical surgery is mandatory. Significant absorption seems to be connected with the development of discrete cerebral oedema and nausea, secondary to dilutional hyponatraemia and elevation of several amino acids.

DESIGN: Retrospective record review.
SETTING: A credentialing program in teaching and nonteaching hospitals.

The hysteroscopic evaluation of uterine openings in 23 sterile women was described. The attention was paid to morphological features of obliterated uterine openings. The hysteroscopic examination of structural changes located in uterine openings seems to be a reliable method for evaluation of potency of oviducts. Clinical presentations of chronic endometritis have been compared with hysteroscopic findings in 60 women of reproductive age. Clinical and hysteroscopic data were indicative of a close correlation of gross endometrial abnormalities with certain symptoms of chronic endometritis and of severity and pattern of mucosal lesions of the corpus mucosa. Elucidation of clinical and hysteroscopic features of chronic endometritis permits a differential approach to the management of these patients.

The advent of hysteroscopic laser surgery has provided an alternative to hysterectomy in women with menorrhagia refractory to other forms of treatment. Seventy patients who underwent hysteroscopic metroplasty for uterine septa during a 6-year period were reviewed for subsequent gynecologic problems and obstetric outcome. Complete follow-up was obtained for 1 year in 60 patients and through the first pregnancy in 64. The 70 patients were divided into three groups based on obstetric history prior to surgery: 1) 40 with only first-trimester losses, 2) 15 whose histories included second-trimester losses or premature delivery, and 3) 15 with primary infertility. Operative hysteroscopy for mullerian fusion defects in properly selected cases and in experienced hands is a safe and effective method of management of uterine septal defects associated with recurrent pregnancy losses. For the patient, it is a more acceptable alternative because it avoids laparotomy, a lengthy hospital stay, and blood transfusions. Medical costs are reduced compared with more traditional approaches. While providing the same level of success, this technique makes a vaginal delivery possible.

Thirty-three patients were diagnosed by hysterosalpingography and laparoscopy as having septate uteri, which were assumed to be responsible for their infertility, recurrent abortions, or premature labor. Twenty-eight patients who underwent resection of a submucous fibroid under hysteroscopic control are reported with a follow-up for one to seven years. Seven patients underwent subsequent hysterectomy for various reasons. Two required repeat hysteroscopic resection and 17 returned to normal menses without difficulty. Five of these patients subsequently had a total of eight pregnancies, with five living children. The implications of this option of therapy are discussed.
