The influence of incubator wall on the irradiation intensity of phototherapy.

A clear dose-response relationship has been demonstrated between the irradiance and the effectiveness of phototherapy. This study is designed to detect if there is any difference in irradiance between the inside and outside of different types of incubators. Forty-four incubators of two different types and with ages up to 18 years were included. In comparison with the measurement taken outside an incubator, the mean spectral irradiance detected within a single-walled incubator was significantly decreased (p < 0.001) while that detected within a double-walled incubator was slightly increased (p

The bedside practice of sonographic guided internal jugular vein access in critically ill premature infants.

BACKGROUND: Intrahospital transport, general anesthesia, and the prolonged duration of the central venous catheterization (CVC) in unfavorable conditions pose a significant risk to a critically-ill premature infant. We aimed to demonstrate a minimalized and safe manner of CVC in this patient population. METHODS: We worked on a prospective study in 51 critically-ill premature infants in which a 22 Gauge catheter was put in one of the central thoracic veins with the guidance of sonography as a bedside procedure. Of the patients, 27 (53%) were extremely premature, and 21 (41%) were extremely low

Incubator baby side-shows: the history and future of neonatology.

The care of infants has a somewhat sordid history, not without its side-show elements, political intrigues, and amazing success. Abandonment of infants and outright infanticide was not uncommon in ancient times. The Roman Church, however, was instrumental in beginning to rescue abandoned infants. Over time, greater recognition was afforded newborns and young children with the advent of dispensaries and hospitals created for the purpose of administering to this population. France was a focal point for the evolution of the modern care of newborns, with development of incubators at the end of the

Postnatal fetal and adult hemoglobin synthesis is preterm infants whose birth weight was less than 1,000 grams.

To determine if environmental factors could effect the switchover from fetal hemoglobin (HbF) to adult hemoglobin (HbA) synthesis, studies were carried out on blood samples from eight infants born at less than 1,000 g, when they had reached their postconceptional age corresponding to term. All of these infants required prolonged intensive care, multiple blood transfusions, and two required exchange transfusions. Several were ventilated mechanically for 60 d and two infants had bronchopulmonary dysplasia at the time of the study. The blood samples were incubated in an amino acid mixture contain

Reducing radiation doses to the breast, thyroid and gonads during diagnostic radiography.

The skin entry and exit doses on patients undergoing routine radiographic examinations of areas in which the breast, thyroid and gonads are included in the primary beam were measured using thermoluminescent dosimeters. To obtain further information about patient doses, measurements were also made on a phantom at similar skin positions and at the positions of these organs. Comparisons of the doses to these radiosensitive organs were made for the antero-posterior and postero-anterior projections. In cases where it was found that the doses were reduced by the use of non-conventional relationship

Osmotic fragility of erythrocytes in newborn infants treated by phototherapy.

In order to determine the in vivo effect of phototherapy on the erythrocytes' osmotic fragility, we have tested 10 preterm infants treated by continuous phototherapy, for jaundice of prematurity. Heparinized blood was obtained daily, starting before the initiation of phototherapy and ending 24 h after stopping the treatment. The osmotic fragility was tested in duplicate immediately, after 1 h incubation at 40 degrees C and after 24 h incubation at 4 degrees C in darkness. The erythrocytes of the preterm infant during and at the end of phototherapy showed a normal osmotic fragility pattern. Com

Hearing loss in low-birth-weight infants.

The hearing of 98 perinatal intensive care survivors with a mean birth weight of 1,540 g was assessed at a mean age of 6 1/2 years. They represented 73% of the long-term survivors with birth weights of 1,800 g or less who had been cared for in our neonatal unit during the three-year period 1971 through 1973. Nine of the 98 infants had sensorineural hearing loss, and 14 had exudative otitis media. During their neonatal period, the infants with hearing loss experienced more frequent apneic attacks, hyperbilirubinemia (serum bilirubin level, greater than 14 mg/dL), and hypothermia compared with t

Rapid detection of microorganisms in blood cultures of newborn infants utilizing an automated blood culture system.

BACKGROUND: Neonatal sepsis is a low incidence, high-risk disease with many sepsis work-ups performed to detect a single case. Seventy-two hours of antibiotic therapy have been traditionally recommended pending negative culture results. Improved culture media and new technology integrated into blood culture systems could shorten incubation time required to detect positive culture results. This would then change the length of antibiotic therapy in the management of the newborn infant with suspected sepsis. In addition, previous data supporting the 72-hour recommendation were retrospectively acq

Transepidermal water loss during halogen spotlight phototherapy in preterm infants.

Among preterm infants there is a relationship between skin blood flow and transepidermal water loss (TEWL). The aim of this study was to assess whether halogen spotlight phototherapy without significant heat stress increases TEWL and affects maintenance fluid requirements in preterm infants. TEWL was measured noninvasively before the start and after 1 h of halogen spotlight phototherapy in a group of preterm infants, nursed in double-walled incubators with moderately high relative humidity. Relative humidity and ambient temperature in the incubator were tightly controlled. Mean +/- SD birth we

Different factors govern surface adhesion and resilience to gastrointestinal conditions in Cronobacter sakazakii.

Cronobacter species are opportunistic pathogenic bacteria present in many foods that can cause life-threatening infections in newborns and young infants. The infective potential in other vulnerable consumer groups remains unclear. In this study, twenty-eight strains of Cronobacter sakazakii from food and human clinical sources were characterized regarding their surface adhesion ability in 96-well plates at different incubation temperatures, incubation times, and cultivation media. Based on that, eight C. sakazakii isolates were selected and further studied regarding their adhesion ability to n

Free fatty acids in the development of breast milk jaundice.

The incubation of milk, at 4 degrees C, from mothers of infants with breast milk jaundice (BMJ) is reported to result in significantly higher levels of free fatty acids (FFA) compared with milk from controls. Single milk samples collected under standard conditions were obtained from four mothers of infants with BMJ and 14 control donors matched for stage of lactation. Milk samples were analyzed for the concentrations of FFA, using thin-layer gas chromatographic techniques. In addition, serum total fatty acids were measured in mothers and infants. The concentrations of FFA increased after stora

Influence of thermal drive on central sleep apnea in the preterm neonate.

BACKGROUND: The incidence of apnea in neonates depends on a number of factors, including sleep state and thermoregulation. OBJECTIVE: To assess the role of thermal drive (body heat loss [BHL]) in the mechanisms underlying short episodes of central apnea during active and quiet sleep in neonates. MATERIAL AND METHOD: Twenty-two neonates (postconceptional age: 36.3 +/- 0.9 weeks) were exposed at thermoneutral (incubator temperature: 32.5 degrees C), warm (34.2 degrees C), and cool (30.4 degrees C) conditions during 3 consecutive morning naps. Oxygen consumption (VO2), skin and rectal temperatu

Aeromedical transport: its hidden problems.

Air transport can move patients safely and rapidly over long distances. However, changes in altitude can have disastrous effects because diminished ambient air pressure may allow gases in closed spaces and tissues to expand rapidly. Even pressurized commercial aircraft do not maintain sea-level pressure: cabin pressures equal to those at yp to 8000 ft may be experienced, diminishing oxygen tension in proportion. Air transport is absolutely contraindicated for patients with untreated pneumothorax, gas gangrene, or air trapped in the cranium and those who have recently undergone abdominal surger

Two-step or one-step - are all methods for neonatal incubator decontamination equal?

Healthcare-associated pathogens, including Staphylococcus capitis, can contaminate incubator surfaces and are of significant concern in neonatal intensive care units (NICUs). Effective incubator decontamination is essential for infection prevention and control, with submersion decontamination often recommended. This may not always be achievable, with wipe decontamination seen as an alternative. Here we compare the ability of a two-step (submersion in enzymatic detergent followed by wiping with hypochlorite-based wipes) with a one-step (wiping with quaternary ammonium compound-impregnated wipes

Mist and water condensation inside incubators reduce the efficacy of phototherapy.

OBJECTIVE: To measure the irradiance in humidified incubator under three different overhead phototherapy devices. DESIGN/METHODS: The effective irradiance of three phototherapy devices was assessed by taking a series of irradiance measurements in the illuminated field. Measurements were made with a fixed bandwidth broadband radiometer (380-530 nm). The distance between the light source and the radiometer was 35 cm for the daylight fluorescent lamp, 40 cm for the light emitting diode (LED) and 50 cm for the halogen phototherapy. A double-wall incubator was kept at 36°C and set at three differe

Improving neonatal unit admission temperatures in preterm babies: exothermic mattresses, polythene bags or a traditional approach?

OBJECTIVE: To investigate whether exothermic sodium acetate mattresses were associated with an improvement in the thermal care of babies <30 weeks gestation between birth and admission to a neonatal unit. STUDY DESIGN: Analysis of a three case series of babies: the first with traditional thermal care of drying and wrapping in a towel, the second with wrapping in food standard polythene bags and the third with wrapping in polythene bags and nursing on an activated exothermic mattress. The main outcome measure was the temperature on admission to the neonatal unit. RESULT: There were no signifi
