Chronic liver injury scores are superior prognostic indicators of outcomes in severe alcohol-related burns.

Acute and chronic alcohol abuse are common among burn patients and may be associated with chronic liver injury, a potential factor influencing outcomes. This study evaluates the predictive power of the blood alcohol concentration (BAC) and non-invasive liver fibrosis scores and their applicability in burn patients. A retrospective analysis was conducted on patients admitted to a high-volume supraregional burn center in Northern Germany between 2007 and 2024. Patients were categorized based on their BAC at admission: low (< 100 mg/dL) vs. high (≥ 100 mg/dL). Data collected included demographics

Derivation of a Predictive Score for Hemorrhagic Progression of Cerebral Contusions in Moderate and Severe Traumatic Brain Injury.

BACKGROUNDS: After traumatic brain injury (TBI), hemorrhagic progression of contusions (HPCs) occurs frequently. However, there is no established predictive score to identify high-risk patients for HPC. METHODS: Consecutive patients who were hospitalized (2008-2013) with non-penetrating moderate or severe TBI were studied. The primary outcome was HPC, defined by both a relative increase in contusion volume by ≥30 % and an absolute increase by ≥10 mL on serial imaging. Logistic regression models were created to identify independent risk factors for HPC. The HPC Score was then derived based on

Outcome of hand trauma: the hand injury severity scoring system (HISS) and subsequent impairment and disability.

This study was designed to evaluate correlations between the hand injury severity scoring system (HISS) and measures of impairment and disability obtained 6 months after a hand injury. A statistically significant positive correlation was found between the severity of the injury (HISS) and residual impairment, as measured with the American Medical Association's (AMA) "Guides to the evaluation of permanent impairment, 4th edition". No statistically significant correlation was found between the severity of the injury (HISS) and the Disabilities of the Arm, Shoulder and Hand (DASH) questionnaire,

Association Between Illness Severity Scores and Quantitatively Measured Brain Injury in Cardiac Arrest Survivors.

Introduction: This study explored how illness severity scores correspond to hypoxic-ischemic brain injury (HIBI) after cardiac arrest. Methods: This study included cardiac arrest survivors with sufficient data to calculate the Pittsburgh Cardiac Arrest Category (PCAC) and revised post-cardiac arrest syndrome for therapeutic hypothermia (rCAST) scores who underwent brain magnetic resonance imaging and cerebrospinal fluid neuron-specific enolase (CSF-NSE) measurement within 6 h after return of spontaneous circulation. The primary outcome was the association of PCAC and rCAST with quantitative br

Development of a pneumonia risk score for post-acute rehabilitation in patients with severe acquired brain injury.

OBJECTIVE: To develop a pneumonia risk score for patients with severe acquired brain injury (sABI) hospitalized for post-acute rehabilitation. METHODS: A prognostic model with data from 1,625 patients with sABI who were pneumonia-free at admission for rehabilitation. Candidate predictors were gathered within 72 hours following admission. Prognostic risk scores were calculated based on hazard ratios and combined in an overall pneumonia risk score ranging 0-10. RESULTS: Predictors were age >50 years hazard ratio (HR) 1,83 (95% CI: 1.33; 2.53), male sex HR 2.43 (95% CI: 1.70; 3.46), diagnosis (

REAL-TIME EVALUATION OF BRAIN INJURY SEVERITY IN SEPTIC MICE USING A NOVEL SCORING SYSTEM.

Objectives : The difficulty in translating findings from basic research on sepsis associated encephalopathy (SAE) into clinical practice may be attributed to the suboptimal assessment methods currently in use. The objective was to develop an assessment index to dynamically evaluate brain injury severity in the septic acute phase, making the experimental data more representatives of clinical SAE patients. Methods : We independently developed the Sepsis-Associated Brain Injury Score (SABIS) based on Quick Sequential Organ Failure Assessment (qSOFA), Full Outline of Unresponsiveness, and Confusio

A contemporary simple risk score for prediction of severe acute kidney injury after heart transplantation.

BACKGROUND: The aim of this study was to develop a simple risk score to estimate severe acute kidney injury (AKI) risk based on a large contemporary heart transplantation (HT) cohort. METHODS: From 1 January 2015 to 31 December 2021, all consecutive HT recipients in our institute were included and analysed for the occurrence of AKI within the first seven postoperative days. Patients transplanted between 2015 and 2019 comprised the derivation cohort, and those transplanted between 2020 and 2021 formed the validation cohort. The primary endpoint was severe AKI (AKI stages 2-3). The impact of se

Optimization of MRI-based scoring scales of brain injury severity in children with unilateral cerebral palsy.

BACKGROUND: Several scoring systems for measuring brain injury severity have been developed to standardize the classification of MRI results, which allows for the prediction of functional outcomes to help plan effective interventions for children with cerebral palsy. OBJECTIVE: The aim of this study is to use statistical techniques to optimize the clinical utility of a recently proposed template-based scoring method by weighting individual anatomical scores of injury, while maintaining its simplicity by retaining only a subset of scored anatomical regions. MATERIALS AND METHODS: Seventy-six

Arterial reconstruction after mangled extremity: injury severity scoring systems are not predictive of limb salvage.

The Vascular Unit at Suez Canal University Hospital in Egypt covers a wide area with high rates of severe injuries. This is a prospective study of mangled extremities to identify risk factors associated with limb loss in these patients. Between December 2000 and August 2003, a prospective study on all patients with arterial injuries in mangled extremities was undertaken. All patients were scored using the Mangled Extremity Severity Score (MESS) and the Mangled Extremity Severity Index (MESI). During this period, arterial reconstruction was performed in 62 patients. Primary patency, secondary p

Load-Sharing Classification Score as Supplemental Grading System in the Decision-Making Process for Patients With Thoracolumbar Injury Classification and Severity 4.

BACKGROUND: Patients with Thoracolumbar Injury Classification and Severity (TLICS) score of 4 fall into a gray zone between surgical and conservative management. The integrity of posterior ligamentous complex (PLC) evaluated by magnetic resonance imaging (MRI) contributes to surgical decision-making. Load-sharing classification (LSC) may provide a modifier to further guide decision-making in these patients. OBJECTIVE: To evaluate associations between LSC score and MRI acquisition, compromise of PLC on MRI, and surgical intervention in TLICS 4 patients. METHODS: A cohort of 111 neurologically

Progressive kyphotic deformity in comminuted burst fractures treated non-operatively: the Achilles tendon of the Thoracolumbar Injury Classification and Severity Score (TLICS).

INTRODUCTION: The Thoracolumbar Injury Classification and Severity (TLICS) scale has been considered one of the best available grading systems for evaluating thoracolumbar fractures, especially due to the fact that, differently from previous classifications, it can be used as a practical algorithm to orient the clinical decision-making between conservative and surgical management. CASE REPORT: The authors describe a case of a 54-year-old patient presenting with low-back pain after having struck her back on the handrail. The neurological exam was unremarkable. The CT-scan of the lumbar spine d

ASA score is an independent predictor of 1-year outcome after moderate-to-severe traumatic brain injury.

PURPOSE: This study aimed to investigate whether incorporating pre-injury health status, measured by the American Society of Anesthesiologists (ASA) score, improves outcome prediction models for moderate-to-severe traumatic brain injury (msTBI) patients. METHODS: We conducted a retrospective single-center study of msTBI patients (2005-2021). The primary outcome was 1-year Glasgow Outcome Scale (GOS, dichotomized as GOS1-3 (unfavorable) vs. 4-5 (favorable)), and secondary outcome was 90-day mortality. Logistic regression evaluated the contribution of ASA score to the International Mission for

Early VTE prophylaxis in severe traumatic brain injury: A propensity score weighted EAST multicenter study.

BACKGROUND: Patients with traumatic brain injury (TBI) are at high risk of venous thromboembolism events (VTE). We hypothesized that early chemical VTE prophylaxis initiation (≤24 hours of a stable head CT) in severe TBI would reduce VTE without increasing risk of intracranial hemorrhage expansion (ICHE). METHODS: A retrospective review of adult patients 18 years or older with isolated severe TBI (Abbreviated Injury Scale score, ≥ 3) who were admitted to 24 Level I and Level II trauma centers from January 1, 2014 to December 31 2020 was conducted. Early Surgical Treatment of Thoracolumbar Fractures With Thoracolumbar Injury Classification and Severity Scores Less Than 4.

BACKGROUND: Thoracolumbar fractures (TLFs) are the most common spinal fractures seen in patients with trauma. The Thoracolumbar Injury Classification and Severity (TLICS) classification system is commonly used to help clinicians make more consistent and objective decisions in assessing the indications for surgical intervention in patients with thoracolumbar fractures. Patients with TLICS scores <4 are treated conservatively, but a percentage of them will have failed conservative treatment and require surgery at a later date. METHODS: All patients who received an orthopaedic consult between Ja

Prehospital Injury Severity Estimate (PHISE) matches in-hospital trauma scores when embedded in AI models.

Trauma severity scores such as the Injury Severity Score (ISS) and New Injury Severity Score (NISS) are widely used for trauma benchmarking, risk stratification and outcome prediction, but rely on diagnostic imaging unavailable in the prehospital setting. We developed and externally validated the Prehospital Injury Severity Estimate (PHISE), a simple, scene-based score designed to be applied using clinical examination and knowledge of the injury mechanism. PHISE was retrospectively derived from the National Trauma Data Bank® (NTDB®) by mapping AIS-coded injury descriptions to eight PHISE body …
