Europe has entered a new era of health technology assessment. Clinical value is now appraised jointly across member states. Economic evaluation remains national. Cost-effectiveness, weighed in cost per quality-adjusted life year, is still decided country by country. Value is therefore appraised for what is used. It is not appraised for how well the care around it is delivered. Private health insurers face a parallel gap. The premiums they set rest largely on the characteristics of the insured and on claims history. They seldom reflect the quality of the care that members will actually receive.

OBJECTIVE: Presentation of data collected on urology claims from the register of a French insurance company.
MATERIAL AND METHOD: Compensation claims involving urologists covering the period 2009-2018 were identified and analyzed.

BACKGROUND: China has continuously promoted the transition of basic medical insurance from municipal-level pooling to provincial-level pooling to strengthen fund risk resilience, mutual-aid capacity, and benefit equalization. Although existing studies suggest that higher pooling levels may influence healthcare utilization, limited micro-level evidence exists on how provincial-level pooling of the Basic Medical Insurance Scheme for Urban and Rural Residents (BMISURR) affects inpatient service utilization, hospitalization costs, and income-related equity. This study examines the impact of provin…

BACKGROUND: Peripheral artery disease (PAD) is linked with an increased risk of lower extremity amputation and multiple socioeconomic factors attenuate this risk. Prior studies have demonstrated increased rates of amputation in PAD patients with suboptimal or no insurance coverage. However, the impact of insurance loss in PAD patients with pre-existing commercial insurance coverage is unclear. In this study, we evaluated the outcomes of PAD patients who lose commercial insurance coverage.
METHODS: The Pearl Diver all-payor insurance claims database was used to identify adult patients (>18 yea…

BACKGROUND: In recent years German long-term care insurance coverage has gradually been extended to include services for people with limitations in daily living skills. Until now, however, it was unclear who exactly people with limited daily living skills are.
AIM: The study aimed to characterize people with limited daily living skills who use additional support services as defined by § 45b of the Social Services Code XI (SGB XI).
MATERIAL AND METHODS: This study was based on a survey of 1284 people with limited daily living skills who used additional support services (§ 45b SGB XI). The dat…

Patients are confronted with an overwhelming number of choices when enrolling in health insurance plans, with marketplace enrollees often having over 100 plan options and Medicare Advantage enrollees having on average 42 plan options. This can fuel confusion, administrative burden, and suboptimal plan selection, which in turn can be costly to the patient, who may face greater cost sharing and thus reduce even necessary health care utilization. This is particularly troubling because most Americans experience low levels of health insurance literacy as well as limited numeracy, making it especial…

Health insurance literacy-the ability to locate, comprehend, and use information about health insurance-is a critical yet understudied dimension of health equity for second-generation Nigerian Americans. Despite health insurance's known role in reducing disparities in preventive care, this population faces unique barriers in navigating the U.S. health system. This qualitative study explores how second-generation Nigerian Americans develop health insurance literacy within familial, environmental, technological, and cultural contexts. Twenty participants aged 18 to 24 were purposively recruited …

Social inequalities are strongly associated with disparities in oral health, particularly in the development of childhood caries. This 4-year cohort study analyzed the impact of socioeconomic status (SES)- defined on the basis of the health insurance type- on the prevalence, treatment, and recurrence of dental caries in children. A total of 198 children (21 with social insurance, 177 with private insurance) were included. Caries prevalence at baseline was significantly higher in children with social insurance (95 % with >4 caries) compared to those with private insurance (29 % with >4 caries).…

OBJECTIVE: To examine how site of care and insurance network contribute to price differences for common adult outpatient surgeries paid by commercial insurers.
STUDY DESIGN: Observational study using a 50-state sample of commercial medical claims data.
METHODS: We compared insurer-paid amounts, patient out-of-pocket payments, and balance billing amounts for 4 common adult outpatient surgeries (arthroscopy, cataract, colonoscopy, and upper gastrointestinal procedures) by site of care (ambulatory surgery center [ASC] vs hospital outpatient department [HOPD]) and insurance network status (in ne…

OBJECTIVE: To assess coverage of male infertility in state laws. Infertility is experienced by 12% of couples, unfortunately, however, infertility treatments are expensive and infrequently covered by insurance. Furthermore, many states lack explicit laws on fertility coverage.
METHODS: The National Conference of State Legislatures and individual state legislature websites were used to assess fertility coverage. These were queried for health insurance mandates or fertility coverage by evaluating healthcare-related legislation.
RESULTS: Less than half (21/50) of states require coverage. Among …

The impact of insurance status on clinical outcomes in Burkitt (BL) and plasmablastic (PBL) lymphomas remains unknown. We used the National Cancer Database to examine insurance status' effect on overall survival (OS) in adults diagnosed with these lymphomas between 2004 and 2014. BL patients with private insurance had significantly better OS compared to those without. In patients aged <65 years, hazard ratios were 1.4 for uninsured status (95% confidence interval 1.2-1.7), 1.2 for Medicaid (95% CI 1.0-1.4), and 1.5 for Medicare (95% CI 1.2-1.9). For patients aged >65 years, hazard ratio for un…

OBJECTIVE: Nonelective case status is the strongest predictor of mortality for thoracic aortic operations. We hypothesized that underinsured patients were more likely to require nonelective thoracic aortic surgery because of reduced access to preventative cardiovascular care and elective surgical services.
METHODS: Between June 2005 and August 2011, 826 patients were admitted to a single aortic referral center and underwent 1 or more thoracic aortic operations. Patients with private insurance or Medicare (insured group, n=736; 89%) were compared with those with Medicaid or no insurance (under…

AIM: The authors report experience with 14 cases where two screws or "double insurance" screws were used for transfacetal fixation of each joint for stabilization of the lumbar spinal segment. The anatomical subtleties of the technique of insertion of screws are elaborated.
MATERIALS AND METHODS: During the period March 2011 to June 2014, 14 patients having lumbar spinal segmental instability related to lumbar canal stenosis were treated by insertion of two screws into each articular assembly by transfacetal technique. After a wide surgical exposure, the articular cartilage was denuded and bo…

BACKGROUND: Given the prevalence of obesity and associated chronic conditions among U.S. adults, wellness benefits are an increasingly popular approach to promoting weight loss.
PURPOSE: The goal of the study was to assess overweight and obese adults' beliefs about the helpfulness of insurance coverage of weight loss-related benefits, their willingness to pay for such benefits, and whether these opinions differ by individuals' weight or health insurance type.
METHODS: A national survey was fielded in 2012 among non-pregnant, overweight, and obese adults who had seen a primary care provider i…

The health insurance system in Japan is compared to the U.S. system from a neurosurgeon's perspective. The Japanese entire population is enrolled in mandatory health insurance without choice based on employment and residence, called "Health-insurance-for-all." Elderly Health Insurance for senior people aged 70 years or older is set within each health organization. As the relative financial conditions are variable among health insurance organizations, financial adjustment is done. The medical fee is set for all the procedures and products that are paid by health insurance which sets the prices.…

Mold contamination is a growing concern for homeowners in terms of both physical health and insurance. Health experts, although they concede that exposure to mold can cause respiratory illnesses, are calling for further research into other mold-related health effects and for development of standards for mold sampling and data analysis. The insurance industry is grappling with how---and whether---to provide coverage for household damage caused by mold, while some state and federal legislators are working to pass laws regarding mold testing and insurance.
