Management of intracranial hypertension with and without invasive intracranial pressure monitoring.

Management of intracranial hypertension (IH) has improved in the last decades driven by advancements in monitoring technologies and a deeper understanding of its pathophysiology. Although intracranial pressure (ICP) catheters are still recommended by current guidelines for monitoring patients at risk of IH, these methods are not without limitations. Challenges include procedural complications, availability of these devices in many healthcare settings and technical issues. In this context, management in the absence of ICP monitoring is common and now it can be augmented by intensivist-led point…

Cognitive performance in idiopathic intracranial hypertension and relevance of intracranial pressure.

Cognitive impairments have been reported in idiopathic intracranial hypertension; however, evidence supporting these deficits is scarce and … -term memory over 12-month follow-up. Improved cognition over time was associated with reduction in intracranial pressure but not body weight. We then evaluated cognition before and after a lumbar puncture with acute reduction in intracranial pressure and noted significant improvement in sustained attention to response task performance. The impact of comorbidities (hea …

Noninvasive assessment of the intracranial pressure in non-traumatic intracranial hemorrhage.

The article describes the modified technique of measuring the diameters of the optic nerve sheath (ONSD) for assessment of the intracranial pressure (ICP) in patients with intracerebral or subarachnoid hemorrhage (SAH). The CT scans of 443 patients were analyzed retrospectively. The ONSDs were measured at 3mm behind the globe and at the point where the ophthalmic artery crosses the optic nerve. The ONSD/eyeball transverse diameter (ETD) ratio was calculated. The correlation analysis was performed with the Glasgow Coma Scale score, Hemispheric Stroke Scale score, Glasgow Outcome Score, and inva…

Optic nerve intraneural pressure trend under intraocular-intracranial pressure effects.

The imbalance between intraocular pressure (IOP) and intracranial pressure (ICP) is a recognised contributor to optic nerve (ON) injury. Clinically, the translaminar pressure difference is often simplistically interpreted as the direct subtraction of ON cerebrospinal fluid pressure (ON - CSFp) from IOP. While finite element analyses suggest that the transition between these two pressure compartments is neither abrupt nor uniform, instead forming a spatially graded finite transition zone, this concept remains unconfirmed from a fluid dynamics perspective. Given the inability to directly measure…

CSF pressure assessed by lumbar puncture agrees with intracranial pressure.

The accuracy of estimating intracranial pressure in brain tissue (ICP(BT)) via lumbar space was investigated using preset pressure levels in the interval 0 to 600 mm H(2)O in patients with communicating hydrocephalus. Lumbar space ICP correlated excellently to ICP(BT), demonstrated by a measured mean difference of 10 mm H(2)O (0.75 mm Hg) and a regression coefficient of 0.98. The concurrence supports the lumbar puncture as an accurate technique to determine ICP in patients with communicating CSF systems.

Intracranial gossypiboma 9 years after intracranial pressure bolt insertion: illustrative case.

BACKGROUND: Resorbable hemostatic agents left behind postoperatively occasionally result in granulomatous space-occupying lesions known as " … th mass effect and midline shift. Nine years earlier, she had had a triple bolt inserted to monitor intracranial pressure after sustaining a traumatic brain injury. Surgicel was used to control bleeding during insertion. Colocation of the lesion with the position of triple bolt 9 years earlier raised suspicion for gossypiboma. However, the minor nature of the surgery a …

Secondary intracranial hypertension with acute intracranial pressure crisis in superficial siderosis.

Superficial siderosis of the central nervous system is a very rare disease related to hemosiderin deposits in the brain, brainstem, cerebellum and spinal cord due to chronic subarachnoid hemorrhage. Chronic increased intracranial pressure develops in about one-third of affected cases. We report a patient with superficial siderosis and sudden intracranial pressure crisis. A 29-year-old man experienced a subacute episode of headache, tinnitus and blurred vision. Magnetic resonance imaging of the brain revealed hemosiderin deposits characteristic of superficial siderosis. Extensive diagnostic wor…

Lumbar Puncture Opening Pressure and Continuous Intracranial Pressure Monitoring: Concordance and Clinical Implications in Idiopathic Intracranial Hypertension.

BACKGROUND AND OBJECTIVES: Lumbar puncture (LP) remains a standard diagnostic tool for assessing intracranial pressure (ICP) in idiopathic intracranial hypertension (IIH), yet it provides only a single static measurement that may not reflect dynamic pressure fluctuations. Although continuous ICP monitoring (ICPm) offers real-time assessment of postural and circadian variability, the relationship between LP and ICPm measurements done after nondiagnostic LP remains unclear. METHODS: We retrospectively reviewed adult patients with IIH at a single center (2016-2023) who underwent ICPm within 9 mo…

Use of the intrathoracic pressure regulator to lower intracranial pressure in patients with altered intracranial elastance: a pilot study.

OBJECT: The intrathoracic pressure regulator (ITPR) is a novel noninvasive device designed to increase circulation and blood pressure. By applying negative pressure during the expiratory phase of ventilation it decreases intrathoracic pressure and enhances venous return, which increases cardiac output. It is possible that the ITPR may both decrease intracranial pressure (ICP) and increase cerebral perfusion pressure (CPP) in brain-injured patients by decreasing cerebral venous blood volume and increasing cardiac output. The authors conducted an open-label, "first-in-humans" study of the ITPR i…

Long-term observations of intracranial pressure after severe head injury. The phenomenon of secondary rise of intracranial pressure.

The long-term course of intracranial pressure (ICP) was studied in 53 patients from a group of 90 patients with severe head injury treated over a 3-year period. In 49 of these, ICP was significantly elevated during the observation period. The maximum in ICP was usually observed 24 to 96 hours posttrauma. A subgroup of patients developed a second rise of ICP. Such a course was observed in 15 (31%) of the 49 patients with intracranial hypertension. In these cases, ICP increased initially to 20 to 30 mm Hg but could be controlled. Thereafter, ICP was decreased again for at least 12 hours. The sec…

Are intracranial pressure fluctuations important in glaucoma?

Glaucoma is one of the leading causes of irreversible blindness. Primary open-angle glaucoma (POAG), the most common type, is a progressive … nvolved in retinal ganglion cell death in glaucoma. An intriguing finding of recent studies is that intracranial pressure (ICP) is lower in patients with POAG and NTG when compared with nonglaucomatous control subjects. It has been suggested that the relationship between IOP and ICP may play a fundamental role in the development of glaucoma. A decreased ICP could resul …

Diagnosis and management of increased intracranial pressure.

Increased intracranial pressure (ICP) is a pathological state common to a variety of neurological diseases, all of which are characterized by the addition of volume to the skull contents. Elevated ICP may lead to brain damage or death by two principle mechanisms: 1) global hypoxic-ischemic injury, as a consequence of reduced cerebral perfusion pressure (CPP) and cerebral blood flow; and 2) mechanical distortion and compression of brain tissue as a result of intracranial mass effect and ICP compartmentalization. All ICP therapies have as a goal, reduction of intracranial volume. In unmonitored …

[Effects of anesthetic agents on intracranial pressure].

Barbiturates, etomidate and propofol decrease cerebral blood flow (CBF), mediated by a decrease in cerebral metabolism, thus decreasing intracranial pressure (ICP). As the reduction in CBF is secondary to a decrease in cerebral metabolism, these agents will have little effect on CBF or ICP in patients without active cerebral metabolic activity. Ketamine is usually not administered for the anaesthetic management of patients at risk of intracranial hypertension because of the reported increases in cerebral metabolism, CBF and ICP. The increase in CBF, however, may be partly mediated by a sympath…

[Hypertonic solutions in treatment of intracranial pressure].

Administration of hypertonic solutions is the method of choice for acute treatment of intracranial hypertension. Recording of the intracranial pressure during treatment facilitates adjustment of the dosis to the actual ICP-response, avoiding thereby administration of an excessive osmotic load as a basis to prolong therapeutical efficacy. The mechanisms underlying reduction of the intracranial pressure by hypertonic solutions are still controversially discussed. Dehydration of normal probably also of edematous brain parenchyma and constriction of cerebral resistance vessels as an autoregulatory…
