Inverted left atrial appendage presenting as a left atrial mass after cardiac surgery.

Inversion of the left atrial appendage can masquerade as a new left atrial mass. Failure to be aware of this entity can result in unnecessary diagnostic and therapeutic procedures. If the entity is diagnosed intraoperatively, treatment is simple with external reduction and ligation.

Atrial fibrillation during a subarachnoid block in a patient with undiagnosed mitral valve prolapse.

Mitral valve prolapse (MVP) is a common cardiac abnormality, and there are numerous documented cases of intraoperative dysrhythmias. However, the authors describe what is believed to be the first case of atrial fibrillation in a patient with an undiagnosed MVP who was given a subarachnoid block. A possible clinical sequencing for the subarachnoid block is offered. The differential diagnosis, etiology, pathology, and pathophysiology of MVP are discussed. Also detailed are the management of intraoperative dysrhythmias. Physicians should be aware, as in the case reported, that a new-onset atrial

Predicting Postoperative Recovery Following Intraoperative Microvascular Complications in Autologous Breast Reconstruction.

BACKGROUND: Intraoperative microvascular complications in autologous breast reconstruction may increase the risk of immediate postoperative flap complications, such as congestion, thrombosis, and flap loss. The impact of specific intraoperative complications on patient outcomes remains unclear. METHODS: A retrospective chart review of microsurgical breast reconstruction cases from 2017 to 2023 was performed to determine intraoperative and postoperative complications, as well as patient outcomes. Statistical analysis was conducted to determine associations between intraoperative complications

Awareness of rhinoplasty-related complications among adolescents and adults: A cross-sectional study.

Rhinoplasty is one of the most frequently performed aesthetic surgical procedures worldwide. Awareness of potential postoperative complications may differ between adolescents and adults and may influence expectations regarding surgery. The objective of this study was to compare baseline awareness of well-established rhinoplasty-related complications between adolescents and adults recruited from a general otorhinolaryngology outpatient setting. This cross-sectional survey included 372 participants (196 adolescents and 176 adults). The questionnaire assessed demographic characteristics, attitude

Bridging neural circuits and clinical outcomes: a novel framework for perioperative disturbances of consciousness and cognition.

Disturbances of consciousness and cognition that happen during the perioperative period remain a critical challenge in anesthesiology, manifested as intraoperative awareness, delayed emergence, and postoperative delirium. These disorders arise from complex interactions among neural connectivity, neurotransmitter dynamics, and the pharmacological modulation of cortical and subcortical circuits. The thalamocortical network, brainstem arousal centers, and frontoparietal integration systems are crucial in facilitating transitions between drug-induced unconsciousness and recovery; however, their fu…

Intraoperative Transesophageal Echocardiography in Acute Type A Aortic Dissection: Contemporary Approach.

Acute type A aortic dissection represents a critical cardiac surgical emergency and carries a significant mortality risk. While computed tomography angiography is the standard for initial diagnosis, transesophageal echocardiography (TEE) is indispensable in the intraoperative setting. This article discusses intraoperative TEE findings in patients undergoing surgery for type A aortic dissection, emphasizing the necessity of real-time imaging to detect complications and guide surgical management. The use of TEE is important in confirming diagnoses, monitoring hemodynamics, evaluating the functio

A Case of Postoperative Pneumothorax due to a Dislodged Reinforced Staple Fragment After Thoracoscopic Segmentectomy: Recommendations for the Safe Use of Reinforced Staplers.

A 70-year-old man underwent a video-assisted thoracoscopic S1 + 2 segmentectomy for suspected left upper lobe lung cancer. Despite an uneventful intraoperative procedure with no air leakage on intraoperative testing, a postoperative pneumothorax developed after the patient returned to the ward. Conservative management, including autologous blood pleurodesis was attempted; however, persistent air leakage necessitated reoperation on postoperative day five. Intraoperatively, the dislodged staple fragment was found embedded in the remaining left upper lobe, causing an air leak. This fragment likel

Left behind: unintentionally retained surgically placed foreign bodies and how to reduce their incidence--pictorial review.

OBJECTIVE: Unintentionally retained surgically placed foreign bodies have been associated with increased morbidity and mortality, as well as increased costs and medicolegal consequences. This article reviews some of the most commonly retained surgical devices and provides a structured approach to intraoperative image acquisition and interpretation. By increasing the awareness of surgically placed foreign bodies, our goal is to reduce the incidence of and the complications associated with this difficult clinical problem. CONCLUSION: Despite various systems and safeguards available, unintention

Designing an operating theatre for awake procedures: A solution to improve multimodality information input.

OBJECTIVE: Many neurosurgical procedures are now performed with the patient aware in order to allow interactions between the patient and healthcare professionals. These procedures include awake brain surgery and spinal cord stimulation (SCS), lead placement for treatment of refractory chronic back and leg pain. Neurosurgical procedures under local anaesthesia require optimal intraoperative cooperation of the patient and all personnel involved in surgery. In addition to accommodating this extra source of intraoperative information all other necessary sources of data relevant to the procedure mu

Saphenous nerve injury. An unusual arthroscopic complication.

The surgeon should use strict technical vigilance to prevent intraoperative arthroscopic complications. An expert understanding of the anatomy, along with visualization of all instruments during surgery, should prevent inadvertent injury to neighboring structures. This case report represents one unusual complication associated with arthroscopic knee surgery and should serve to heighten the surgeon's awareness that arthroscopy is not a benign procedure.

Neurological sedative indicators during general anesthesia with remimazolam.

BACKGROUND: The bispectral index (BIS) value during general anesthesia with the newly developed anesthetic remimazolam is reported to be relatively high; however, the reason for this and the appropriate indicator for assessing the sedation level during remimazolam anesthesia have not been determined. In this study, the level of sedation during general anesthesia with remimazolam was evaluated using several different indicators. METHODS: Thirty patients who underwent breast surgery under general anesthesia with remimazolam were included. BIS®, Sedline® and the pupil resting diameters were meas

Intraoperative conditions of patients undergoing pancreatoduodenectomy.

BACKGROUND: Postoperative pancreatic fistula (POPF) is a severe complication following pancreatoduodenectomy (PD). Previous research in colorectal surgery demonstrated suboptimal intraoperative conditions to be related with an increased risk of anastomotic leakage. Aim of this study was to evaluate the intraoperative condition of patients undergoing PD by both assessing whether these known intraoperative modifiable risk factors in colorectal surgery are also present during PD and by measuring compliance to intraoperative ERAS guidelines. Secondly, to determine the relation of these factors wit

Successful anticoagulation with fresh frozen plasma for cardiopulmonary bypass in a patient with heparin resistance: a cautionary tale.

Heparin resistance (HR) is an increasingly common occurrence due to a greater awareness of the benefits of antithrombosis prophylaxis in hospitalised patients with low molecular weight and unfractionated heparin. Furthermore as more high-risk patients with prior heparin exposure undergo cardiac surgery we can expect to encounter more such cases. Adequate anticoagulation is essential for the safe conduct of any operation requiring cardiopulmonary bypass and is usually achieved with systemic heparinisation. We report a case of successful anticoagulation with the intraoperative administration of

Distal femoral complications following antegrade intramedullary nail placement.

While antegrade nailing for proximal and diaphyseal femur fractures is a commonly utilized fixation method with benefits including early mobilization and high rates of fracture union, both intraoperative and postoperative complications may occur. Intraoperative errors include leg length discrepancy, anterior cortical perforation, malreduction of the fracture, and neurovascular injury, and postoperative complications include nonunion, malunion, infection, and hardware failure. This case series reviews complications affecting the distal femur after intramedullary nailing including fracture surro

Goal-directed therapy: what is the goal again?

Recent attention has focused on intraoperative hypotension (IOH) and hemodynamic instability. This enhanced awareness to limit IOH, combined with fluid restriction and increased vasopressor use, has been associated with an increase in postoperative acute kidney injury. Recent literature supports improved intraoperative monitoring of mean arterial pressure (MAP), fluid management, and appropriate use of vasopressors and inotropic support for hemodynamic management. Implementing an algorithm to manage the causes of IOH minimizes iatrogenic harm by guiding anesthesia clinicians to select the appr

Flash pulmonary edema during cesarean section in a woman with preeclampsia.

BACKGROUND: Pulmonary edema may complicate preeclampsia. We report intraoperative flash pulmonary edema in a preeclamptic woman with Rendu-Osler-Weber syndrome. CASE: The patient was admitted at 33(+6) weeks gestation with preeclampsia. After rapid sequence induction and endotracheal intubation for cesarean section, flash pulmonary edema developed without evidence of cardiac dysfunction. She was mechanically ventilated and treated with furosemide. Following brisk diuresis she was extubated the next day and discharged on postoperative day 9 in good clinical condition. CONCLUSION: Endotracheal

The influence of non-technical performance on technical outcome in laparoscopic cholecystectomy.

INTRODUCTION: Evidence from other professions suggests that training in teamwork and general cognitive abilities, collectively described as non-technical skills, may reduce accidents and errors. The relationship between non-technical teamwork skills and technical errors was studied using a behavioural marker system validated in aviation and adapted for use in surgery. METHOD: 26 elective laparoscopic cholecystectomies were observed. Simultaneous assessments were made of surgical technical errors, by observation clinical human reliability assessment (OCHRA) task analysis, and non-technical per

Maternal and neonatal responses related to the volatile agent used to maintain anaesthesia at caesarean section.

A standardized technique of general anaesthesia, with one randomly selected variable, was provided for 237 patients undergoing elective and 540 undergoing emergency Caesarean section. The variable was the volatile agent used to maintain anaesthesia, the choice resting between trichloroethylene (0.2 or 0.3 vol.%) and halothane (0.2, 0.3, 0.4 or 0.5 vol.%). No association was found between the type of agent and the duration of anaesthesia, or the duration of either the I-D or the U-D interval. A smaller total dose of suxamethonium was administered in conjunction with the higher concentrations of

Are Cephalomedullary Nail Guides Accurate? A Case Report of an Unexpected Complication After Nailing an Intertrochanteric Femoral Fracture.

Intramedullary nailing is the mainstay of treatment for unstable intertrochanteric hip fractures. Various complications have been described with the use of these nails. We report an unusual complication whereby the lag screw completely missed the nail. We hypothesize that this previously unreported complication may be related to a specific flexible carbon fiber aiming device. Surgeon awareness and thorough intraoperative imaging are crucial to avoiding this complication.
