Intraoperative Echocardiography as a Cornerstone in Anesthetic Care for Mayo Level IV Clear Cell Renal Cell Carcinoma with Cavoatrial Invasion in a Patient with Sickle Cell Trait.

This case report describes the successful perioperative anesthetic management of a 60-year-old woman with Mayo Level IV clear cell renal cell carcinoma and sickle cell trait who underwent radical nephrectomy, inferior vena cava thrombectomy, and right atrial thrombus excision under cardiopulmonary bypass. The anesthetic challenges included a large intracardiac mass with a high probability of producing mechanical obstruction across the right ventricular inflow and embolization, risk of massive hemorrhage, and perioperative concerns related to sickle cell trait. Intraoperative transesophageal ec

Proportional Condylectomy for the Treatment of Unilateral Condylar Hyperplasia With Maloclussion: Role of Point-of-Care Manufacturing Surgical Guides and Intraoperative Computed Tomography to Avoid Secondary Orthognatic Surgery.

Condylar hyperplasia (CH) is a complex, usually unilateral pathology affecting the jaw, leading to facial asymmetry and malocclusion. Its etiology is unclear, with potential links to endocrine disorders, trauma, and genetic factors. Unilateral condylar hyperplasia (UCH) typically presents in young patients but can occur at any age. Common symptoms include progressive facial asymmetry, malocclusion, and temporomandibular joint dysfunction. Diagnostic imaging like single positron emission computed tomography helps assess condylar activity. Treatment aims to halt condylar growth and correct facia

The effect of near-infrared spectroscopy guided intraoperative care on neurocognitive and functional outcomes after cardiac surgery: a systematic review and meta-analysis of randomised controlled trials.

BACKGROUND: Adverse neurocognitive and functional outcomes are common after cardiac surgery. Cerebral near-infrared spectroscopy (NIRS) moni … one evaluated validated measures of quality of life or function. CONCLUSIONS: Although NIRS-guided intraoperative care shows promise for reducing early postoperative cognitive decline after cardiac surgery, evidence of benefit for other neurocognitive or clinical outcomes remains limited. Large multicentre studies are needed to confirm its utility, refine interventi …

Perfusion Index and Its Correlation With Intraoperative Hypotension in Lower-Segment Cesarean Section Under Spinal Anesthesia: A Prospective Observational Study in a Tertiary Care Hospital in Eastern India.

Background Hypotension is commonly encountered in patients undergoing lower-segment cesarean section (LSCS) under the subarachnoid block (SAB) owing to decreased vascular resistance caused by the sympathetic blockade and decreased cardiac output because of blood pooling in blocked areas of the body. Perfusion index (PI) is a good indicator of systemic vascular resistance and can foretell hypotension. This study aimed to associate baseline PI with intraoperative hypotension after SAB in LSCS. Methodology This was a prospective observational study with a sample size of 50. The baseline PI was re

Guidelines for Perioperative Care in Cytoreductive Surgery (CRS) with or without hyperthermic IntraPEritoneal chemotherapy (HIPEC): Enhanced recovery after surgery (ERAS®) Society Recommendations - Part I: Preoperative and intraoperative management.

BACKGROUND: Enhanced recovery after surgery (ERAS) pathways have been shown to considerably reduce complications, length of stay and costs after most of surgical procedures by standardised application of best evidence-based perioperative care. The aim was to elaborate dedicated recommendations for cytoreductive surgery (CRS) ± hyperthermic intraperitoneal chemotherapy (HIPEC) in a two-part series of guidelines based on expert consensus. The present part I of the guidelines highlights preoperative and intraoperative management. METHODS: The core group assembled a multidisciplinary panel of 24

Intraoperative Detection and Management of Euglycemic Ketonemia During Emergency Surgery Using Point-of-Care Blood Beta-Hydroxybutyrate Testing in a Patient Receiving Sodium-Glucose Cotransporter 2 (SGLT2) Inhibitors: A Case Report.

Patients who receive sodium-glucose cotransporter 2 inhibitors (SGLT2is) are at risk of developing euglycemic ketoacidosis (EDKA), particularly during the perioperative period, if drug withdrawal is insufficient. Although current guidelines recommend withholding SGLT2is prior to surgery, adequate withdrawal periods cannot always be ensured in emergency situations. However, to date, no standardized intraoperative management strategies have been established to address this risk in such situations. To address this gap, we developed a prototype intraoperative management regimen at our institution

Cytoreductive surgery (CRS) with hyperthermic intraoperative peritoneal chemotherapy (HIPEC) versus standard of care (SoC) in people with peritoneal metastases from colorectal, ovarian or gastric origin: protocol for a systematic review and individual participant data (IPD) meta-analyses of effectiveness and cost-effectiveness.

INTRODUCTION: There is uncertainty about whether cytoreductive surgery (CRS)+hyperthermic intraoperative peritoneal chemotherapy (HIPEC) improves survival and/or quality of life compared with standard of care (SoC) in people with peritoneal metastases who can withstand major surgery. PRIMARY OBJECTIVES: To compare the relative benefits and harms of CRS+HIPEC versus SoC in people with peritoneal metastases from colorectal, ovarian or gastric cancers eligible to undergo CRS+HIPEC by a systematic review and individual participant data (IPD) meta-analysis. SECONDARY OBJECTIVES: To compare the co

A Process-Based Standardization of an Intraoperative Escalation Protocol in Anesthesiology.

Prompt recognition and management of critical events is pivotal for the provision of safe anesthetic care. This requires a well-functioning team that focuses on effective communication, timely decision-making, and escalation of potential complications. We believe that variation in bedside care leads to "near-misses," adverse outcomes, and serious safety events (SSEs). The principles of an escalation culture have been used successfully in other highly reliable industries such as aviation, military, and manufacturing. We discuss here the introduction of a unique and compelling thought-process fo

[Intraoperative complications of the lower gastrointestinal tract : Prevention, recognition and therapy].

Every surgical intervention is associated with the risk of intraoperative complications. These occur in approximately 2-12% of patients but significantly influence the postoperative outcome, overall complication and mortality rates. This article presents the treatment of typical intraoperative complications during surgery of the lower gastrointestinal tract with a focus on the prevention and identification of risk factors. Especially changes in the regular anatomy caused by previous surgery, inflammation, tumors and emergency situations carry the risk of iatrogenic injuries to the bowels, sple

Evaluation and treatment of dural tears in lumbar spine surgery: a review.

Incidental durotomy is a frequent complication of lumbar spinal surgery. The number and complexity of spinal procedures is increasing, leading to a greater prevalence of dural tears; therefore, it is imperative that spine surgeons be familiar with safe and effective closure techniques. Occasionally, a tear may not be recognized during the procedure, so that one must recognize the signs and symptoms of a cerebrospinal fluid leak postoperatively. Several newer treatment concepts show promise. The current study represents an extensive review of the recent literature on the prevalence, mechanism,

Anesthetic management for small bowel transplantation.

Small bowel transplantation is quickly becoming the therapy of choice for short-gut syndrome. Improvements in surgical techniques, immunosuppressants, and anesthetic management of patients have allowed this procedure to become the standard of care for patients with irreversible intestinal failure who are unable to continue total parenteral nutrition therapy. This article discusses the evaluation process for candidate selection, including work-up for vascular access and hypercoagulability, intraoperative management of patients (including appropriate monitoring, hemodynamic changes, fluid manage

Intraoperative frozen section diagnosis of lung specimens: An updated review.

Intraoperative frozen section (FS) diagnosis is a critical step in the management of patients with pulmonary lesions, which provides guidance for surgical resection procedures. Intraoperative FS diagnosis requires a comprehensive approach that integrates clinical information, imaging findings, and histopathological evaluation. Effective communication between pathologists and surgeons is vital for achieving the best practice result. Intraoperative FS diagnosis faces new challenges in the era of new lung cancer screening strategy, changes in histological tumor classification and addition of new

[Autotransfusion technic in surgery of craniostenoses in children].

The extensive blood loss during the surgical correction of craniosynostosis in infancy needs usually homologous transfusion. The Authors report a program of preoperative and intraoperative hemodilution and their experience in six infants, the very first in pediatric neurosurgery. Prevention of homologous blood transfusion achieved in 4 out 6 cases can be considered a success that could be further improved introducing minor changes in our protocol.

Anesthetic management of patients undergoing pituitary surgery.

Pituitary tumors account for more than 10% of all intracranial tumors. They often present with symptoms of hormonal hypersecretion, although they may also cause hypopituitarism. Transsphenoidal pituitary surgery has become a commonly performed neurosurgical procedure, which has certain challenges for the anesthesiologist due to many distinct comorbidities associated with various adenomas. This article provides a review of perioperative concerns regarding transsphenoidal pituitary surgery, encountered in a number of these patients. Thorough understanding of preoperative assessment, intraoperati

Perioperative evaluation of myasthenia gravis.

Myasthenia gravis (MG) is the prototype of antibody mediated autoimmune disease and results from the production of autoantibodies against the acetylcholine receptor (AChR) of the neuromuscular synapse. Adequate preoperative evaluation of the myasthenic patient must be carried out carefully. Age, sex, onset and duration of the disease as well as the presence of thymoma may determine the response to thymectomy. Specific attention should be paid to voluntary and respiratory muscle strength. The preoperative preparation of MG patients is essential for the success of surgery. It depends on the seve

Anesthetic concerns in lung transplantation for severe pulmonary hypertension.

Lung transplantation has become a consolidated treatment for patients with severe pulmonary hypertension (PH). Several difficulties are encountered during the procedure in such candidates, who are still recognized as more severely affected by perioperative morbility and mortality than those undergoing lung transplantation for other diseases. Right ventricular (RV) enlargement with tricuspid regurgitation, small left ventricle (LV) with an asymmetric hypetrophic wall, interventricular septal shift toward the left, with ventricular stiffness and diastolic incompetence, are typical preoperative e

Anesthesia considerations for orthotopic liver transplantation.

An institutionwide commitment is necessary for the success of a liver transplantation program. Although the number of people available to manage anesthesia for such surgery may vary, a minimally staffed transplantation team must include a staff anesthesiologist, an anesthesia resident, one or two certified nurse-anesthetists, and a nonprofessional to obtain and run specimens to the laboratory and blood bank; the operating room must be connected by intercom and telephone to support services. Another technician may be present to collect specimens and data for research purposes. Additional staff
