ISOFLURANE RESPIRATORY ANESTHETIC INDEX IN CHICKENS (GALLUS GALLUS DOMESTICUS).

Respiratory depression from isoflurane seems to be greater in birds than in mammals. Isoflurane respiratory anesthetic index (AI) has only been evaluated in ducks (Anas platyrhynchos), which indeed showed a lower AI compared to mammals, but the isoflurane AI for other avian species is not known. The aim of this study was to evaluate the isoflurane AI in chickens (Gallus gallus domesticus). Six adult hens were anesthetized with isoflurane for determination of the minimum anesthetic concentration (MAC) using the bracketing method. During a second anesthetic event, the isoflurane AI was determine…

Nutritional ketosis delays the onset of isoflurane induced anesthesia.

BACKGROUND: Ketogenic diet (KD) and exogenous ketone supplements can evoke sustained ketosis, which may modulate sleep and sleep-like effects. However, no studies have been published examining the effect of ketosis on the onset of general isoflurane induced anesthesia. Therefore, we investigated the effect of the KD and different exogenous ketogenic supplements on the onset of akinesia induced by inhalation of isoflurane. METHODS: We used a high fat, medium protein and low carbohydrate diet (KD) chronically (10 weeks) in the glucose transporter 1 (GLUT1) deficiency (G1D) syndrome mice model a…

Isoflurane provides neuroprotection in neonatal hypoxic ischemic brain injury.

Isoflurane is a volatile anesthetic that is widely used clinically as an inhalational anesthetic. In recent years, several studies have indicated that isoflurane has neuroprotective properties. This has led to the beneficial effects of isoflurane being analyzed in both cell culture and animal models, including various models of brain injury. Neonatal hypoxia ischemia may be characterized as injury that occurs in the immature brain, resulting in delayed cell death via excitotoxicity and oxidative stress. These adverse events in the developing brain often lead to detrimental neurological defects…

Isoflurane anesthesia elicits protein pattern changes in rat hippocampus.

Postoperative cognitive dysfunction (POCD) is a known phenomenon occurring after anesthesia with volatile anesthetics (VA), such as isoflurane. Recent reports suggest that VA interact with neurodegenerative disease-associated proteins including compounds with pathogenic relevance in Alzheimer disease (AD) and induce processes that may be linked to AD neuropathology. Unfortunately, our present understanding of the exact anesthetics' molecular mechanisms of action, their side effects on the brain, and their catenation with AD pathology is still limited. The present study analyzes the differentia…

[Status asthmaticus treated with inhaled isoflurane and intravenous isoproterenol].

We report the case of an 18-year-old man with status asthmaticus who was treated with inhaled isoflurane and intravenous isoproterenol. The patient was intubated and mechanical ventilation was began immediately after admission to the hospital. He received intravenous methylprednisolone and aminophyline, and frequent inhalation of isoproterenol aerosol. However, his respiratory status deteriorated: peak inspiratory pressure increased markedly, and right pneumothorax, right lower lobe atelectasis, and hypotension developed. He was then given isoflurane by inhalation and a continuous intravenous …

Towards a standardized anaesthetic state using isoflurane and morphine.

In 34 patients undergoing major surgery, the inspired isoflurane concentration was adjusted by a control system designed to maintain systolic arterial pressure at a predetermined value. An empirical rule allowed additional morphine administration if the demand of the system for isoflurane was excessive. Satisfactory control of systolic arterial pressure was achieved in 31 patients and the anaesthetic state was clinically acceptable to an independent observer; no awareness was reported and the mean recovery time was 9.6 min. In these patients, control of systolic arterial pressure produced a pa…

Isoflurane: its use to induce hypotension in neurosurgical patients.

The use of isoflurane to induce arterial hypotension was studied in 40 adults undergoing neurosurgery. In 70% of patients, isoflurane, up to 2.0% (inspired), decreased mean arterial pressure (MAP) to the desired level for surgery; for the remainder, a higher concentration (3.0-4.0%) was required. Tachycardia, which could be slowed by labetalol, occurred in 13 patients (32%); the incidence was significantly higher in those who had not received an opiate either as premedication or intra-operatively. No post-operative complications attributable to hypotension were seen. It is concluded that isofl…

A fatal case of malignant hyperthermia following isoflurane anaesthesia.

To date, only two cases of malignant hyperthermia following isoflurane anaesthesia have been reported, neither of which were fatal. This case describes a very malignant form of hyperthermia after isoflurane, in which the laboratory values were grossly abnormal, and with multi-organ failure and death finally supervening.

Effect of pentobarbital and isoflurane on acute stress response in rat.

BACKGROUND: Anesthesia administration before sacrificing animals is a common practice in stress-related studies, but the effect of anesthesia on the results remains understudied. We aimed to reveal the interference of different anesthetics, i.e. intraperitoneal (i.p.) sodium-pentobarbital injection or isoflurane inhalation, with the acute stress responses in rats. METHODS: Rats were randomly divided into foot shock (FS) and non-stressed control groups, and further grouped according to the sacrificing procedure: direct decapitation, decapitation after i.p. sodium-pentobarbital injection, or is…

Effect of dexmedetomidine on the minimum alveolar concentration of isoflurane in cats.

This study reports the effects of dexmedetomidine on the minimum alveolar concentration of isoflurane (MAC(iso) ) in cats. Six healthy adult female cats were used. MAC(iso) and dexmedetomidine pharmacokinetics had previously been determined in each individual. Cats were anesthetized with isoflurane in oxygen. Dexmedetomidine was administered intravenously using target-controlled infusions to maintain plasma concentrations of 0.16, 0.31, 0.63, 1.25, 2.5, 5, 10, and 20 ng/mL. MAC(iso) was determined in triplicate at each target plasma dexmedetomidine concentration. Blood samples were collected a…

Isoflurane-induced neuroapoptosis in the developing brain of nonhypoglycemic mice.

Drugs that suppress neuronal activity, including general anesthetics used in pediatric and obstetric medicine, trigger neuroapoptosis in the developing rodent brain. Exposure of infant rats for 6 hours to a combination of anesthetic drugs (midazolam, nitrous oxide, isoflurane) reportedly causes widespread apoptotic neurodegeneration, followed by lifelong cognitive deficits. Isoflurane, the dominant ingredient in this triple cocktail, has not been evaluated individually for apoptogenic potential. It was recently reported that (1) the minimum alveolar concentration (MAC) for anesthetizing infant…

Hepatic effects of halothane, isoflurane or sevoflurane anaesthesia in dogs.

The effects of halothane, isoflurane and sevoflurane anaesthesia on hepatic function and hepatocellular damage were investigated in dogs, comparing the activity of hepatic enzymes and bilirubin concentration in serum. An experimental study was designed. Twenty-one clinically normal mongrel dogs were divided into three groups and accordingly anaesthetized with halothane (n = 7), isoflurane (n = 7) and sevoflurane (n = 7). The dogs were 1-4 years old, and weighed between 13.5 and 27 kg (18.4 +/- 3.9). Xylazine HCI (1-2 mg/kg) i.m. was used as pre-anaesthetic medication. Anaesthesia was induced w…

Mask induction of anaesthesia with isoflurane or sevoflurane in premedicated cats.

A comparison was made of the time to and quality of induction of anaesthesia when sevoflurane (n=14) or isoflurane (n=14) was delivered by mask in premedicated healthy adult cats presented for elective surgery. Times to induction and intubation were significantly shorter with sevoflurane (210 +/- 57 seconds and 236 +/- 60 seconds, respectively) than with isoflurane (264 +/- 75 seconds and 292 +/- 73 seconds). The quality of induction was similar for both agents. Two cats in each group developed opisthotonus of less than 45 seconds' duration. Both sevoflurane and isoflurane produced mask induct…

[Transcranial doppler sonography. Effect of sevoflurane in comparison to isoflurane].

METHODS: Using transcranial Doppler sonography (TCD), we studied the effects of sevoflurane compared to equipotent doses of isoflurane on blood-flow velocity in the middle cerebral artery (MCA) before, during, and after general anaesthesia. In random order, 30 patients received sevoflurane (n = 15) or isoflurane (n = 15) given in stepwise-increasing doses of 0.5, 1.0, and 1.5 MAC in oxygen/air (FiO2 = 0.5). Oxygen/air was then replaced by oxygen/nitrous oxide 33%/65% with decreasing doses (1.5, 1.0, 0.5 MAC) of sevoflurane or isoflurane. During each step, ventilation was controlled to provide …

Influence of halothane, enflurane, and isoflurane on motor evoked potentials.

The influence of the inhalational anesthetics halothane, enflurane, and isoflurane on motor evoked potentials was studied in a total of 10 rabbits. Motor evoked potentials were recorded from the extremity muscles as well as from the epidural space of the spinal cord and cauda equina in response to electrical stimulation of the motor cortex at baseline conditions and equianesthetic concentrations (0.25 to 1.5 minimal alveolar concentration). Our results show a dose-dependent suppression of the electromyographic responses, which was similar with all anesthetics. Beyond 0.5 minimal alveolar conce…

Influence of ventilatory and circulatory changes on the pharmacokinetics of halothane and isoflurane.

In two groups of dogs, uptake and elimination of halothane and isoflurane were studied using a closed-loop anesthesia system which automatically controlled end-tidal halothane or isoflurane partial pressure at minimal alveolar concentration (MAC) equivalent levels. Hemodynamic and respiratory variables were recorded and the anesthetic partial pressure was measured in the inspired and expired air, as well as in the arterial, cerebrovenous and mixed venous blood. Data were recorded during wash-in, hyperventilation, hypercirculation, hypotension and wash-out. For halothane, the controller deliver…

The minimum alveolar concentration (MAC) of isoflurane in preterm neonates.

Studies in fetal lambs suggested that the minimum alveolar concentration (MAC) in preterm neonates may be less than that in full-term neonates and older infants. To determine the MAC of isoflurane in preterm neonates, 20 patients less than 32 weeks gestation at birth and 16 patients 32-37 weeks gestation at birth, all less than 1 month post-natal age, were studied. Following tracheal intubation, the neonates were anesthetized with a predetermined end-tidal concentration of isoflurane in oxygen and air. The move-no move responses to skin incision were recorded, and MAC was determined using the …

Isoflurane and cranial extradural pressure. A study in neurosurgical patients.

The effect of isoflurane on cranial extradural pressure (EDP) was studied in 10 patients. In eight patients with normal intracranial pressure, the addition of 1.0% isoflurane to nitrous oxide plus fentanyl in oxygen anaesthesia, at physiological carbon dioxide tensions, did not cause any significant change in EDP; 1.5% resulted in a small but significant increase. In two patients with clinical evidence of increased intracranial pressure isoflurane decreased EDP during the administration of isoflurane. In all patients, hyperventilation (3.2% carbon dioxide concentration) decreased EDP rapidly. …
