Unusual Presentation of Pythium Keratitis as Peripheral Ulcerative Keratitis: Clinical Dilemma.

PURPOSE: To describe the clinical picture and outcome of a case of Pythium keratitis, presenting as peripheral ulcerative keratitis (PUK). CASE SUMMARY: A 54- year- old male presented to the clinic with a PUK in the right eye. Systemic investigations were performed to rule out associated collagen vascular disorders. Microbiological examination of corneal scrapings was performed to rule out secondary infection which revealed gram positive cocci on Gram stain. There was no growth of any organism on culture. Topical vancomycin 5% and ciprofloxacin 0.3% were started initially. On regular follow-u…

Peripheral ulcerative keratitis.

Peripheral ulcerative keratitis (PUK) is an inflammatory condition of the peripheral cornea with hallmark features of epithelial defects and stromal destruction as a result of a complex interplay of factors including host autoimmunity and the peculiar anatomic and physiologic features of the peripheral cornea and environmental factors. PUK may be the result of local or systemic causes and infectious or noninfectious causes. Arriving at a specific etiological diagnosis requires a meticulous clinical workup that may include a battery of laboratory and radiological investigations. Management by a…

[Peripheral ulcerative keratitis].

Peripheral ulcerative keratitis (PUK) is a disorder of the juxtalimbal cornea characterised by a crescent-shape destructive inflammation of corneal stroma. PUK can be associated with various ocular and systemic infectious and non infectious diseases. Different systemic autoimmune vasculitis that can prove potentially fatal may present as PUK. Potentially serious ocular complications of PUK exist. A careful clinical evaluation helps in timely diagnosis and prevention of complications.

Shewanella algae keratitis.

A 75-year-old male with a right eye history of chronic dry eye syndrome, glaucoma status post tube shunt, and Fuchs dystrophy status post Descemet stripping endothelial keratoplasty followed by penetrating keratoplasty (PKP) presented with a 2.7 × 4.2 mm corneal ulcer, culture positive for Shewanella algae and Klebsiella oxytoca. A topical antibiotic regimen of gentamicin 14 mg/mL and vancomycin 50 mg/mL was administered according to culture sensitivities. There was concurrent use of loteprednol 0.5% (Lotemax Gel, Bausch and Lomb, Rochester, NY, USA) and later addition of erythromycin 0.5% oin

Recalcitrant Paecilomyces keratitis.

A 54-year-old Caucasian woman presented with corneal ulcer of the right eye of 4 weeks duration after scratching her cornea while removing her contact lens and artificial eye lashes. Her visual acuity was 20/32 (left eye) and finger counting (right eye). She had a 3x3 mm epithelial defect with underlying corneal oedema and hypopyon. Right eye cultures grew Paecilomyces species. Topical and systemic antifungal agents were initiated. Due to the sight-threatening disease, the patient underwent surgical intervention with intrastromal injection of amphotericin B and a large conjunctival flap coveri

Neonatal infectious keratitis.

PURPOSE: The aim of this study was to determine predisposing factors, clinico-microbiological profile, and outcomes of neonatal infectious keratitis. METHODS: The retrospective study included 39 eyes of 34 neonates treated for microbial keratitis at the L. V. Prasad Eye Institute, India, between January 2006 and December 2011. Etiologic microorganisms, predisposing factors, and management outcomes were evaluated. RESULTS: The Mean age at presentation was 16.9 + 7.7 days, male:female = 16:18. The potential risk factors and coexisting conditions identified were prematurity and prolonged Neonat…

Recurrent bacterial keratitis.

PURPOSE: To investigate the rate of recurrent bacterial keratitis, associated bacteria, and surgical intervention. METHODS: Patients with suspected bacterial keratitis were identified from microbiological requests over a 16-year period between 1995 and 2010. Recurrences and number of surgical interventions were analyzed according to bacterial type. RESULTS: A total of 2418 patients were included, of whom 2124 (87.84%) had only one episode of keratitis, 294 (12.15%) at least two, 88 (3.63%) at least three, 40 (1.65%) at least four, and 22 (0.91%) five or more episodes. The bacterial isolation…

Herpes simplex keratitis.

Herpes simplex keratitis (HSK) results from an infection with the herpes simplex virus type 1 (HSV-1) also known as human herpesvirus type 1 (HHV-1). Primary infection may involve an ocular or non-ocular site, following which latency might be established principally in the trigeminal ganglion but also in the cornea. During latency, the virus appears as a circular episome associated with histones with active transcription only from the region encoding the latency-associated transcript (LAT). The LAT region is implicated in neuronal survival, anti-apoptosis, virulence, suppression of transcripti…

Chryseobacterium meningosepticum keratitis.

PURPOSE: To describe a case of a corneal ulcer caused by Chryseobacterium meningosepticum in an eye without significant comorbidities. DESIGN: Interventional case report. METHODS: Retrospective case review. RESULTS: A 48-year-old man presented with a corneal ulcer in his right eye and was treated with ofloxacin and fortified bacitracin ophthalmic solution. Cultures grew C. meningosepticum, and the infection resolved. Two previous case reports in the literature have described ocular infections with this bacterium, both in postoperative patients with significant ocular comorbidities. CONCLUS
