A complete understanding of traumatic knee dislocations begins with a review of their definition, classification, initial examination, and management options. Special emphasis is placed on associated injuries and their early detection and treatment. Decision making regarding ligament reconstruction must only be made in the context of the associated injuries or "personality" of the injury and are secondary to decisions regarding vascular compromise. These injuries can be limb threatening and deserve special respect and prompt attention when recognized.

Congenital knee dislocation is a rare condition of unknown aetiology. It could be associated with syndromes or may occur as an isolated entity. The severity of the deformity determines the method of treatment. Treatment options range from conservative casting to surgical correction. The case presented is of a newborn with an isolated grade II dislocation treated with serial casting. On follow-up at 2 years, the patient had a good outcome, with full range of motion and independent mobility.

Bilateral knee dislocation is an extremely rare and devastating injury to the knee joint. This injury is a true emergency as concomitant injuries may threaten life and limb. Here, we report the case of a 26-year-old male patient who sustained bilateral knee dislocation due to a high-velocity motor vehicle accident. In this case, the patient suffered a feared complication associated with knee dislocation, popliteal artery disruption with peroneal nerve damage. Abdominal and skeletal injuries were also discovered and treated appropriately by a multidisciplinary team. A staged surgical approach t…

The multiple ligament injured knee (knee dislocation) is, often times, part of a multisystem injury complex that can include not only injuries to knee ligaments but also to blood vessels, skin, nerves, bones (fractures), head injuries, and other organ system trauma. These additional injuries can affect surgical timing for knee ligament reconstruction and also affect the results of the treatment. This article will present the author's approach and experience in the initial assessment and treatment of the acute multiple ligament injured (dislocated) knee, and also present considerations in the t…

The knee consists of three articulations: the patellofemoral, the tibiofemoral, and the proximal tibiofibular joint. Any of these joints can … associated with peroneal nerve injury. The purpose of this article is to review the epidemiology of knee dislocations, important imaging findings, and the most common complications.

Despite the theoretical advantages of mobile bearings for lateral unicompartmental replacement (UKR), the failure rate in the initial published series of lateral Oxford UKR's was unacceptably high. The main cause of failure was bearing dislocation. To address this problem we first modified the surgical technique and then introduced a new design with a convex domed tibial plateau. This paper presents the results of these changes. In the original series (n=53), implanted using a standard open approach, there were six dislocations, all of which occurred in the first year. Five of the dislocations

We present a patient who sustained an open knee dislocation with a periprosthetic fracture of the patella and who was treated satisfactorily with tension band wiring and a conservative rehabilitation program. In addition to describing this unique combination of injuries, we review the literature on the treatment of periprosthetic patellar fractures. Although internal fixation of these fractures historically has provided unpredictable results, we think that it is warranted if the patella has adequate bone stock.

Imaging in knee dislocation requires multiple modalities and a comprehensive assessment of ligaments, cartilage, bone, nerve, and vascular anatomy. Magnetic resonance imaging is essential in the work-up of these patients and MRA is a promising modality for detecting vascular injury.

We report on a 12 year old mentally retarded boy who presented at birth with bilateral knee dislocations, dislocation of the right hip, and general joint laxity. Cytogenetic studies showed a 49,XXXXY karyotype. Hyperlaxity of joints is known to occur in 49,XXXXY patients, but congenital knee dislocation has not been reported. Rarely in 49,XXXXY and 49,XXXXX syndromes Larsen-like features may be seen. Patients with congenital joint dislocation or laxity, combined with other malformations, especially if psychomotor development is delayed, should be karyotyped to exclude chromosomal abnormalities…

Congenital dislocation of the knee (CDK) is a rare condition, often associated with syndromes and other limb anomalies. It is typically diagnosed and treated in early infancy. Neglected cases of CDK can present with significant deformities and loss of function. Their treatment is challenging and scarcely reported in the literature. The primary purpose of this study is to report on management of neglected CDK presenting in second decade of life. Four patients (eight knees) with severe, neglected, bilateral CDK who presented in their second decade of life were identified. Their preoperative radi

PURPOSE: Although treatment strategies of knee joint dislocations have evolved, there is still no consensus on the best method and timing. N … .
METHODS: In this retrospective single center study, 38 patients with acute Schenck type II to IV knee dislocations were treated over an eight-year period in a level I trauma center. At follow-up, various clinical scores, such as the International Knee Documentation Committee (IKDC) Score, Lysholm Score, and Tegner Activity Scale (TAS), and individual questions …

Although knee dislocations are relatively rare, serious complications make treatment difficult. Common peroneal nerve (CPN) palsy is a debilitating complication and its incidence has been reported as high as 50%. Even after successful ligament construction, unresolved CPN palsy is a major factor contributing to poor outcomes after knee dislocations. CPN palsy is more common with open dislocations, rotatory dislocations, and especially occurs in patients with posterolateral corner injuries. CPN palsy can be readily diagnosed clinically, although a high index of suspicion is needed. The risk ver…

Because of the nature and extreme severity of the injury, complications will continue to be a common denominator in traumatic knee dislocations. The major complications that cause significant morbidity, including amputation and death, are now relatively infrequent. Prompt recognition and treatment of these complications are of utmost importance to assure functional viability of the extremity. These complications must continue to be addressed preoperatively and postoperatively. Minor complications will persist after significant knee surgery. New complications will evolve as newer techniques, in…

Knee dislocation remains a devastating injury with many complications. It necessitates prompt diagnosis, reduction if needed, and emergent repair of any vascular injury. Serial physical examinations and frequent use of arteriograms are necessary to avoid late vascular complications. Many authors are concerned that normal pulses, normal Doppler signals, and normal ABIs have preceded late ischemia and documented intimal tear, demonstrated by arteriography. More recently, other authors have challenged the gold standard of mandatory arteriography by describing studies in which physical examination…

BACKGROUND: Total knee arthroplasty (TKA) is a successful therapy for functional improvement and pain relief in advanced symptomatic degeneration of the knee joint. But it can be associated with many complications, one of which is instability.
CASE PRESENTATION: A 70-year-old woman was referred to our hospital because of right knee dislocation after TKA was performed on her right knee due to severe varus deformity and flexion contracture. This instability was caused by persistent MCL tightness and iatrogenic lateral collateral, arcuate ligament, and popliteus tendon injury. The torn lateral co…
