Type II floating knee: ipsilateral femoral and tibial fractures with intraarticular extension into the knee joint.

Thirty-four patients with ipsilateral fractures of the femur and tibia with intraarticular extension into the knee of at least one fracture were reviewed at an average follow-up of 38 months. Joint involvement was present in 22 (65%) femoral fractures and 23 (68%) tibial fractures. In 11 (32%) patients, both fractures were intraarticular. In 71% of the patients, there were major associated injuries. Open fractures were common, occurring in 21 (62%) extremities. Associated vascular injuries were seen in seven (21%) cases. Ninety percent of the fractures were surgically stabilized. The average t

Ipsilateral Knee Dislocation and Ankle Fracture Dislocation.

INTRODUCTION: A dislocated knee is a potential limb-threatening injury. Simultaneous dislocation of knee and ankle joint in an ipsilateral limb is a very rare pattern of injury. A few cases of ipsilateral hip and knee dislocation were described in literature. Rare injury such as posterior dislocation knee with ipsilateral ankle fracture dislocation cases was also presented. However, lateral dislocation of knee along with the same side ankle fracture dislocation is a rare scenario. To the best of our knowledge, these two simultaneous lesions do not seem to have been reported previously. Immedia

Fixation of Acute Chondral Fractures in Adolescent Knees.

OBJECTIVES: Chondral fractures are focal cartilage lesions without osseous attachment, most commonly seen in adolescent knees. They have limited capacity for intrinsic healing and traditional treatment has been removal of loose fragments. However, case reports of successful healing after fixation indicate that repair of the joint surface is possible. We wanted to evaluate the outcome in a cohort of patients who underwent fixation of acute chondral fractures in the knee. DESIGN: Patients treated with fixation of a chondral fracture in the knee at our institution were invited to participate in

Arthrofibrosis of the knee following a fracture of the tibial plateau.

The aim of this study was to report the incidence of arthrofibrosis of the knee and identify risk factors for its development following a fracture of the tibial plateau. We carried out a retrospective review of 186 patients (114 male, 72 female) with a fracture of the tibial plateau who underwent open reduction and internal fixation. Their mean age was 46.4 years (19 to 83) and the mean follow-up was16.0 months (6 to 80). A total of 27 patients (14.5%) developed arthrofibrosis requiring a further intervention. Using multivariate regression analysis, the use of a provisional external fixator (o

The floating knee: Ipsilateral fractures of the tibia and femur.

Fifty-one extremities with ipsilateral tibial and femoral fractures are reviewed. Some children are included in the study. Most of the 47 patients are young men who have multiple injuries with high velocity trauma. The involved extremities are classified according to the injury site: Type 1 fractures involve both shafts; type 2-A fractures involve the knee joint; type 2-B fractures involve hip or ankle joints. Complications are frequent. Delayed union or nonunion occurred in 44.6% of the total number of bones involved. Sixty to seventy percent of the adult patients showed significant permanent

International, consensus-based, indications and treatment options for knee arthroplasty in acute fractures around the knee.

BACKGROUND: In the landscape of knee-related health issues there has been a notable shift in treatment protocols. Nowadays, there is a growing trend toward primary total knee arthroplasty (TKA) in the event of periarticular knee fractures. A review of the literature on TKA in acute knee fractures has been done in order to provide scientific evidence to the four statements submitted for voting to the members of the European Knee Society (EKS). MATERIALS AND METHODS: A literature review has been performed around four topics of TKA in acute knee fractures, specifically: 1) The indications for TK

Surgical treatment of insufficiency fractures after primary total knee arthroplasty.

Insufficiency periprosthetic fractures after total knee arthroplasty are uncommon, typically nontraumatic events; however, clinical outcome data on their surgical management remain limited. We retrospectively reviewed 21 patients (18 women, 3 men; mean age = 76 years) who sustained femoral or tibial insufficiency periprosthetic fractures between November 2016 and January 2022 and underwent revision total knee arthroplasty with stemmed components augmented by bulk femoral-head allografts and autologous bone graft. Five patients with preoperative valgus alignment presented medial condylar fractu

Knee dislocations and associated fractures: risk factors for surgical reduction.

INTRODUCTION: Dissociation of the knee joint, or knee dislocations (KD), can lead to severe complications, often resulting in multiligament injuries. A subset of these injuries are irreducible by closed reduction and require open reduction. Identifying KDs that necessitate surgical intervention is crucial for optimal outcomes. While previous studies have explored various risk factors, the influence of associated fractures is less understood. MATERIALS AND METHODS: We queried the Trauma Quality Improvement Program (TQIP) database from 2017 to 2021, for non-congenital closed knee dislocations r

CT of Periarticular Adult Knee Fractures: Classification and Management Implications.

Periarticular knee fractures, which include fractures of the distal femur, tibial plateau, and patella, account for 5%-10% of musculoskeletal injuries encountered in trauma centers and emergency rooms. These injuries are frequently complex, with articular surface involvement. Surgical principles center on reconstruction of the articular surface as well as restoration of limb length, alignment, and rotation to reestablish functional knee biomechanics. Fixation principles are guided by fracture morphology, and thus, CT with multiplanar reformats and volume rendering is routinely used to help pla

Tibial periprosthetic fractures in Total Knee Arthroplasty - A scoping review.

BACKGROUND: Periprosthetic tibial fractures in Total Knee Arthroplasty are much less commonly seen than femoral fractures, and there is a paucity of available literature and management recommendation for these fractures. We aimed to identify the relevant and up-to-date literature on this subject to analyse the incidence, risk factors, and management recommendations. METHODS: A literature search was done on the databases of PubMed and SCOPUS using appropriate keywords. All the published literature in the English language was included for this review. RESULTS: We included 21 studies comprising

The proprioception of the knee joint following tibia plateau fractures.

Proprioception is a conscious and/or unconscious perception of position change in an extremity or joint in space. In our study our purpose was to evaluate whether the lower extremity proprioception in long term, is altered following tibia plateau fractures and to assess its relation with age and type of fracture. This retrospective study includes the evaluation of proprioception in 38 tibia plateau fracture patients (29 male, 9 female) of various types who were operated with open reduction and internal fixation (ORIF) technique in our clinic, by comparison of both operated knee and unaffected

Revision Total Knee Arthroplasty for the Management of Periprosthetic Fractures.

Periprosthetic fractures after total knee arthroplasty (TKA) can present reconstructive challenges. Not only is the procedure technically complex, but patients with these fractures may have multiple comorbidities, making them prone to postoperative complications. Early mobilization is particularly beneficial in patients with multiple comorbidities. Certain patient factors and fracture types may make revision TKA the ideal management option. Periprosthetic fractures around the knee implant occur most frequently in the distal femur, followed by the tibia and the patella. Risk factors typically a

The treatment of periprosthetic femur fractures after total knee arthroplasty.

Periprosthetic femur fractures after total knee arthroplasty are a rising concern; however, when properly diagnosed, they can be managed nonoperatively or operatively in the form of locking plate fixation, intramedullary nailing, and arthroplasty. The degree of osteoporosis, stability of the femoral implant, and goals of the patient are a few critical variables in determining the ideal treatment. Despite excellent outcomes from each of these operative choices, the risk of nonunion, malunion, instability, and refracture cannot be ignored.

Periarticular fractures around the hip and knee: fix or replace?

The number of periarticular fragility fractures around the hip and knee is increasing. If surgical treatment is indicated, open reduction and internal fixation or replacement can often be viable options. In contrast to secondary replacement, the use of replacement in the acute stage might be advantageous because early rehabilitation and weightbearing can be initiated. This article describes the current literature related to internal fixation or primary replacement in periarticular fractures around the hip and knee.
