Knee dislocations (KD) and multiligamentous knee injuries (MLKI) are challenging injuries to manage due to the high incidence of associated neurovascular compromise, extensive peri-articular soft-tissue trauma, and long-term functional deficits. Knee-spanning external fixation (KSEF) is occasionally utilized in the acute management of these injuries with the aim of maintaining alignment, protecting vascular repairs, allowing soft-tissue recovery prior to definitive treatment, and protecting freshly reconstructed or repaired ligaments. Despite its use in 5% of MLKIs and up to 50% of KDs, the cl…

Cycling is currently one of the most popular recreational activities. Hence, there are increases in the numbers of injuries resulting from cycling. Most of the injuries are to the knee and are of non-contact nature and therefore, preventable. The majority of pain syndromes are non-traumatic and caused by overuse, inadequate preparation, inappropriate equipment, poor technique and lack of training. The treatment of non-traumatic knee injuries is regularly conservative, including: rest, ice, NSAID's training programs modification and adequate equipment. Delayed treatment can cause chronic injuri…

The diagnosis approach after knee injury in children must be carefully taken care of the growth plates but it is necessary to emphasize the frequency of the ligament and intra-articular lesions, and on the other hand to consider their benign reputation. Clinical and conventional radiologic examination give in most of the cases the correct diagnosis. For the others an examination under general anesthesia ++ is necessary to distinguish the ligament and the growth plate lesions. MRI must not replace the arthroscopy which is necessary for an haemarthrosis with a stable knee or a little isolated an

An in-depth study of 74 motor vehicle accidents involving 222 people revealed that 57 of the 153 people injured sustained 69 knee injuries from contact with the instrument panel. There were 51 mild, 10 moderate, and 8 severe knee injuries. Low force contact caused bruising, and more serious injuries occurred when a greater magnitude of force was applied. Injury was most severe when this force was concentrated in a small area and caused lacerations, fractured patella, and open wounds of the joint. Legs trapped under the dashboard with simultaneous rotational, varus, valgus, or frontal forces by…

The incidence of rupture of the knee ligaments was retrospectively studied in 47 patients with ipsilateral fractures of the femoral and tibial diaphyseal shaft. Fifteen patients proved to have an instability of the knee at the time of follow up. Disruption of the knee ligaments had not been recognised initially. At the time of initial treatment, injury of the knee ligaments had been diagnosed in only three cases. After stabilisation of both fractures in these cases, the knee ligaments had been repaired; at re-examination these patients had no complaints and their knees were perfectly stable. I

Recurrent or chronic activity related knee pain is common in young athletes. Numerous intrinsic conditions affecting the knee can cause such pain. In addition, knee pain can be referred pain from low back, hip or pelvic pathology. The most common cause of knee pain in young athletes is patellofemoral pain syndrome, or more appropriately termed idiopathic anterior knee pain. Although, numerous anatomical and biomechanical factors have been postulated to contribute the knee pain in young athletes, the most common underlying reason is overuse injury. In this paper, we have reviewed selected condi

OBJECTIVE: We aimed to evaluate whether a recent knee injury was associated with accelerated knee osteoarthritis (OA) progression.
METHODS: … associated with accelerated (OR 8.46) and common knee OA progression (OR 3.12).
CONCLUSION: Recent knee injuries are associated with accelerated knee OA. Most concerning is that certain injuries may be associated with a rapid cascade toward joint failure in less than 1 year.

Intraligamentous injuries to the anterior cruciate ligament (ACL) and meniscus injuries are rare in children and adolescents and often occur as a result of sports injuries. Clinically, they usually present as a hemarthrosis. The diagnosis of choice is the MRI examination. Surgical treatment of intraligamentary ACL injuries using the transphyseal technique is now also the gold standard treatment for children and adolescents, leg axis and length checks are necessary after surgery until growth is complete. Meniscus injuries are also addressed surgically. Postoperative follow-up treatment and reha

Knee ligament and meniscus injuries in children and teenagers are becoming more numerous because of increased sports participation but also better diagnosis. Meniscus injuries occur either in a normal meniscus or due to a congenital anomaly. The diagnosis is made clinically and confirmed by MRI. Treatment depends on the findings: meniscoplasty for discoid meniscus and primary repair of meniscus tears. Meniscus preservation is the rule. Injuries to the central pivot of the knee typically involve either the anterior cruciate ligament (ACL) or the tibial spine. Age, anatomy and the mechanism of i

In the last few decades, sports injuries in pediatric and adolescent athletes have increased dramatically, with ∼ 38 million young athletes participating in organized sports annually in the United States. Starting at the age of 2 years, an increase of the incidence of knee injuries until adulthood with a peak at 6 and at 13 years of age has been observed. Due to a physiologic laxity of the ligaments, ligament injuries are uncommon in these patients, but they are getting more frequent. In the growing knee, open physis are the points of minor resistance. Therefore apophyseal injuries of the pedi…

BACKGROUND: Hemarthrosis after acute knee trauma is a sign of a potentially serious knee injury. Few studies have described the epidemiology and detailed injury spectrum of acute knee injuries in a general pediatric population.
PURPOSE: To document the current injury spectrum of acute knee injuries with hemarthrosis in children aged 9 to 14 years and to describe the distribution of sex, age at injury, type of activity, and activity frequency in this population.
STUDY DESIGN: Descriptive epidemiology study.
METHODS: All patients in the Stockholm County area aged 9 to 14 years who suffered ac…

An athlete who has an acute knee injury should be assessed rapidly on the field and then more thoroughly on the sideline or in the training room. On-the-field assessment includes questions about the mechanism of injury and any similar previous injuries, a visual check for knee deformities and skin injuries, a neurovascular exam, and, ideally, tests for flexion and hyperextension. On the sideline or in the training room, standard physical tests are likely to reveal any significant injuries. These include the patellar apprehension, Lachman, posterior sag, quadriceps active, posterior drawer, pos

Arthroscopic debridement of penetrating knee joint injuries has become a common treatment method. A comparative study was undertaken to compare this method with open joint debridement. Fourteen penetrating knee joint injuries (fourteen patients) were treated by arthroscopic examination and debridement and were compared to sixteen penetrating knee joint injuries (fifteen patients) treated by open debridement. There were no resultant infections or operative complications in either group. Of note, the arthroscopic debridement group had a shorter postoperative hospital stay [mean of 1.6 days compa

To determine the frequency and type of arterial injuries associated with complete dislocation of the knee and to correlate the findings at physical examination with those at arteriography, a computerized search for patients with a diagnosis of dislocation of the knee at discharge from an urban trauma hospital was performed. Nineteen patients with complete dislocation of the knee were found. Arterial injuries were seen in six patients (32%). Four of the 19 patients had no pedal pulse at physical examination. In three of these four patients, occlusion of the popliteal artery was seen on arteriog

During a 1-year period, data on all acute injuries treated at emergency departments were entered into a registry; of these injuries, 6% involved the knee joint. The patients were followed until a definite diagnosis was established. The rate of anterior cruciate ligament injuries and meniscus tears was, respectively, 0.3 and 0.7 (injuries per 1,000 inhabitants per year). The rate of dislocations of the patellofemoral joints was equal to the rate of anterior cruciate ligament injuries. Ten percent of the cases involved ligament injuries, and isolated ruptures of the anterior cruciate ligaments w

Eighty-two patients took part in a prospective trial to assess the need and timing for arthroscopy in acute knee injuries. Patients with a suspected ligament injury, a suspected meniscal tear, or a haemarthrosis in the absence of fracture were included in the trial. Patients were entered randomly into two groups. In group I early arthroscopy was performed (within 48 h), in group II arthroscopy was performed within 21 days, but only when it was felt to be clinically indicated. Patients were assessed using the OAK knee score 1 year after the injury. There was no difference in the final result be…
