Traumatic lacerations: what are the risks for infection and has the 'golden period' of laceration care disappeared?

OBJECTIVE: To determine risk factors associated with infection and traumatic lacerations and to see if a relationship exists between infection and time to wound closure after injury. METHODS: Consecutive patients presenting with traumatic lacerations at three diverse emergency departments were prospectively enrolled and 27 variables were collected at the time of treatment. Patients were followed for 30 days to determine the development of a wound infection and desire for scar revision. RESULTS: 2663 patients completed follow-up and 69 (2.6%, 95% CI 2.0% to 3.3%) developed infection. Infected…

Effectiveness of tranexamic acid in intervening postpartum vaginal lacerations.

AIMS: This prospective randomized controlled study aimed to investigate the effectiveness of tranexamic acid in controlling bleeding due to postpartum vaginal lacerations and to compare the intravenous (IV) and topical routes of administration. The rationale for exploring topical administration was to identify a more practical and less invasive alternative to IV administration. METHODS: Between January 2024 and November 2024, 161 women aged 20-35 years who had spontaneous vaginal deliveries without episiotomy at Mardin Education and Research Hospital, experienced superficial vaginall aceratio…

Use of topical local anaesthetics for paediatric facial lacerations.

BACKGROUND: Closure of facial lacerations in young children can be difficult under local anaesthetic in an emergency department setting. Sedation or general anaesthetic are often required to close lacerations safely and to a good surgical standard. Our hospital introduced the use of local anaesthetic gel (LAT) following an audit, to reduce the need for paediatric admissions. In light of the current COVID-19 pandemic, this can be a valuable tool to help avoid admissions. OBJECTIVES: To ascertain the usefulness of LAT gel in reducing paediatric admissions. METHODS: Data was collected prospecti…

Pediatric canalicular lacerations: epidemiology and variables affecting repair success.

PURPOSE: To evaluate the epidemiology and outcomes of all pediatric canalicular lacerations at a high-volume urban tertiary eye care center. Variables of repair were evaluated in relation to age and success of repair. METHODS: A retrospective review over a 10-year span of all primary pediatric canalicular laceration repairs was performed. Variables included patient demographics, repair setting, mechanism of injury, associated injuries, type of stent used, and success of repair. RESULTS: Of 137 canalicular lacerations evaluated, 27.7% occurred in patients younger than 18 years. The majority o…

Risk of intrapartum cervical lacerations in women with cerclage.

OBJECTIVE: The aim of our study was to estimate whether the placement of cerclage in pregnancy to prevent preterm birth (PTB) is associated with higher incidence of intrapartum cervical lacerations. METHODS: A retrospective cohort study was conducted on singleton pregnancies with risk factors for PTB. The study group consisted of women with either a history- or ultrasound-indicated cerclage placed between 12 and 24 weeks of gestation, while the control group consisted of women with similar risk factors for PTB but who did not receive a cerclage. Primary outcome was the incidence of intrapartu…

[Surgical treatment of Morel-Lavallée lesion with perineal lacerations].

OBJECTIVE: To investigate the surgical treatments of Morel-Lavallée lesion with perineal lacerations. METHODS: From March 2003 to June 2009, 18 cases of Morel-Lavallée lesion with perineal lacerations were treated. There were 16 males and 2 females with a median age of 28.5 years (range, 3-54 years). The time between the injury and hospitalization ranged from 3 hours to 7 days. The lesions were in the greater trochanter (2 cases), the gluteal (2 cases), the lumbar flank (1 case), the lumbar sacrum (3 cases), the pelvic girdle (7 cases), and the hip (3 cases). All the cases were complicated wi…

The diagnostic accuracy of clinical examination in hand lacerations.

INTRODUCTION: Hand injuries account for a significant proportion of emergency department attendance. We investigated the diagnostic accuracy of clinical examination in patients with simple hand lacerations undergoing surgical exploration at our unit. METHODS: One hundred and sixty-five consecutive patients were identified as undergoing exploration of the hand. Case notes of these patients were reviewed. The clinical findings, made by emergency department doctors (ED) and hand surgeons (HS), were compared with the operative findings. RESULTS: A total of 101 patients were included following ex…

Decreased anal sphincter lacerations associated with restrictive episiotomy use.

OBJECTIVE: To determine whether restrictive episiotomy use was associated with decreases in anal sphincter lacerations and the risk of anal sphincter laceration attributable to episiotomy. STUDY DESIGN: This was a retrospective database study. Rates of episiotomy, anal sphincter laceration (third- or fourth-degree tear), and other confounding variables were compared among vaginal deliveries before (1999) and after (2002) restrictive episiotomy use was implemented at our institution. Logistic regression was used to estimate the odds ratio of anal sphincter laceration that was due to episiotomy…

Tissue adhesives for traumatic lacerations in children and adults.

BACKGROUND: Tissue adhesives have been used for many years to close simple lacerations as an alternative to standard wound closure (sutures, staples, adhesive strips). They offer many potential advantages over standard wound closure, including ease of use, decrease in pain and time to apply, as well as not requiring a follow-up visit for removal. Many studies have compared tissue adhesives and standard wound closure to determine the cosmetic outcome as well as these other secondary outcomes in their respective study populations. However, due to the wide variation in study parameters, there are…

Extensor tendon lacerations in horses: 50 cases (1982-1988).

Medical records of 50 horses with extensor tendon lacerations affecting 53 limbs over 7 years (1982 to 1988) were reviewed to determine the clinical features and prognosis for athletic soundness after treatment. Mean and median ages were 4.8 and 3 years, respectively. Eighty-nine percent of the injuries involved the hind limbs, with approximately equal distribution between the right and left limbs. Hind limb lacerations involved the long digital extensor tendon (28 limbs), lateral digital extensor tendon (3 limbs), or both tendons (16 limbs). Forelimb lacerations involved the common digital ex…

The management and outcome of lacerations in urban children.

We prospectively studied the management and outcome of 2,834 children, aged 1 month to 18 years, who presented to the emergency department of the Children's Hospital of Philadelphia for laceration repair. Patients with bite wounds were excluded from the study. Eight percent (239) of all patients had complications on initial evaluation; the most common was the presence of a foreign body (55). Infection on presentation was diagnosed in 22 cases (0.8%). All of these patients had delayed their initial care beyond 18 hours (range, 18 to 288 hours; mean, 18 hours). Other factors significantly associ

Conservative management of aortic lacerations due to blunt trauma.

Three patients with angiographically documented thoracic aortic lacerations were managed conservatively over 8 years due to the nonthreatening appearance of the injuries in two and the presence of an associated major closed head injury in a third. The lesion(s) resolved in one, diminished in another, and remained unchanged in the third. At least ten other cases managed similarly are recorded in the literature. In certain selected circumstances this approach may represent a viable alternative to the current standard of immediate surgical correction of aortic injuries.
