Efficacy of lacosamide on seizures and myoclonus in a patient with epilepsia partialis continua.

A 39-year-old male with epilepsia partialis continua, refractory to first- and second-line antiepileptic drugs, is described. Lacosamide produced a progressive antiepileptic effect on Jacksonian motor seizures and subsequently on positive myoclonus, which developed into negative myoclonus before complete resolution. Our report confirms the efficacy of lacosamide on focal motor refractory status epilepticus and documents a unique effect of lacosamide on seizure semiology.

Pharmacotherapy of the third-generation AEDs: lacosamide, retigabine and eslicarbazepine acetate.

INTRODUCTION: The search for new, more effective antiepileptic drugs (AEDs) continues. The three most recently approved drugs, the so-called third-generation AEDs, include lacosamide, retigabine and eslicarbazepine acetate and are licensed as adjunctive treatment of partial epilepsy in adults. AREAS COVERED: For the above three AEDs, their mechanisms of action, pharmacokinetic characteristics, drug-drug interactions, pharmacotherapeutics, dose and administration and therapeutic drug monitoring are reviewed in this paper. EXPERT OPINION: Lacosamide and retigabine act through novel mechanisms,

Lacosamide for refractory trigeminal neuralgia and other facial pain-Case report.

OBJECTIVE: To demonstrate effect of lacosamide monotherapy in three patients with refractory facial pain of various etiologies. BACKGROUND: Many medications used to treat trigeminal neuralgia and other facial pain, including first- and second-generation anticonvulsants, are often ineffective or have intolerable side-effects. Lacosamide, a third-generation anticonvulsant, has fewer side-effects and is a potential treatment of facial pain. METHODS: Retrospective review of three patients treated with lacosamide for facial pain. RESULTS: The etiologies of the facial pain were idiopathic trigemi

Lacosamide-associated second-degree atrioventricular block in a healthy, young athlete.

Lacosamide enhances slow inactivation of voltage-gated sodium channels and can lead to dose-dependent PR interval prolongation. Previously, lacosamide has been associated with second-degree atrioventricular (AV) heart block in the context of multiple medical comorbidities and/or in the elderly with multimorbidity on other dromotropic agents. We report a case of second-degree AV block occurring in a healthy, athletic young adult. The patient had baseline bradycardia with no known cardiac comorbidities. He was exquisitely sensitive to lacosamide with EKG and telemetry changes developing on the o

Lacosamide concentration-dose (C/D) ratios during pregnancy: a case series.

PURPOSE: Lacosamide is well known for its additional anti-seizure activity, especially during pregnancy when monotherapy of lamotrigine or levetiracetam fails in people with focal seizures. Pharmacokinetics change during pregnancy and limited results of changes in lacosamide plasma concentrations are available. METHODS: In this case series, 6 women were retrospectively studied in change of concentration dose ratios (C/D ratios) during pregnancy. RESULTS: We found that C/D ratios changed during the trimesters of pregnancy between 48% and 146% compared to baseline values. We couldn’t find a rela

Saliva-based lacosamide monitoring paves the way toward personalized epilepsy pharmacotherapy.

Saliva, known for better patient compliance and simpler collection, is ideal for monitoring antiseizure medication (ASM) levels. This study aimed to validate saliva for measuring lacosamide, develop a pharmacokinetic (PK) model, and determine the optimal saliva concentration for seizure control in epilepsy patients. In our prospective study at Seoul National University Hospital from August 2021 to November 2022, we enrolled lacosamide-prescribed epilepsy patients, collecting their saliva and blood samples. We developed a population PK model with nonlinear mixed-effects modeling, incorporating

Low-dose lacosamide-induced atrial fibrillation: Case analysis with literature review.

Lacosamide (LCM) is a novel antiepileptic drug (AED) approved by the FDA for adjunctive treatment of partial epilepsy with and without secondary generalization. Lacosamide dose-dependent dysrhythmias (PR-interval prolongation, AV block, and atrial fibrillation/flutter) have been reported. This case represents the first instance of LCM-induced atrial fibrillation following a low loading dose (200 mg). Risk factors for atrial fibrillation are addressed and discussed in the context of this case. Full cardiac history is recommended prior to patients being initiated on LCM. Cardiac monitoring may b

Successful treatment for refractory convulsive status epilepticus by non-parenteral lacosamide.

Lacosamide (Vimpat) is a newly licensed novel antiepileptic drug. We report a case of refractory convulsive status epilepticus (CSE) that was successfully controlled with lacosamide. The 38-year-old male patient was admitted for a series of complex partial seizures with secondary generalization leading to refractory CSE. During the transport to the hospital the patient was given 22.5 mg diazepam, 12.5 mg etomidate, and 5 mg midazolam without success. An additional dose of 4 mg lorazepam and a dose of 1,500 mg levetiracetam after admission were yet without clinical effect. A further treatment w

Lacosamide, the new anticonvulsant, effectively reduces harmaline-induced tremors in rats.

The present study aimed at evaluating the efficacy of lacosamide (0.3 to 30 mg/kg), a new anticonvulsant drug, in a model of essential tremor in comparison to the reference compounds propranolol and primidone. We observed a high tremorlytic effect of lacosamide reducing the intensity of tremors following harmaline administration in a dose-dependent manner. The highest dose also modified the latency and intensity of tremor at onset. The effect of lacosamide was equal or even superior to propranolol and primidone indicating that lacosamide may be a new antitremorgenic drug that merits further cl
