Lactase persistence is a heritable, autosomal dominant, condition that results in a sustained ability to digest the milk sugar lactose throughout adulthood. The majority of the world's human population experiences a decline in production of the digestive enzyme lactase-phlorizin hydrolase during maturation. However, individuals with lactase persistence continue to express high levels of the lactase gene into adulthood. Lactase persistence has been strongly correlated with single nucleotide genetic variants, C/T_(13910) and G/A_(22018), located 13.9 and 22 kb upstream from the lactase structura…

Adult-type hypolactasia is a common phenotype caused by the lactase enzyme deficiency. The -13910 C>T polymorphism, located 14 Kb upstream of the lactase gene (LCT) in the MCM6 gene was associated with lactase persistence (LP) in Europeans. This polymorphism is rare in Africa but several other variants associated with lactase persistence were observed in Africans. The aims of this study were to identify polymorphisms in the MCM6 region associated with the lactase persistence phenotype and to determine the distribution of LCT gene haplotypes in 981 individuals from North, Northeast and South Br…

We compared two lactase preparations derived from Aspergillus orizae (AOL) and Penicillinase multicolor (PML) for stability in the stomach and overall enzymatic activity in 10 asymptomatic subjects with lactase deficiency. The subjects were given 10,000 FCC units of either AOL or PML 30 min prior to or simultaneously with 300 ml of milk. Gastric juice was withdrawn through a nasogastric tube immediately after and every 15 min for 60 min, and breath was sampled before and every 15 min for 6 h after the milk ingestion. When lactase was given simultaneously with the milk, gastric juice lactase ac…

Lactase is the enzyme responsible for the digestion of the disaccharide lactose, and deficiency in this enzyme causes the prevalent medical condition lactose intolerance. Management of lactose intolerance can be achieved through the administration of lactase supplements. Lactase is an appropriate platform for advanced enzymatic study because its medical application is a motivator for student learning. The following is an upper-level biochemistry laboratory sequence that integrates student inquiry and exposure to advanced laboratory techniques. Students investigate three different lactase suppl…

A total of 122 apparently healthy Bolivian Aymara-mixed children ranging in age from 2 to 12 years were tested for lactose absorption using the standard lactose tolerance test with blood glucose determination. Analysing the sample according to the age of the probands, we found an increased age related frequency of lactose malabsorption showing final percentages of 55% in the 2 to 5 years group, 57.4% in the 6 to 10 years group, and 77.4% in the 11 to 15 years group.

The prevalence of lactase deficiency in British adults is unknown. Examination of the distribution of lactase activities in single and multiple biopsies of jejunum indicated that 0.8 U/g wet weight was a suitable cut-off point to separate lactase deficient patients from others. One hundred and fifty white British adults were selected, none of whom had significant intestinal disease and all of whom had normal histopathology of a jejunal biopsy and normal activities of other disaccharidases measured in the biopsy. The prevalence of lactase deficiency in this group was 4.7%. In contrast, 15 of 20…

The enzyme lactase that is located in the villus enterocytes of the small intestine is responsible for digestion of lactose in milk. Lactase activity is high and vital during infancy, but in most mammals, including most humans, lactase activity declines after the weaning phase. In other healthy humans, lactase activity persists at a high level throughout adult life, enabling them to digest lactose as adults. This dominantly inherited genetic trait is known as lactase persistence. The distribution of these different lactase phenotypes in human populations is highly variable and is controlled by…

The term lactase deficiency is widely used to indicate a low or absent level of lactase enzyme in the small intestine, leading to lactose intolerance. This term is correctly used when the intestinal mucosa is damaged and results in secondary lactase deficiency. In the case of the genetically determined decrease of lactase activity during childhood, however, low lactase levels suggest that the majority of the world's population is "abnormal," whereas individuals from caucasian extraction with high levels of lactase enzyme throughout life are then considered "normal." It would be better to ascri…

Sixty patients having schistosomiasis mansoni infection were studied for lactase deficiency by different methods including lactase assay in intestinal biopsies. Thirty patients suffering from simple intestinal (Group I) and 30 patients suffering from hepatointestinal (Group II) schistosomiasis were compared to 60 controls (Group III). Lactase deficiency was evaluated by symptoms of lactose intolerance, stool pH and osmolarity, oral lactose tolerance test (OLLT), oral lactose hydrogen breath test (OLHBT) and lactase activity (LA) in small intestinal biopsies. OLTT showed intolerance in 36.7% of…

The intestinal brush-border enzyme lactase splits lactose into its component monosaccharides, glucose and galactose. Relative deficiency of the enzyme during adulthood is a common condition worldwide and is frequently associated with symptoms of lactose intolerance. We studied the synthesis and processing of lactase in normal and adult hypolactasic subjects using human intestinal explants in organ culture. Metabolic labeling experiments in our control subjects with [35S]methionine followed by immunoprecipitation, sodium dodecyl sulfate-polyacrylamide-gel electrophoresis, and fluorography demon…

The results of previous investigations of lactase deficiency and lactose malabsorption are reviewed. It showed that lactase activity and its decline in animals and humans is controlled genetically, but also that its phenotypic expression as lactose malabsorption is influenced by nongenetic factors: adaptation, biological (circadian) rhythmicity, hormones, gastrointestinal functions, and nutritional components can alter the response to lactose intake.
