Developmental language learning impairments.

Developmental language learning impairments (LLI) are one of the most prevalent of all developmental disabilities, can occur in children for a wide variety of reasons, and have been shown to co-occur frequently with other developmental social, emotional and behavioral disorders, as well as with academic achievement problems. Research pertaining to developmental LLI of unknown origin, with an emphasis on the continuum between oral and written language impairment, is the focus of this review. Given the complexity of language learning, research has focused on multiple levels of analysis, includin…

[Language in autistic disorders].

Autism is a developmental disorder affecting social relationships, communication and flexibility of thought. These three basic aspects of autism may present in many different forms and degrees. Therefore autism should be considered to be a spectrum of autistic disorders rather than a single strictly defined condition. The spectrum of autistic disorders extends from intelligent individuals with acceptable social integration, to severely retarded patients with scarcely any social interaction. Language is almost always affected either in its formal aspects or in its usage. Autistic linguistic dis…

Understanding childhood language disorders.

Developmental language disorders exist in 5% to 10% of preschoolers and have strong genetic implications. There are several variants of dysphasia: mixed receptive/expressive, expressive, or higher order language processing. Preschool children with pervasive developmental disorders are dysphasic as well as autistic. Some undergo a language and behavioral regression, most often as toddlers. The role of subclinical epilepsy in this regression is unknown because it is often ignored. Most dysphasic children speak by schoolage but are at substantial risk for reading/academic difficulty. Powerful new…

Language-apparent reflex epilepsy.

An adolescent with photosensitive epilepsy had absence seizures reproduced by forced voluntary eye closure in light or in darkness, independent of eyelid immobilization or ocular elevation. Attacks preferably occurred while speaking Spanish, her second language, mediated through brief, strong, automatic closing of the eyes during difficult word retrieval. Complex other multidirectional ocular movements or blinking, cognitive and language tasks were ineffective. Frontorolandic cortical epileptogenesis is seemingly responsible for light-provoked seizures in Papio papio, acting via hyperexcitable…

Somatization, paranoia, and language.

Somatization and paranoia are circumscribed distortions of reality that are impervious to the normative process of consensual validation. These distortions are often postulated as a means of bolstering lowered self-esteem. We used computerized content analysis of the free speech of patients with these disorders in order to identify and compare dimensions of self-concept reflected in their lexical choices. Interestingly, patients with these disorders differed in the themes prominent in their speech. The higher frequency categories used by the somatization disorder group conveyed an overwhelming

Language training for generalization.

This paper presents a classroom-based language intervention program for preschool children with language delays or deficits. A major goal of the program is to ensure that children acquire missing skills through daily speech therapy and that they actively generalize these skills from therapy times to other classroom activities. This paper presents data collected with two children during daily speech therapy sessions and daily play activities. Instances in which the children generalized new language acquired in therapy to play activities will be discussed as well as instances in which the childr…

"Language construction in an autistic child: thoughts regarding language acquisition and language therapy": translation, update, and commentary on a 1977 case report.

A 1977 Swiss case study is presented in English translation: a mute child with infantile autism is taught to speak starting at the relatively late age of 6. The author, who is the primary therapist and the child's father, details the conditioning procedure, discusses theoretical considerations in speech acquisition, and outlines the limits of the training. The author and translator update the child's status and add commentary.

The language and culture of delay.

Satish is a 3 (1/2)-year-old boy you are seeing in your primary care office for a "sick visit" due to parental concerns about his language development. He is the only child of a couple who immigrated to the United States from India shortly before his birth. He received early intervention services for speech and language delays for a few months before he attained 2 years of age. However, services were discontinued when the family moved back to India for a year. After the family returned to the United States, they lived in a different state for several months before moving again recently to his …

Language and the infant brain.

Three logically and empirically independent issues are often conflated in theory and research on brain and language: localization, innateness, and domain specificity. Research on adults and infants with focal brain injury support the following conclusions: (a) linguistic knowledge is not innate, and it is not localized in a clear and compact from in either the infant or adult brain; (b) the infant brain is not, however, a tabala rasa-it is already highly differentiated at birth, and certain regions are biased from the beginning toward modes of information processing that are particularly usefu…

Speech Language Pathology Rehabilitation.

The speech language pathologist has a vital management role in patients with voice and swallow concerns, as well as stroke patients and patients with fluency problems. This article summarizes the variety of speech and swallow rehabilitation that adult patients may require or seek. The case examples allow the reader to base the clinical decision-making process within the context of a patient presentation and elucidate the role of speech and language pathology services for the primary care provider in order to refer patients with symptoms and concerns to the right provider early in their medical…

Rethinking language in autism.

In this article, we invite a rethinking of traditional perspectives of language in autism. We advocate a theoretical reappraisal that offers a corrective to the dominant and largely tacitly held view that language, in its essence, is a referential system and a reflection of the individual's cognition. Drawing on scholarship in Conversation Analysis and linguistic anthropology, we present a multidimensional view of language, showing how it also functions as interactional accomplishment, social action, and mode of experience. From such a multidimensional perspective, we revisit data presented by…

Aging in language dynamics.

Human languages evolve continuously, and a puzzling problem is how to reconcile the apparent robustness of most of the deep linguistic structures we use with the evidence that they undergo possibly slow, yet ceaseless, changes. Is the state in which we observe languages today closer to what would be a dynamical attractor with statistically stationary properties or rather closer to a non-steady state slowly evolving in time? Here we address this question in the framework of the emergence of shared linguistic categories in a population of individuals interacting through language games. The obser…

Neuropsychological evaluation of language.

Neuropsychological evaluation of language may be based on the psychometric approach or on extensive exploration of modular functions according to cognitive or linguistic theory. Various approaches have several features in common, although some emphasize individual functions, and others explore syndromes in brain-damaged populations. Systematic assessment of language has been developed according to the goals of various clinicians and researchers. Beyond the diagnostic need, prognosis, and planning for therapy, there are important theoretical reasons to have standardized aphasia tests and comple…
